New Delhi metallo-beta-lactamase: a weapon for the newly emerging drug-resistant bacteria.

The world has seen the emergence of many micro-organisms in the recent past, which can curb human population with their newly built genetic make-up. The latest addition to this list of panic creating organisms is, bacteria encoding the gene for New Delhi metallo-beta-lactamase (NDM-1). NDM-1 is an enzyme that can hydrolyze and inactivate carbapenems, which are used as a last resort for the treatment of multi-resistant bacterial infections. Names of these bacteria were not found in the medical literature before December 2009, because of which it can take the credit of becoming a powerful emergi

[Epidemiology of patients harboring extended-spectrum beta-lactamase-producing enterobacteriaceae (ESBLE), on admission].

OBJECTIVE: The incidence of extended-spectrum beta-lactamase-producing enterobacteria (ESBLE) has regularly increased over the last few years. However, little is known about epidemiology of ESBLE carriers in France. The objective of this study was to determine the ESBLE carriers or infected patients profile, identified within 48 hours following hospital admission. DESIGN: This retrospective study included all patients admitted in 2006 and 2007 at the Necker-Enfants-Malades (NEM) teaching hospital, carrying or infected with ESBLE isolated within 48 hours following admission. The pediatric and

Metallic nanoparticles bioassay for Enterobacter cloacae P99 beta-lactamase activity and inhibitor screening.

This article presents a simple colorimetric assay for Enterobacter cloacae P99 beta-lactamase activity detection and its inhibitors screening on the basis of silver and gold nanoparticles aggregation. In the presence of E. cloacae P99 beta-lactamase, the beta-lactam ring in the cephalosporin substrate was opened and resulted in releasing the active sites-modified linker which induced significant aggregation of silver or gold nanoparticles based on the electrostatic interactions and metal-thiols conjugation between the flexible linker and citrates on the surfaces of silver and gold nanoparticle

RAHN-2, a chromosomal extended-spectrum class A beta-lactamase from Rahnella aquatilis.

OBJECTIVES: Rahnella aquatilis is an environmental enterobacterial species with a chromosomal bla(RAHN-1) gene encoding extended-spectrum class A beta-lactamase RAHN-1. We describe the diversity of bla(RAHN) genes from two groups of strains, G1 and G2, isolated from raw fruits and vegetables, and the new class A beta-lactamase RAHN-2. METHODS: MICs were determined by Etest. bla(RAHN) genes were amplified by PCR, sequenced, and cloned to produce RAHN-1 and RAHN-2 proteins whose kinetic parameters were determined. RESULTS: All strains had similar beta-lactam resistance patterns. However, isola

Extended-spectrum beta-lactamase-producing pathogens in a children's hospital: a 5-year experience.

BACKGROUND: Pediatric infection with bacteria producing extended-spectrum beta-lactamases (ESBLs) has not been well described. We sought to determine the proportion of isolates producing ESBLs and the incidence of infection or colonization with these organisms in our tertiary care pediatric facility over 5 years. In addition, we sought to evaluate the characteristics of children affected. METHODS: We identified all Escherichia coli or Klebsiella spp cultured from children younger than 18 years of age at our facility between January 2003 and December 2007. Medical records were reviewed for aff…

Surveillance of extended-spectrum-beta-lactamase-producing Enterobacteriaceae in a Swiss Tertiary Care Hospital.

BACKGROUND: Extended spectrum beta-lactamase producing enterobacteriaceae (ESBL-E) are increasing worldwide, but there is sparse data on patient-to-patient transmission and the prevalence among risk groups in Switzerland. A prospective, observational cohort study was performed to: 1) assess the prevalence of ESBL-E at admission among at-risk groups; 2) evaluate nosocomial cross-transmission in acute care (ACF) versus long-term care facilities (LTCF); and 3) evaluate prevalent mutations of the detected beta-lactamase genes. METHODS: Predefined risk groups were screened either on admission or a

Purification, crystallization and preliminary crystallographic analysis of Est-Y29: a novel oligomeric beta-lactamase.

