Biomechanical and Cortical Control of Tongue Movements During Chewing and Swallowing.

Tongue function is vital for chewing and swallowing and lingual dysfunction is often associated with dysphagia. Better treatment of dysphagia depends on a better understanding of hyolingual morphology, biomechanics, and neural control in humans and animal models. Recent research has revealed significant variation among animal models in morphology of the hyoid chain and suprahyoid muscles which may be associated with variation in swallowing mechanisms. The recent deployment of XROMM (X-ray Reconstruction of Moving Morphology) to quantify 3D hyolingual kinematics has revealed new details on flex

Identifying Non-Traditional Approaches to Swallowing Rehabilitation: A Scoping Review.

In recent years, dysphagia care has shifted toward a more patient-centered approach. This means that dysphagia clinicians are considering more factors in the treatment process including the goals of care, quality of life, and cultural values. The purpose of this scoping review was to examine relevant research and identify dysphagia interventions outside of those traditionally used by dysphagia clinicians that may improve swallowing function. A review of relevant studies was conducted using search terms related to swallowing and treatment. The search yielded 8439 unique studies. Of the 283 arti

Dynamic Imaging Grade of Swallowing Toxicity in Children with Esophageal Atresia.

INTRODUCTION: The Dynamic Imaging Grade of Swallowing Toxicity (DIGEST) scale was developed to evaluate the safety, efficiency, and overall pharyngeal swallowing performance in patients with dysphagia (DIGESTs, DIGESTe, and DIGESTt, respectively). Although various types of swallowing dysfunction are encountered in children with esophageal atresia (EA), oropharyngeal dysphagia poses risk for aspiration. Therefore, a retrospective study was performed to evaluate the safety and efficacy of swallowing by using DIGEST score in children with EA. PATIENTS AND METHODS: Thirty-nine EA patients were in

Videoendoscopic evaluation of swallowing using normal saline in NICU infants.

BACKGROUND: Videoendoscopic evaluation (VE) of swallowing involves inserting a small flexible endoscope transnasally to allow direct observation of pharyngeal and laryngeal structures and swallowing functions. In our neonatal intensive care unit (NICU) VE is performed in infants suspected of severe swallowing dysfunction, using normal saline (NS) with the aim of minimizing the detrimental effect of aspiration on respiration caused by VE. This retrospective study was conducted to evaluate the usefulness of VE using NS in NICU infants. METHODS: We enrolled 28 infants who demonstrated clinical s

Swallowing and laryngeal complications in lung and heart transplantation: Etiologies and diagnosis.

Despite continued surgical advancements in the field of cardiothoracic transplantation, post-operative complications remain a burden for the patient and the multidisciplinary team. Lesser-known complications including swallowing disorders (dysphagia), and voice disorders (dysphonia), are now being reported. Such disorders are known to be associated with increased morbidity and mortality in other medical populations, however their etiology amongst the heart and lung transplant populations has received little attention in the literature. This paper explores the potential mechanisms of oropharyng

Delayed Anterior Cervical Screw Migration Causing Swallowing Difficulty.

Cervical disc disease is a common pathology that may require surgical intervention. A common surgical intervention for cervical disc disease is anterior discectomy and fusion. A number of complications are associated with this technique, including hoarseness, dysphagia, pseudoarthrosis, adjacent segment disease, and hardware failure. Here, we present a case detailing a unique complication of delayed migration of an anterior cervical screw which resulted in swallowing difficulties. Subsequently, we discuss the related reports and highlight the literature on the topic. We conclude with a brief d

Suppressive effect of the swallowing reflex by stimulation of the pedunculopontine tegmental nucleus.

This study investigates whether the swallowing reflex is modulated by stimulation of the pedunculopontine tegmental nucleus (PTg). Sprague-Dawley rats under urethane anesthesia were used. The swallowing reflex was induced by electrical stimulation of the superior laryngeal nerve and was identified by the electromyographic activities from the mylohyoid muscle. The number of swallows was reduced by electrical stimulation of the PTg. The latency of the onset of the first swallow was increased during stimulation of the PTg. The duration of electromyogram bursts of the mylohyoid muscle was signific…

The Trouble with Swallowing: Dysphagia as the Presenting Symptom in Lateral Medullary Syndrome.

BACKGROUND: Posterior circulation strokes account for approximately one-fourth of all ischemic strokes, but are frequently misdiagnosed by emergency providers. Current standard stroke screening tools such as the National Institutes of Health Stroke Scale and the Cincinnati Prehospital Stroke Scale are weighted toward anterior circulation stroke diagnosis. Lateral medullary syndrome, a type of posterior circulation stroke, can be particularly challenging to diagnose due to nonspecific presenting symptoms, such as dysphagia. CASE REPORT: This report describes a 65-year-old man who presented wit

Clinical reasoning and hypothesis generation in expert clinical swallowing examinations.

BACKGROUND: A clinical swallow examination (CSE) provides integral information that informs the diagnostic decision-making process within dysphagia management. However, multiple studies have highlighted a high degree of reported variability within the CSE process. It has been hypothesized that such variability may be the result of the clinical reasoning process rather than poor practices. AIMS: To elucidate the nature of expert, speech-language therapists' (SLTs) clinical reasoning during an initial bedside assessment of patients referred for suspected dysphagia in the acute care environment.
