[Entomological survey in the historical focus of human African trypanosomiasis of Bendje (Gabon)].

The situation of human African trypanosomiasis (sleeping sickness) is poorly known in Gabon. Most of the historical foci have not been investigated for more than 15 years. Few cases are passively recorded from the historical focus of Bendjé; they involved mainly fishermen but determining their contamination site is difficult because of their mobility due to their activity. The presence of these cases in that focus could favour its reactivation if the vector is still there. In order to assess a potential transmission risk in that area, an entomological survey has been carried out in it. Traps w…

The central role of macrophages in trypanosomiasis-associated anemia: rationale for therapeutical approaches.

Bovine African trypanosomiasis causes severe economical problems on the African continent and one of the most prominent immunopathological parameters associated with this parasitic infection is anemia. In this report we review the current knowledge of the mechanisms underlying trypanosomiasis-associated anemia. In first instance, the central role of macrophages and particularly their activation state in determining the outcome of the disease (i.e. trypanosusceptibility versus trypanotolerance) will be discussed. In essence, while persistence of classically activated macrophages (M1) contribute…

Cross-resistance to nitro drugs and implications for treatment of human African trypanosomiasis.

The success of nifurtimox-eflornithine combination therapy (NECT) for the treatment of human African trypanosomiasis (HAT) has renewed interest in the potential of nitro drugs as chemotherapeutics. In order to study the implications of the more widespread use of nitro drugs against these parasites, we examined the in vivo and in vitro resistance potentials of nifurtimox and fexinidazole and its metabolites. Following selection in vitro by exposure to increasing concentrations of nifurtimox, Trypanosoma brucei brucei nifurtimox-resistant clones designated NfxR1 and NfxR2 were generated. Both ce…

Entomological patterns in the human African trypanosomiasis focus of Komo Mondah, Gabon.

BACKGROUND: The incidence of sleeping sickness is still considerable in the Komo Mondah focus, in spite of case-detection strategy. A combined strategy that associated both mass screening and vector control is effective for the control of the disease. In the perspective of a targeted vector control in main transmission sites, we have carried out an entomological survey in the epicentre of the focus. OBJECTIVES: To determine tsetse flies distribution, human-fly contact point and eventually risk factors for acquisition of the disease. METHODS: "Vavoua" traps were set for Glossina in four bioto

The epidemiology of trypanosomiasis in Rumphi district, Malawi: a ten year retrospective study.

BACKGROUND: Human African Trypanosomiasis (HAT) is caused by two species of the tsetse fly vectored protozoan hemoflagellates belonging to Trypanosma brucei, namely T.b gambiense which predominates in Western Africa and follows a chronic disease course and T.b rhodensiense which is more prevalent in Southern and Eastern Africa, Malawi included, and follows a more acute and aggressive disease course. Previous studies in the Democratic Republic of Congo, Angola, Uganda and Sudan have demonstrated that the prevalence rates of T.b rhodensiense infection have reached epidemic proportions. OBJECTIV…

Chemical validation of trypanothione synthetase: a potential drug target for human trypanosomiasis.

In the search for new therapeutics for the treatment of human African trypanosomiasis, many potential drug targets in Trypanosoma brucei have been validated by genetic means, but very few have been chemically validated. Trypanothione synthetase (TryS; EC 6.3.1.9; spermidine/glutathionylspermidine:glutathione ligase (ADP-forming)) is one such target. To identify novel inhibitors of T. brucei TryS, we developed an in vitro enzyme assay, which was amenable to high throughput screening. The subsequent screen of a diverse compound library resulted in the identification of three novel series of TryS…

[Criteria for diagnosis of the neurological stage of human African trypanosomiasis: update and perspectives].

Sleeping sickness or human African trypanosomiasis (HAT) is due to parasite infection by a sanguicolous flagellate protozoan of the Trypanosoma brucei genus. The disease is classically divided into two stages, i.e., the hemolymphatic stage and the CNS stage. Disease staging is currently a major challenge for therapeutic decision-making. In the field, diagnosis is based solely on white blood cell (WBC) count and detection of the parasite in the patient's cerebrospinal fluid (CSF). This technique is unreliable and invasive. Numerous studies are now under way to adapt staging to field conditions …

Experimental therapy of African trypanosomiasis with a nanobody-conjugated human trypanolytic factor.

High systemic drug toxicity and increasing prevalence of drug resistance hampers efficient treatment of human African trypanosomiasis (HAT). Hence, development of new highly specific trypanocidal drugs is necessary. Normal human serum (NHS) contains apolipoprotein L-I (apoL-I), which lyses African trypanosomes except resistant forms such as Trypanosoma brucei rhodesiense. T. b. rhodesiense expresses the apoL-I-neutralizing serum resistance-associated (SRA) protein, endowing this parasite with the ability to infect humans and cause HAT. A truncated apoL-I (Tr-apoL-I) has been engineered by dele…

Association between human African trypanosomiasis and the IL6 gene in a Congolese population.

Despite the importance of behavioural and environmental risk factors, there are arguments consistent with the existence of a genetic susceptibility to human African trypanosomiasis (HAT). A candidate gene association study was conducted in the Democratic Republic of Congo using a family-based sample which included a total of 353 subjects (86 trios; one case and parents (n=258) and 23 families with more than one case and parents (n=95)). Polymorphisms located on the IL1alpha, IL4, IL6, IL8, IL10, TNFalpha and IFNgamma genes were genotyped after re-sequencing of the genes for extensive SNP searc…

[Human African trypanosomiasis: urban transmission in the focus of Bonon (Côte d'Ivoire)].

