Free amino acid patterns of cerebrospinal fluid in infants and children with various types of convulsive disorders were compared with those … that each type of convulsive disorder shows the specific amino acid pattern, and the effects of anticonvulsants may be partially understood through the changes of the free amino acid patterns in the brain.

Status epilepticus and acute prolonged seizures are the most commonly occurring neurological emergencies in children. Such events have high morbidity and mortality rates along with poor long-term outcomes, depending on their duration and causes. Therefore, such seizures warrant urgent treatment using appropriate doses of anticonvulsants. Benzodiazepines, phenobarbital, and phenytoin are the most commonly used anticonvulsants for controlling status epilepticus and acute prolonged seizures. However, these medications have several well-known adverse effects. Previous studies on both adults and ch…

A questionnaire about convulsions and other adverse events after vaccination was sent to doctors who administered a diphtheria-pertussis-tetanus (DPT) vaccine (the first dose) or a measles vaccine between April 1, 1995 and December 31, 1997 in Takamatsu City to children with convulsions. DPT and measles vaccines were administered to 300 and 339 such children, respectively. Many of them had febrile seizures, the last of which had occurred before more than 1 year. Among them, 175 cases were administered with DPT and 180 with measles vaccine. There were recurrences of febrile convulsions after im

To elucidate the changes of free amino acid patterns in brains of El mice induced by several anticonvulsants, the free amino acid levels in brains were measured by an amino acid autoanalyzer 24 hours after intraperitoneal injection of PB, PHT or vitamin B6 or subcutaneous injection of ACTH. Compared to the preconvulsion group, the total free amino acid level increased in the ACTH group clearly. In the PB group, the levels of threonine, glutamine, lysine, histidine and homocarnosine increased and that of ornithine decreased; in the PHT group, an increase of glutamine and histidine and a decreas…

Extreme anaemia (haemoglobin <2g/dL) is rarely encountered, and the clinical manifestation of this entity remains unclear. A woman in her 60s who enjoyed good past health presented with a two-week history of generalized malaise and then a sudden onset of convulsive seizure. Blood tests revealed haemoglobin of 1.7g/dL with acute kidney injury and elevated liver enzymes and creatine kinase. She was subsequently diagnosed with myelodysplastic syndrome with erythroid hypoplasia. After receiving a supportive blood transfusion, she had significant improvement in both clinical and biochemical paramet

Seizure-like episodes may occasionally arise from cardiovascular causes rather than primary neurological disease, leading to diagnostic delay. We report an adolescent girl who presented with recurrent generalised tonic episodes with cyanosis and transient loss of consciousness. Initial evaluation for epilepsy, including video electroencephalography during an event, was unremarkable. Cardiac assessment revealed marked biatrial dilatation with restrictive ventricular physiology on echocardiography, consistent with restrictive cardiomyopathy (RCM). Ambulatory monitoring documented atrial tachycar

STUDY OBJECTIVE: To determine if ketamine, when added to midazolam for the treatment of out-of-hospital seizures, is associated with an increase in the rate of cessation of convulsions prior to hospital arrival.
METHODS: We performed a retrospective cohort study of out-of-hospital patients with an active convulsive seizure being transported to a hospital by a large emergency medical services system in Florida, using data from August 1, 2015 and August 5, 2024. Per protocol, patients received midazolam first for their seizure. Starting in June 2017, a new protocol was developed in which patien

Loss of consciousness ranks among very common causes for emergency medical service actions and is common occurrence in the emergency department. Its differential diagnosis is very broad and includes many possible causes, not in the least an intoxication. The same applies to convulsive states. Clinical course of mushroom poisoning varies depending on the particular fungal species, with some of the species causing loss of consciousness. One typical representative of such species is panther cap (Amanita pantherina). This case report introduces panther cap poisoning, initially presenting in given

We describe a 48-year-old man who experienced a sudden, pervasive change of mental status following a generalized tonic-clonic seizure. In the absence of typical epileptic phenomena, diagnosis of non convulsive status epilepticus (NCSE) was delayed. Diagnosis depends upon clinical presentation, EEG findings and, in difficult cases, the effects of a trial with antiepileptic drugs (AEDs). In this case report the diagnosis of NCSE was made ‘a posteriori’ based upon symptom remission and EEG normalization following introduction of AEDs. We discuss epidemiology, clinical manifestations, morbidity,

During physiological stress responses such as vigorous exercise, emotional states of fear and rage, and asphyxia, the nervous system induces a massive release of systemic catecholamines that prepares the body for survival by increasing cardiac output and redirecting blood flow from non-essential organs into the cardiopulmonary circulation. A curious byproduct of this vital response is a sudden, transient, and redistributive leukocytosis provoked mostly by the resultant shear forces exerted by rapid blood flow on marginated leukocytes. Generalized convulsive seizures, too, result in catecholami
