Regression of nifedipine-induced gingival hyperplasia following switch to a same class calcium channel blocker, isradipine.

Patients with nifedipine-induced gingival hyperplasia (GH) often require continued calcium channel blocker therapy. Switches to diltiazem and verapamil have been described; however, these drugs are of a different chemical class and present therapeutic limitations in some patients. The purpose of this study was to evaluate the effect on nifedipine-induced GH of a switch to a dihydropyridine derivative with a low incidence of GH. Fourteen patients with nifedipine-induced GH were given a medical exam and a periodontal exam. The following parameters were assessed: probing depth (PD), gingival marg

[Evaluation of trandolapril alone or in combination with a calcium channel blocker in hypertensive patients over 60 years of age].

The efficacy and safety of trandolapril alone and in combination with a calcium channel blocker were evaluated in 13,147 hypertensive patients over 60 years old. Two patient groups were constituted. After a 2-week wash-out period, the patients in group I received monotherapy with trandolapril 2 mg/day for 4 weeks. Trandolapril was continued for another 4 weeks in responding patient, otherwise the dosage of trandolapril was doubled or another antihypertensive was added. Group 2, composed of patients previously treated with a calcium channel blocker with insufficient efficacy, was treated accord

The efficacy and safety of triple vs dual combination of angiotensin II receptor blocker and calcium channel blocker and diuretic: a systematic review and meta-analysis.

Many hypertensive patients require ≥2 drugs to achieve blood pressure targets. This study aims to review and analyze the clinical studies conducted with dual or triple combination of angiotensin II receptor blockers (ARBs), calcium channel blockers (CCBs), and diuretics. Medical literature between January 1990 and April 2012 was reviewed systematically and data from eligible studies were abstracted. Data were analyzed using random-effects models. Of the 224 studies screened, 7563 eligible patients from 11 studies were included. Triple combinations of ARBs (olmesartan or valsartan), CCBs (amlod…

Implementation of a clinical decision support alert to prevent use of intravenous β-blockers and nondihydropyridine calcium channel blockers in hospitalized patients with acute decompensated heart failure.

PURPOSE: Intravenous (IV) β-blockers (BBs) and nondihydropyridine calcium channel blockers (NDCCBs) are harmful in patients with acute decompensated heart failure (ADHF), but they are commonly used for rate control in atrial fibrillation (AF). This study evaluated the implementation of a clinical decision support (CDS) alert in the electronic health record (EHR) to prevent the use of these agents for AF in patients with ADHF, as well as results from the alert's continuous quality improvement. METHODS: This was a single-center, retrospective, quasi-experimental pre/post analysis of hospitalize…

Proposed echocardiography-guided flowchart for calcium channel blocker overdose: distinguishing vasoplegia from cardiogenic shock.

BACKGROUND: Calcium channel blocker (CCB) overdose can present as vasoplegic or cardiogenic shock, but current guidance does not operationalize bedside echocardiography to direct therapy. OBJECTIVES: We propose an echocardiography-centered, phenotype-driven flowchart to standardize decisions. METHODS: Our proposed algorithm (1) prioritizes standard airway, breathing and circulatory management and an initial mean arterial blood pressure (MAP) target ≥ 65 mmHg, with higher targets (for example, 75–80 mmHg) considered in selected patients such as those with chronic hypertension; (2) embeds immedi

Risk of adverse effects of the concomitant use of statins with calcium channel blockers: a systematic review and meta-analysis.

Calcium channel blockers (CCBs) and statins are commonly combined to prevent cardiovascular diseases (CVDs). However, their concomitant use may lead to drug-drug interactions (DDIs) and increased adverse effects (AEs). This systematic review and meta-analysis aimed to evaluate the evidence of adverse effects (AEs) associated with the calcium channel blockers (CCBs) and statins concomitant use. We searched MEDLINE, Embase, Web of Science, and CENTRAL from database inception to 4 April 2025. Eligible studies included randomized controlled trials (RCTs) and observational studies reporting AEs of …

Cilnidipine: An L- and N-Type Blocker of Calcium Channels Ameliorating Renal Damage in Experimental Hypertensive Rats.

