Orthostatic Hypotension in the Hypertensive Patient.

Orthostatic hypotension (OH) is an important and common medical problem, particularly in the frail elderly with multiple comorbidities and polypharmacy. OH is an independent risk factor for falls and overall mortality. Hypertension is among the most common comorbidities associated with OH, and its presence complicates the management of these patients because treatment of one can worsen the other. However, there is evidence that uncontrolled hypertension worsens OH so that both should be managed. The limited data available suggest that angiotensin receptor blockers and calcium channel blockers

Lymphatic drainage pathways from the cervix uteri: implications for radical hysterectomy?

OBJECTIVE: Radical hysterectomy with pelvic lymphadenectomy is the treatment of choice for early-stage cervical cancer. Wertheim's original technique has been often modified, mainly in the extent of parametrectomy. Okabayashi's technique is considered as the most radical variant regarding removal of the ventral parametrium and paracolpal tissues. Surgical outcome concerning recurrence and survival is good, but morbidity is high due to autonomic nerve damage. While the autonomic network has been studied extensively, the lymphatic system is less understood. This study describes the lymphatic dra

The role of low-level vagus nerve stimulation in cardiac therapy.

Introduction: Cardiovascular diseases are accompanied by autonomic nervous system (ANS) imbalance which is characterized by decreased vagal tone. Preclinical and clinical studies have revealed that increasing vagal activity via vagus nerve stimulation (VNS) could protect the heart. Based on these studies, VNS has emerged as a potential non-pharmaceutical treatment strategy. Although it's still difficult to find the optimal stimulus parameters, however, in arrhythmia model, it is reported that low-level VNS (LL-VNS) exacts paradoxical effects from the high-level VNS. Thus, the concept of LL-VNS

Lower airway smooth muscle contraction induced by Ptychodiscus brevis (red tide) toxin.

Ptychodiscus brevis toxin (PBTX) is produced by the organism Ptychodiscus brevis. This toxin causes a phenomenon that has come to be known as Florida red tide. It also stimulates neuronal sodium channels, resulting in activation of the cholinergic and adrenergic nerve endings of the autonomic nervous system in upper airway smooth muscle and rat vas deferens, respectively, as previously reported. It is the cholinergic stimulating action that has been implicated as a possible "triggering" event in bronchial asthma. This article concerns the investigation of whether PBTX may also affect lower air

Activation of peripheral muscarinic receptors rescues depressive-like behaviors via gut-brain-axis, involving parasympathetic excitation.

Dysfunction of the autonomic nervous system (ANS) is strongly linked to the pathophysiological mechanisms of depression. Unfortunately, the correlation between depressive-like behaviors and the activation or inhibition of the sympathetic/parasympathetic nervous system has not been systematically explored. This study demonstrated that the pharmacological activation of sympathetic α-/β-adrenergic receptors increased stress susceptibility, whereas their antagonists enhanced stress resistance, thus suggesting a close correlation between sympathetic activity and depressive-like behaviors. Furthermo

Autonomic response characteristics of porcine airway smooth muscle in vivo.

We studied the autonomic response characteristics of airways in 65 swine in vivo. Tracheal smooth muscle response was measured isometrically in situ; bronchial response was measured simultaneously as change in airway resistance and dynamic compliance. To determine the optimal resting length at which maximal tracheal contraction was obtained, length-tension studies were generated in four animals using maximal electrical stimulation of the vagus nerves determined from stimulus-response characteristics in eight other swine. Pharmacological studies were performed in 25 animals to determine the rel

[Bilateral posterior thoracoscopic splanchnicectomy in a face-down position in the management of chronic pancreatic pain].

UNLABELLED: Abdominal pain is the primary symptom in most patients with chronic pancreatic diseases. Many authors have reported that thoracoscopic splanchnicectomy is a effective and recognize method for relief of chronic pain due to pancreatic cancer and chronic pancreatitis. Time ago only few authors used bilateral toracoscopic splanchnicectomy in one session because they observed side effects as transient orthostatic hypotension and/or diarrhea. The aim of this study was retrospective analysis of two methods of the bilateral thoracoscopy splanchnicectomy at one session. We comparsion operat

Autonomic and sensory nerve modulation of peristalsis in the upper urinary tract.

