BACKGROUND: Ankle osteoarthritis (OA) mainly arises from trauma, particularly lateral ligament injuries. Among lateral ligament injuries, ankles with calcaneofibular ligament (CFL) injuries exhibit increased instability and can be a risk factor ankle OA progression. However, the relationship between CFL injury and OA progression remains unclear. Therefore, this study aims to assess the relationship between CFL injuries and ankle OA by investigating stress changes and osteophyte formation in subtalar joint.
METHODS: We retrospectively reviewed the magnetic resonance imaging (MRI) and plain rad

BACKGROUND: : Walking disability caused by central nervous system injury often lingers. In the chronic phase, there is great need to improve walking speed and gait, even for patients who walk independently. Robot-assisted gait training (RAGT) has been widely used, but few studies have focused on improving gait patterns, and its effectiveness for motor function has been limited. This report describes the combination of "RAGT to learn the gait pattern" and "ankle robot training to improve motor function" in a patient with chronic stage brain injury.
CASE: : A 34-year-old woman suffered a trauma

Background: The ankle is prone to injury during drop landing with usual residual symptoms, and functional ankle instability (FAI) is the most common. Vision guarantees the postural stability of patients with FAI, and visual deprivation (VD) increases their risk of injury when completing various movements. This study explored injury risk during drop landing in patients with FAI under VD through the kinetics of lower extremities. Methods: A total of 12 males with FAI participated in the study (age, 23.0 ± 0.8 years; height, 1.68 ± 0.06 m; weight, and 62.2 ± 10.4 kg) completed single-leg drop lan

Inhibitory pathways from Golgi tendon organs project widely between muscles crossing different joints and axes of rotation. Evidence suggests that the strength and distribution of this intermuscular inhibition is dependent on motor task and corresponding signals from the brainstem. The purpose of the present study was to investigate whether this sensory network is altered after spinal cord hemisection as a potential explanation for motor deficits observed after spinal cord injury (SCI). Force feedback was assessed between the long toe flexor and ankle plantarflexor (flexor hallucis longus), an

BACKGROUND: Arthroscopic ankle arthrodesis (AAA) is minimally invasive surgery, whereby percutaneous screw fixation is used through the medial aspect of the distal tibia to fusion the ankle, but it carries the risk of the saphenous vein and nerve injuries. The near-infrared (NIR) vascular imaging system, the VeinViewer® Flex, projects an image of the vein onto the skin, and the visualization of the vein may reduce the vein and nerve injuries. The purpose of this study is to investigate the risk of the saphenous vein injury by the percutaneous screw insertion during AAA, and to evaluate the eff

Instrumented postural control analysis plays an important role in evaluating the effects of injury on dynamic stability during balance tasks, and is often conveyed with measures based on the displacement of the center-of-pressure (COP) assessed with a force platform. However, the desired outcome of the task is frequently characterized by a loss of dynamic stability, secondary to injury. Typically, these failed trials are discarded during research investigations, with the potential loss of informative data pertaining to task success. The novelty of the present study is that COP characteristics

BACKGROUND: Ankle inversion injury is common in military populations but associated biomechanical risk factors are largely unknown. This prospective study examined the association between pressure and kinematic variables, and ankle inversion injury risk in Royal Marine (RM) recruits. It was hypothesised that a more medially concentrated pressure at the heel-off phase of stance, greater impulse and peak pressure at the first metatarsal head, greater peak rearfoot eversion angle and greater eversion excursion would be associated with ankle inversion injury.
METHODS: Data from 145 male, injury-f

The time evolution of the postural behavior of 23 lateral ankle sprain patients (degrees I and II) were evaluated 14 h and 10 and 30 days on average after their injury and compared with those of 30 age-matched healthy individuals. The patients were tested with separate measurements of the reaction forces under each limb to highlight the possible compensatory mechanisms between the sound and the injured legs. Their postural behavior in bipedal stance was characterised by a weight-bearing asymmetry with more weight on the sound leg and an asymmetry of the postural stabilisation mechanisms, which

The deltoid ligament complex (DLC) is well-known as an ankle joint stabilizer; however, through the superficial DLC, it also acts at the subtalar joint and helps to maintain the medial longitudinal arch. DLC repair in the presence of an ankle fracture remains a topic of controversy in the current literature. There is a paucity within the literature surrounding clinical tests that could be performed to assess for hindfoot instability in patients with DLC injuries associated with ankle fracture. However, hindfoot instability has been observed intraoperatively under fluoroscopy via valgus stressi

This study investigated the association between ankle injury and the subsequent onset of non-specific low back pain (LBP), and identified the potential contributions of age, sex, body mass index (BMI), and physical activity (PA) to LBP development. This retrospective cohort study used data from the Korean National Health Insurance Sharing Service (2002-2021). Kaplan-Meier analyses estimated cumulative incidence and mean survival time to LBP onset. Cox proportional hazards models were employed to calculate adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) for LBP onset, adjusting

The computer-controlled knee-ankle-foot orthosis C-brace (OttoBock, Duderstadt, Germany) reportedly improves walking ability and balance. However, the physical therapy contents and duration needed to obtain these benefits have not been described. Therefore, we present a rehabilitation program designed to achieve walking ability using the C-brace in a patient with chronic traumatic incomplete spinal cord injury (iSCI). A 53-year-old man with chronic traumatic iSCI was able to walk with braces and aids after undergoing outpatient physical therapy using a C-brace at a university hospital. The pat

BACKGROUND: Anterior cruciate ligament (ACL) injuries may correlate with lower limb angles and biomechanical factors in both dominant and non-dominant legs at initial contact (IC) post-landing. This study aims to investigate the correlation between ankle angles in three axes at IC and knee and hip joint angles during post-spike landings in professional volleyball players, both pre- and post-fatigue induction.
RESEARCH QUESTION: To what extent does fatigue influence lower limb joint angles, and what is the relationship between ankle joint angles and hip and knee angles at IC during the landing

BACKGROUND Lateral malleolus fractures, typically from trauma, sports, or accidents, are common, with supination external rotation (SER) injuries being most prevalent. SER injuries involve complex joint mechanics and often necessitate surgical intervention for instability. This study compares intramedullary nail and plate fixation for fibula fractures in SER type IV ankle injuries, considering their biomechanical properties and influence on fracture healing. MATERIAL AND METHODS A prospective, randomized study was conducted between January 2021 and December 2021. A total of 81 patients with SE…

Background Ankle syndesmotic injuries represent complex orthopaedic injuries, commonly requiring open reduction and fixation. Several techniques have been described for fixation, with syndesmotic screw fixation being traditionally considered as the 'gold standard'. Among the relatively new techniques developed, the TightRope system stabilisation provides 'dynamic' stabilisation with promising results. We aimed to evaluate the radiographic performance of these two different surgical techniques in the management of ankle fractures with an underlying syndesmotic injury. Methods A total of 85 case

Previous findings indicate that co-contractions of plantarflexors and dorsiflexors during quiet standing increase the ankle mechanical joint stiffness, resulting in increased postural sway. Balance impairments in individuals with incomplete spinal cord injury (iSCI) may be due to co-contractions like in other individuals with reduced balance ability. Here we investigated the effect of co-contraction between plantar- and dorsiflexors on postural balance in individuals with iSCI (iSCI-group) and able-bodied individuals (AB-group). Thirteen able-bodied individuals and 13 individuals with iSCI wer