beta-Lactam antibiotics such as penicillins and cephalosporins have a four-atom ring as a common element in their structure. The beta-lactamases, which catalyze the inactivation of these antibiotics, are of great interest because of their high incidence in pathogenic bacteria. A novel oligomeric class C beta-lactamase (Est-Y29) from a metagenomic library was expressed, purified and crystallized. The recombinant protein was expressed in Escherichia coli with an N-terminal 6xHis tag and purified to homogeneity. EstY-29 was crystallized and X-ray intensity data were collected to 1.49 A resolution…

OXA-143, a novel carbapenem-hydrolyzing class D beta-lactamase in Acinetobacter baumannii.

A carbapenem-resistant Acinetobacter baumannii strain was isolated in Brazil in 2004 in which no known carbapenemase gene was detected by PCR. Cloning experiments, followed by expression in Escherichia coli, gave an E. coli recombinant strain expressing a novel carbapenem-hydrolyzing class D beta-lactamase (CHDL). OXA-143 showed 88% amino acid sequence identity with OXA-40, 63% identity with OXA-23, and 52% identity with OXA-58. It hydrolyzed penicillins, oxacillin, meropenem, and imipenem but not expanded-spectrum cephalosporins. The bla(OXA-143) gene was located on a ca. 30-kb plasmid. After

Prevalence and characterization of extended-spectrum beta-lactamase-producing Enterobacteriaceae isolated in Korean hospitals.

Prevalence and characteristics of extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae in Korean hospitals were assessed. A total of 1484 clinical Enterobacteriaceae isolates were collected from 8 tertiary-care hospitals in various regions of Korea over a 3-month period (June to August) in 2005. Among 546 Klebsiella pneumoniae isolates, 123 isolates (22.4%) showed ESBL-producing activity, and 47 (10.2%) of 460 isolates of Escherichia coli were ESBL producers. Of the Enterobacter cloacae isolates, 16.2% (17/105) evidenced ESBL-producing activity. The most prevalent ESBLs were SH

[Dissemination of IMP-1 and OXA type beta-lactamase in carbapenem-resistant Acinetobacter baumannii].

BACKGROUND: Acinetobacter baumannii is an aerobic, gram-negative, glucose-nonfermenting bacterium, which has emerged as a serious opportunistic pathogen. In recent years, the increasing instance of carbapenem-resistant A. baumannii producing metallo-beta-lactamases (MBLs) or OXAtype beta-lactamases is causing a serious clinical problem. In this study, we investigated the prevalence of Ambler class A, B, and D beta-lactamases and their extended-spectrum derivatives in carbapenem-resistant A. baumannii isolates. METHODS: A total of 31 consecutive, non-duplicate, carbapenem-resistant A. baumanni…

Evaluation of four commercially available extended-spectrum beta-lactamase phenotypic confirmation tests.

Extended-spectrum beta-lactamase (ESBL) production in members of the Enterobacteriaceae can confer resistance to extended-spectrum cephalosporins, aztreonam, and penicillin. As such, the accurate detection of ESBL producers is essential for the appropriate selection of antibiotic therapy. Twenty previously characterized isolates and 49 clinical isolates suspected of ESBL production were tested by four ESBL phenotypic confirmatory methods for accuracy and ease of use. The four ESBL phenotypic confirmation tests included Dried MicroScan ESBL plus ESBL Confirmation panels (Dade Behring, Inc., Wes

Dissemination of CTX-M-type extended-spectrum beta-lactamase genes to unusual hosts.

A Citrobacter amalonaticus and a Morganella morganii producing the CTX-M-1 extended-spectrum beta-lactamase (ESBL) were isolated from an area where this enzyme is now widespread in Escherichia coli. This is the first report of CTX-M-1 in the former species. In both cases the ESBL determinant was possibly acquired by these unusual hosts in vivo, after coinfection with E. coli strains carrying conjugative plasmids encoding CTX-M-1.