Human African trypanosomiasis (HAT) is a vector-borne parasitic disease which has often been considered a rural disease. Population increases in African countries have entailed the spread of urban centres, creating favourable conditions for the appearance of new epidemiological conditions. In Cote d'Ivoire, HAT transmission has been described in the surroundings of towns such as Daloa or Sinfra. In the focus of Bonon, located in central-western Cote d'Ivoire, a medical survey detected 96 patients. The sites visited by the patients every day were geo-referenced and the routes between them recor…

Clinical assessment of the entry into neurological state in rat experimental African trypanosomiasis.

Human African trypanosomiasis, caused by Trypanosoma brucei (T.b.) gambiense or rhodesiense, evolves in two stages: haemolymphatic stage and meningo-encephalitic stages, the latter featuring numerous neurological disorders. In experimental models infected with diverse T.b. sub-species, body weight (BW) loss, drop in food intake (FI), and hypo-activity after an asymptomatic period suggest the occurrence of a similar two-stage organization. In addition to daily measurement of BW and FI, body core temperature (T(co)) and spontaneous activity (SA) were recorded by telemetry in T.b. brucei-infected…

Treatment of African trypanosomiasis with cordycepin and adenosine deaminase inhibitors in a mouse model.

There is an urgent need to discontinue the use of highly toxic compounds still in use for treatment of the encephalitic stage of human African trypanosomiasis (HAT). We show here that intraperitoneal injection of the adenosine analogue cordycepin (3'-deoxyadenosine), together with an adenosine deaminase (ADA) inhibitor (coformycin or deoxycoformycin), cures Trypanosoma brucei brucei infection in mice. Treatment was also effective at a stage when the trypanosomes had penetrated into the brain parenchyma, as determined by double immunolabeling of parasites and cerebral vessel endothelial cells i…

[Distribution and spread of human African trypanosomiasis: value of genetic identification of the trypanosomes].

Numerous factors extrinsic to trypanosome populations have been implicated in the distribution and spread of human African trypanosomiasis (HAT), but quantification of these factors has proven difficult. An easier method of monitoring HAT consists of tracking parasites by genetic identification of trypanosomes in hosts and vectors. This method requires distinction between Trypanosoma brucei rhodesiense and Trypanosoma brucei gambiense followed by determination of the genotype of each subspecies in the host and vector. The role of vertebrate hosts in the distribution and spread of HAT has been …

Chemokines are produced in the brain early during the course of experimental African trypanosomiasis.

African trypanosomiasis is characterized by progressive central nervous system (CNS) involvement. Using single and double immunohistochemistry, we evaluated the induction of alpha- and beta-chemokines in brains of Sprague-Dawley rats infected with Trypanosoma brucei brucei (T. b. brucei) and identified their cellular source. The results showed high production of MIP-2, RANTES and MIP-1alpha and to a lower extend MCP-1 in infected animals compared to controls. MIP-2, RANTES and MIP-1alpha were produced early by astrocytes and microglia and later by macrophages and T-cells. These findings sugges…

Evidence for genomic imprinting of the major QTL controlling susceptibility to trypanosomiasis in mice.

Inbred strains of laboratory mice exhibit marked differences in survival time following infection with Trypanosoma congolense, the principal cause of trypanosomiasis in African livestock. The difference in survival time between the relatively resistant C57BL/6 J and more susceptible BALB/c inbred strains has been attributed to three quantitative trait loci (QTLs), Tir1, Tir2 and Tir3. In order to determine whether there was a parent-of-origin effect on this trait, four backcross populations derived from the C57BL/6 J and BALB/c parental strains were bred and inoculated with T. congolense. The …

Use of a PCR assay for diagnosing African trypanosomiasis of the CNS: a case report.

Источник пометил публикацию как отозванную

The diagnosis of African trypanosomiasis is parasitologic and often can be difficult, especially in patients infected with Trypanosoma brucei gambiense, the cause of West African sleeping sickness. In the United States imported cases of sleeping sickness are rare, and most occur in tourists returning from East African game parks rather than among immigrants. I report here the use of a T. brucei specific PCR assay in a West African immigrant who presented with neurological symptoms more than 12 years after he had last been in Africa. The patient's historical and physical findings, as well as ab…

[Contribution of biochemical tests in the diagnosis of the nervous phase of human African trypanosomiasis].

The stage of human African trypanosomiasis (HAT) is important to define precisely as far as it is directly related to the type of treatment used. The beginning of the neurological involvement is difficult to find out because there is no known specific clinical or biological sign. This study is trying to look for a precise marker and has been realized in Congo. 70 subjects with parasitologically confirmed HAT and 70 controls are included. The stage of HAT is determined according to the classical definition on the field using the cerebrospinal fluid (CSF) cell count: less than 5 cells/microliter…

The role of the polyamine inhibitor eflornithine in the neuropathogenesis of experimental murine African trypanosomiasis.

The treatment of late-stage human African trypanosomiasis is complicated by a post-treatment reactive encephalopathy, also referred to as a 'reactive arsenical encephalopathy', that may be fatal. This study used a well established experimental mouse system to assess the use of the trypanostatic drug, eflornithine, in the management of this post-treatment reaction. Female CD-1 mice infected with an eflornithine-resistant trypanosome stabilate and treated with the trypanocidal compound diminazene aceturate on or after day 21 post-infection develop a reactive encephalopathy and relapsing parasita…
