Percuflex Helical Ureteral Stents Significantly Reduce Patient Analgesic Requirements Compared to Control Stents.

INTRODUCTION AND OBJECTIVES: Ureteral stent symptoms are experienced by the majority of patients with ureteral stents. No one stent or technology has shown a clear advantage. The Helical™ stent, a spirally cut stent made of proprietary Percuflex™ material, was designed to conform to the shape of the ureter and better accommodate patient movement. We prospectively sought to compare unscheduled visits, analgesic use, and pain scores in patients who received a Percuflex Helical ureteral stent following ureteroscopy compared to a historical control group. METHODS: Fifteen (n = 15) patients underg

Assessment of urology postgraduate trainees' competencies in flexible ureteroscopic stone extraction.

INTRODUCTION: We aimed to assess flexible ureteroscopic stone extraction skill of urology postgraduate trainees (PGTs) at an Objective Structured Clinical Examination (OSCE) and to determine whether previous experience in the operating theatre or practice on the simulator correlated with performance. METHODS: After obtaining ethics approval, PGTs from postgraduate years (PGYs) 3-5 were recruited from all four Quebec urology training programs during an OSCE. After a short orientation to the UroMentor™ simulator, PGTs were asked to perform Task 10 for 15 minutes, where two small stones from the

Using percutaneous transhepatic cholangioscopic lithotripsy for intrahepatic calculus in hostile abdomen.

BACKGROUND: Hepatolithiasis is a challenging condition to treat especially in patients with previous hepatobiliary surgery. Percutaneous Transhepatic Cholangioscopic Lithotripsy (PTCSL) is an attractive salvage option for the treatment of recurrent hepatolithiasis. We reviewed our experience using PTCSL in treating 4 patients with previous complex abdominal surgery. METHODS: We studied the 4 patients who underwent PTCSL from October 2007 to July 2009. We reviewed the operative procedures, workflow of performing PTCSL in our institution and the outcome of the procedure. PTCSL was performed in

Are Coronary Calcium-Modifying Techniques Levelling the Playfield?

Patients with heavily calcified coronary arteries represent a challenge in percutaneous coronary intervention (PCI), as severe calcification impairs device delivery and limits optimal stent expansion, leading to higher risks of stent thrombosis, restenosis, and adverse clinical outcomes. Approximately 20% of patients undergoing PCI exhibit severe coronary calcification, which independently predicts incomplete revascularization, increased mortality, and higher rates of major adverse cardiovascular events over mid-term follow-up. Recent advances have focused on improving the assessment and manag

A Systematic Review and Meta-Analysis Including 354 Patients from 13 Studies of Intravascular Lithotripsy for the Treatment of Underexpanded Coronary Stents.

Calcified coronary plaque (CCP) represents a challenging scenario for interventional cardiologists. Stent underexpansion (SU), often associated with CCP, can predispose to stent thrombosis and in-stent restenosis. To date, SU with heavily CCP can be addressed using very high-/high-pressure noncompliant balloons, off-label rotational atherectomy/orbital atherectomy, excimer laser atherectomy, and intravascular lithotripsy (IVL). In this meta-analysis, we investigated the success rate of IVL for the treatment of SU because of CCP. Studies and case-based experiences reporting on the use of IVL st

[The 1-cm lower calyx calculus: SWT, URS or Mini-PCNL? Guidelines vs. reality.].

Urinary stones of the upper urinary tract can be considered a widespread public health concern due to their high incidence and prevalence and their health policy-related and financial implications. A significant proportion of newly diagnosed kidney stones are lower-pole stones, i.e., stones affecting the lower calyx group of the renal pelvicalyceal system. These are often diagnosed by chance, i.e., as incidental findings during ultrasound or CT scans performed for other reasons, or as "secondary stones" detected during the diagnostic work-up of symptomatic urinary stones in other locations. Does the renal parenchymal thickness affect the efficacy of the retrograde intrarenal surgery? A prospective cohort study.

Retrograde intrarenal surgery (RIRS) is one of the minimally invasive main treatment modalities in renal stone disease. There are many factors which affect stone-free rate (SFR). Our study was based on the hypothesis that higher renal parenchymal thickness (RPT) which may include higher average number of nephrons provides better diuresis. We investigated the efficacy of RPT on success of RIRS. This study is a single-centered prospective surgical cohort study. A total of 383 patients were analyzed. Regularly followed 304 patients with unilateral kidney stone at single pole or renal pelvis and w

Emergency Management of Obstructing Ureteric Stones: A Closed-Loop Audit of Hot Ureteroscopy Uptake Against Getting It Right First Time (GIRFT) Standards.

INTRODUCTION AND AIM: National guidance recommends hot (same-admission) ureteroscopy and laser lithotripsy as the default definitive treatment for suitable obstructing ureteric stones, rather than defaulting to temporizing measures such as ureteric stent insertion. The primary aim was (1) to determine the proportion of emergency ureteric stone cases managed by hot ureteroscopy against the GIRFT standard. Secondary aims were (2) to describe the pattern of emergency management and identify deviations from evidence-based practice, and (3) to assess the impact of a targeted action plan on the hot

Ex vivo retrograde stone removal in donor kidneys using an ultra-mini nephroscope prior to renal transplantation in Indonesia: a case series.

INTRODUCTION: Urolithiasis in donor kidneys presents a relative contraindication for transplantation due to the potential risk of obstruction and impaired graft function. Although less common in renal transplant recipients, obstructive urolithiasis remains a significant concern. Ex vivo stone removal during bench surgery has been explored as a safe and effective method to mitigate this risk. OBJECTIVE: To present a case series demonstrating the feasibility and outcomes of ex vivo stone clearance using a rigid ultra-mini percutaneous nephrolithotomy (PCNL) set with a 7-Fr nephroscope via a ret

Comprehensive Management of Complex and Refractory Urolithiasis: Minimally Invasive Techniques, Perioperative Optimisation, and Recurrence Prevention.

BACKGROUND: Complex and refractory urolithiasis represents a growing clinical challenge, characterised by a large stone burden, recurrent disease, anatomical abnormalities, infection, and metabolic risk factors, all of which complicate management and increase morbidity. Contemporary management has shifted toward risk-stratified, minimally invasive strategies supported by advances in endourology, perioperative optimisation, and individualised patient assessment. To synthesise current evidence on the diagnostic evaluation, risk stratification, minimally invasive management, perioperative optimis
