This report concerns three children with His bundle tachycardia who died following cardiac surgery. At autopsy the conduction system was examined in detail. In all three, the sinus node was intact and supplied by a well-formed artery. Haemorrhagic tracks were identified invading the penetrating atrioventricular bundle. The tracks originated from stitches placed close to the conduction tissue. The hypothesis that disruption of the conduction tissue results in an arrhythmogenic focus is discussed.

This report describes a case of severe bradyarrhythmia during anesthetic induction with propofol, midazolam, sufentanil, and vecuronium. The patient took nifedipine sustained-release tablets on the morning of surgery as routine treatment for hypertension, and this medication may have contributed to the bradyarrhythmia. Ictal asystole due to sinus node suppression is a cause of sudden unexplained death in epilepsy. Here, for the first time, we describe a complete atrioventricular nodal block in a patient with non-compressive traumatic subdural hematoma, who developed ictal asystole as a delayed presentation. A leadless VVI pacemaker (ventricular paced, ventricular sensed, and pacing inhibited in response to a sensed beat) was implanted as a preventive measure against seizure-related heart block.

The prevalence of cardiac complications of dermatomyositis and polymyositis is generally underestimated. The authors report the case of heart block as the presenting symptom of the disease, situated in the atrioventricular node. Initially paroxysmal, it eventually became permanent. It was associated with atrial (fibrillation and flutter) and ventricular hyperexcitability (ventricular tachycardia in runs). Myocardial biopsy provided histological proof of the cardiac disease and the definitive link between the nodal conduction defect and the polymyositis.

We describe the results of surgical dissection of multiple accessory pathways in two brothers. A significant family history of pre-excitation syndrome was present: two children died suddenly and at the autopsy of one of them an accessory atrioventricular connection was found.