BACKGROUND: Cilnidipine is both an L-type and N-type calcium channel blocker (CCB). Cilnidipine suppresses hyperactivity in the renin-angiotensin system and sympathetic nervous system by blocking N-type calcium channels. We hypothesized that through its N-type calcium channel blockade, cilnidipine may reduce proteinuria and improve creatinine clearance in hypertensive rats.  Aim of the study: The current study was done to show the renal protective effect of cilnidipine on chronic hypertensive rats. Our primary objective is to develop a hypertensive rat model by giving L-NAME (NG-nitro-L-Argini

Hyperglycemia and Severe Medical Outcomes in Calcium Channel Blocker Exposures Reported to United States Poison Centers.

INTRODUCTION: Calcium channel blockers (CCBs) antagonize L-type calcium channels, primarily in the cardiovascular system. In overdose, the pancreas also is affected, which prevents the release of insulin leading to hyperglycemia. We sought to determine the incidence of hyperglycemia in CCB exposures reported to US poison centers and to compare the incidence of hyperglycemia with severe medical outcomes. METHODS: We performed a retrospective chart review of CCB exposures using data from the National Poison Database System from January 1, 2007, through December 31, 2017. Exposures with co-inges…

Antiseizure potential of the triple T-type calcium channel blocker ACT-709478 (apinocaltamide) in rodent models.

OBJECTIVE: T-type Ca2+ channels in the brain contribute to the generation of spike-and-wave discharges (SWDs). Absence seizures are defined by the occurrence of SWDs and are associated with impairment of consciousness. ACT-709478 is a selective and highly potent blocker of the low-voltage-activated T-type Ca2+ channels, Cav3.1, Cav3.2, and Cav3.3, with no relevant activity on other targets. Our aim was to investigate the efficacy of ACT-709478 in reducing absence-like and other types of seizures in rodents. METHODS: We studied the effect of oral ACT-709478 on SWDs in two models of absence-lik

Calcium channel blocker use and outcomes in patients with heart failure and mildly reduced and preserved ejection fraction.

AIMS: Patients with heart failure (HF) and mildly reduced ejection fraction (HFmrEF) and preserved ejection fraction (HFpEF) are often treated with calcium channel blockers (CCBs), although the safety of CCBs in these patients is uncertain. We aimed to investigate the association between CCB use and clinical outcomes in patients with HFmrEF/HFpEF; CCBs were examined overall, as well as by subtype (dihydropyridine and non-dihydropyridine). METHODS AND RESULTS: We pooled individual patient data from four large HFpEF/HFmrEF trials. The association between CCB use and outcomes was assessed. Among…

Association of a calcium channel blocker and diuretic prescribing cascade with adverse events: A population-based cohort study.

BACKGROUND: Prescribing cascades occur when a drug adverse event is misinterpreted as a new medical condition and a second, potentially unnecessary drug, is prescribed to treat the adverse event. The population-level consequences of prescribing cascades remain unknown. METHODS: This population-based cohort study used linked health administrative databases in Ontario, Canada. The study included community-dwelling adults, 66 years of age or older with hypertension and no history of heart failure (HF) or diuretic use in the prior year, newly dispensed a calcium channel blocker (CCB). Individuals

Spotted fever rickettsia-induced microvascular endothelial barrier dysfunction is delayed by the calcium channel blocker benidipine.

The tick-borne bacterium Rickettsia parkeri is an obligate intracellular pathogen that belongs to spotted fever group rickettsia (SFGR). The SFG pathogens are characterized by their ability to infect and rapidly proliferate inside host vascular endothelial cells that eventually result in impairment of vascular endothelium barrier functions. Benidipine, a wide range dihydropyridine calcium channel blocker, is used to prevent and treat cardiovascular diseases. In this study, we tested whether benidipine has protective effects against rickettsia-induced microvascular endothelial cell barrier dysf

Calcium Channel Blocker Use and the Risk for Breast Cancer: A Population-Based Nested Case-Control Study.

