Revision Total Elbow Arthroplasty.

Despite recent technologic advances, total elbow arthroplasty has complication rates higher than that of total joint arthroplasty in other joints. With new antirheumatic treatments, the population receiving total elbow arthroplasty has shifted from patients with rheumatoid arthritis to those with posttraumatic arthritis, further compounding the high complication rate. The most common reasons for revision include infection, aseptic loosening, fracture, and component failure. Common mechanisms of total elbow arthroplasty failure include infection, aseptic loosening, fracture, component failure,

[Bipolar dislocation of the forearm].

The authors report the case of a 23 year old patient, who presented the following combination of injuries after a fall, namely a divergent dislocation of the elbow, a fracture of the radial head, a fracture of the ulna and a perilunate dislocation with scaphoid fracture. The entire injury thus represented a bipolar dislocation of the forearm. The emergency management consisted of a closed reduction of the elbow together with osteosynthesis of the ulna, internal fixation of the scaphoid with a compression screw, and scaphocapitate K wiring. After 26 months, the function of the elbow was satisfa

ARM: a modular hinged joint for the AO tubular external fixator.

Certain complex traumatic elbow lesions challenge the orthopaedic and trauma surgeon. If they are not treated correctly, they cause a high rate of disability, arising from elbow instablility and stiffness, either by fibrosis or joint incongruity. Injuries such as complex fractures of the proximal third of the ulna, coronoid fractures associated with radial head fractures (the "terrible triad"), are even worse if they are accompanied by soft tissue lesions. Hinged external fixators, complemented by other surgical procedures, are, for many, a recommended alternative when dealing with irreparable

Antegrade interlocking nailing of acute humeral shaft fractures.

Between 1992 and 1996, 73 acute humeral shaft fractures in 71 patients were treated with antegrade Russell-Taylor humeral nailing. Sixty (84%) patients sustained multiple trauma, and 26 (36%) fractures were open. Three patients had preoperative brachial artery injuries, and 12 had preoperative nerve palsies. Sixty-nine (94.5%) fractures united primarily, and two additional fractures united after bone grafting. There were no infections. Two (2.7%) iatrogenic radial nerve palsies occurred, and both were transient. Full shoulder function returned in 66 (90%) patients. Two (2.7%) patients had impi

Imaging of elbow replacement arthroplasty.

Elbow replacement arthroplasty has become a standard surgical treatment for a variety of diseases of the elbow. First popularized for the treatment of late-stage rheumatoid arthritis and other debilitating forms of joint disease, the current indications have expanded to include primary treatment of elbow trauma. The most commonly used total elbow replacements are linked semiconstrained chromium-molybdenum alloy or titanium alloy prostheses with polyethylene bearing surfaces. These are inserted after resection of the ulnotrochlear joint and typically cemented in place; the radial head is often

Paediatric fracture dislocation of the forearm: an unusual presentation.

Traumatic injuries of the elbow in children pose a diagnostic challenge. They demand precise knowledge of radiographic anatomy, growth characteristics of healthy and injured bones as well as specific fracture dynamics in this age group. Dislocation of the elbow is rare in children, accounting for 3-6% of all childhood elbow injuries. Fractures at the distal humerus and proximal forearm have been extensively discussed in the literature. We describe a unique case of a 4-year-old child presenting with a traumatic radial head fracture and ulnar dislocation not previously reported. Diagnosis and re

Results of non-operative treatment of olecranon fracture in over 75-year-olds.

INTRODUCTION: Surgery is the gold-standard treatment of displaced olecranon fracture, but is associated with numerous complications, especially in the elderly. Functional results of non-operative treatment in this population have never been analyzed in a prospective study. STUDY HYPOTHESIS: Non-operative treatment of isolated olecranon fracture with stable elbow-joint in over 75-year-olds gives functional results comparable to those of surgery as reported in the literature, with fewer complications. MATERIAL AND METHODS: A prospective study analyzed functional results of non-operative treatm

Bilateral radial neck fractures in a child: report of a case.

Fractures of the proximal radius are among the most common fractures and account for 9% to 25% of the elbow fractures in children. However, bilateral radial neck fractures are extremely rare injuries in children and have been reported only once before in present English literature. We report a case of bilateral radial neck fractures in a 10-year-old girl resulting from a fall on outstretched hands. Careful physical examination and proper radiographs are necessary for the diagnosis. Early recognition, proper management, and physical therapy led to complete recovery and full functional movement …

Fractures of the coronoid process: state of the art.

Coronoid fractures are rarely isolated and are much more frequently associated with other osseous or ligamentous structures injuries. On the basis of the coronoid fracture patterns, described by the O'Driscoll classification, it is possible to recognize three main patterns of injury that differ on traumatic mechanism and on associated lesions: posterolateral rotatory instability, posteromedial rotatory instability, and axial load injuries. The management of coronoid fractures is challenging and varies according to characteristics of the fracture, associated lesions, and amount of elbow instabi

Complex distal humeral fractures: internal fixation with a principle-based parallel-plate technique. Surgical technique.

BACKGROUND: Severe comminution, bone loss, and osteopenia at the site of a distal humeral fracture increase the risk of an unsatisfactory result, often secondary to inadequate fixation. The purpose of this study was to determine the outcome of treating these fractures with a principle-based technique that maximizes fixation in the articular fragments and stability at the supracondylar level. METHODS: Thirty-four consecutive complex distal humeral fractures were fixed with two parallel plates applied (medially and laterally) in approximately the sagittal plane. The technique was specifically d

Apparent capitellar fractures.

Isolated capitellar fractures are rare but are identified as such, even when they are more complex, because the displaced capitellar fracture is usually the most obvious and identifiable radiographic finding and because teaching has traditionally underemphasized the involvement of the trochlea in such fractures. The author prefers the term 'apparent capitellar fractures' and draws on his experience to explain why he favors three-dimensional CT for depicting fracture detail. This article discusses treatment options, emphasizing open reduction and internal fixation to restore the native elbow. O

[Traumatologic problems in old age (author's transl)].

In all fractures of the extremities survival has priority to local problems. The first thing to take care of is restitution of joint mobility, especially at the upper extremity (shoulder-and elbow joint). Early ambulation is facilitated by hanging cast and correctly indicated surgery, especially concerning humerus and elbow. At the lower extremities painless stability is more important than full mobilisation of the joints. Frequently a walking plaster cast is suited best for early ambulation and self support. Special problems are connected with fractures of the upper end of the femur. The incr

Olecranon Fractures: A Critical Analysis Review.

Surgical treatment of coronal shear fractures: short- to mid-term results and risk factor analysis.

BACKGROUND: Coronal shear fractures of the distal humerus are rare but severe injuries. Reconstruction is often challenging, especially in comminuted cases, which is why many surgeons opt for an elbow arthroplasty in those cases. However, total elbow arthroplasty is associated with a variety of potential problems itself. Therefore, the aim of this study was to present the functional and clinical outcome of coronal shear fractures treated by osteosynthetic reconstruction in a short- to mid-term follow-up, and to identify possible risk factors for an inferior outcome. METHODS: We performed a re
