Treatment with daily oral cyclophosphamide (CY) has improved survival in Wegener's granulomatosis (WG), but is associated with severe and potentially lethal adverse effects. Less toxic treatment regimens, such as pulse CY, have been used, but the effect has been questioned. Weber-Christian disease is an uncommon disorder of unclear etiology whose unifying feature is fatty tissue inflammation. Its association with alpha 1-antitrypsin deficiency produces severe clinical manifestations. Although many agents have been used to treat Weber-Christian disease, current therapy is based on the poorly documented use of corticosteroids. We have reported the case of a young man with life-threatening Weber-Christian disease associated with alpha 1-antitrypsin deficiency unresponsive to corticosteroids who responded to cyclophosphamide.

Two patients with Wegener's granulomatosis, crescentic glomerulonephritis, and severe renal failure are described. Both patients required hemodialysis for control of uremic symptoms. Cyclophosphamide-prednisone therapy successfully controlled the extra-renal manifestations of their disease and reversed the renal failure so that dialysis could be discontinued.