We investigated whether long-term exposure to calcium channel blockers (CCBs) is associated with an increased risk of breast cancer (BCa). We designed a nested case-control study based on data from the Clalit electronic database, the largest Israeli Health Services organization. All newly diagnosed breast cancer (BCa) cases were selected from a cohort of patients with hypertension. Ten controls were matched for each BCa case. The odds ratios (ORs) of BCa among CCBs users were calculated using multivariate conditional logistic regression analyses. A total of 4875 patients with newly diagnosed B…

Pulmonary arterial hypertension sensitive to calcium channel blocker, but not advanced pulmonary hypertension treatment: a case report.

BACKGROUND: Calcium channel blockers (CCB), the first accepted treatment, is effective only in a small number of idiopathic pulmonary arterial hypertension (I-PAH) patients with vasoreactivity [these patients are identified by a positive acute pulmonary vasoreactive test (AVT) response]. While the majority of I-PAH patients is non-vasoreactive and CCB non-responders, modern advanced pulmonary hypertension (PH)-specific therapies, which act on one of the three different mechanistic pathways-endothelin, nitric oxide (NO), and prostacyclin pathways, are effective. Treatment response to advanced P…

T-type calcium channels blockers inhibit HSV-2 infection at the late stage of genome replication.

Herpes simplex virus type 2 (HSV-2) is a highly contagious sexually transmitted virus. The increasing emergence of drug-resistant viral strains has highlighted the crucial need for the development of new anti-HSV-2 drugs with different mechanisms. Ion channels that govern a wide range of cellular functions represent attractive targets for viral manipulation. Here, we tried to identify novel compounds to suppress HSV-2 infection in vitro by screening a small library with ion channels modulators. We found that several T-type calcium channel blockers including benidipine, lercanidipine, lomerizin…

Calcium Channel Blocker Use and the Risk for Prostate Cancer: A Population-Based Nested Case-Control Study.

INTRODUCTION: Calcium channels play a significant role in the regulation of cell proliferation and apoptosis. This study investigates associations between calcium channel blocker (CCB) use and the incidence of prostate cancer (PCa). METHODS: A nested case-control study was conducted using the Clalit Health Services database. We formed a population-based cohort of patients who were prescribed their first antihypertensive agent between 2000 and 2014. For each newly diagnosed PCa case in the cohort, 10 controls were matched by age, calendar year of cohort entry, and duration of follow-up. Multiv

Case report on low dose of Cilnidipine: A fourth-generation calcium channel blocker-induced gingival overgrowth.

Drug-induced gingival overgrowth is a frequently observed adverse effect of antihypertensive drug calcium channel blockers (CCBs). Gingival enlargements are more common with the dihydropyridine class of CCBs. The fourth-generation dihydropyridine CCB Cilnidipine was introduced in 1995 and is used as an antihypertensive agent for patients in the Indian subcontinent. This is the first report in literature to document an isolated case of generalized gingival overgrowth induced by the long-term use of low dose of cilnidipine in an elderly female patient who is under antihypertensive therapy. Gingi…

Reduced Lymphatic Function Predisposes to Calcium Channel Blocker Edema: A Randomized Placebo-Controlled Clinical Trial.

Background: The current belief is that the calcium channel blocker (CCB)-induced edema is due to a preferential arterial over venous dilatation leading to increased fluid filtration. We challenged this conviction by measuring the lymphatic removal of interstitial fluid during chronic systemic treatment with the CCB, amlodipine. Lymphatic vessels could potentially be an off-target effect of the drugs and play a role in CCB edema. Methods and Results: Sixteen healthy postmenopausal women completed a 12-week double-blinded randomized placebo-controlled crossover trial. Lymphatic function was asse