The primary function of the upper urinary tract is to propel urine and various water-soluble toxic compounds from the kidneys to the bladder for storage and evacuation to maintain body ionic balance and contribute to the regulation of blood volume and pressure. The mechanism by which the upper urinary tract propels urine has long been considered to be myogenic in origin as peristaltic contractions in vivo and in vitro (pyeloureteric peristalsis) propagate in a manner little affected by drugs that block nerve conduction or the sympathetic and parasympathetic transmission. However, it is now wel

Targeting dual pathways in refractory coccydynia: A comparative study of ganglion impar block alone versus combined with pericoccygeal injection.

OBJECTIVES: Coccydynia is a complex and often refractory pain condition involving both sympathetic and somatic components. While ganglion impar block (GIB) is a well-established interventional technique for cases unresponsive to conservative treatment, pericoccygeal injection may offer additional benefits by targeting peripheral sensitization and anococcygeal nerve-mediated pain. This study aimed to evaluate and compare the clinical effectiveness of GIB alone versus GIB combined with pericoccygeal injection in patients with refractory coccydynia. METHODS: This retrospective cohort study inclu

Intraoperative autonomic neural blockade: comparison between different local anesthetics combinations: a randomized clinical trial.

BACKGROUND: Advances in minimally invasive techniques, enhanced recovery protocols, and regional pain management have improved postoperative outcomes. However, visceral pain and postoperative nausea and vomiting (PONV) remain challenging, particularly after laparoscopic sleeve gastrectomy (LSG) and other minimally invasive procedures. Intraoperative autonomic neural blockade (ANB) has demonstrated efficacy in addressing these issues, with the duration of the effect its most important limitation. METHODS: This randomized clinical trial (RCT) evaluated the effectiveness of three anesthetic comb

A Multicenter Study of Stellate Ganglion Block as a Temporizing Treatment for Refractory Ventricular Arrhythmias.

BACKGROUND: Ventricular tachycardia (VT) and ventricular fibrillation (VF) are life-threatening conditions and can be refractory to conventional drug and device interventions. Stellate ganglion blockade (SGB) has been described as an adjunct, temporizing intervention in patients with refractory ventricular arrhythmia. We examined the association of SGB with VT/VF in a multicenter registry. OBJECTIVES: This study examined the efficacy of SGB for treatment/temporization of refractory VT/VF. METHODS: The authors present the first analysis from a multicenter registry of patients treated for refr

Neuropathic pain with features of complex regional syndrome in the upper extremity after herpes zoster.

We report a case of a 73-year-old female who developed unbearable neuropathic pain after a herpes zoster episode. The pain persisted and could not be controlled despite multimodal analgesia. In addition to postherpetic neuralgia, myelitis and complex regional pain syndrome were diagnosed during the evolution of neuropathic pain. This complex neuropathic pain was resolved after sympathetic ganglion block.

Local anaesthetics for acute reversible blockade of the sympathetic baroreceptor reflex in the rat.

Sinoaortic denervation is a common method used in the investigation of function in the cardiovascular system. In the rat, this is usually accomplished by transection of the carotid sinus and aortic depressor nerves, which is permanent. We propose a novel, and simple method for acute baroreceptor denervation in the rat in which the region around the carotid sinuses is superfused with local anaesthetic agents. We demonstrate that complete baroreceptor denervation can be achieved for longer (bupivacaine, 30-60 min) or shorter (lignocaine, 10-30 min) periods, without harming the physiological stat

Effects of stellate ganglion block on cerebral haemodynamics as assessed by transcranial Doppler ultrasonography.

BACKGROUND: Stellate ganglion block (SGB) causes vasodilatation in the skin of the head and neck because of regional sympathetic block. Its effects on cerebral haemodynamics, in health or in disease, are not clear. We evaluated the effects of SGB on ipsilateral middle cerebral artery flow velocity (MCAFV), estimated cerebral perfusion pressure (eCPP), zero flow pressure (ZFP), carbon dioxide reactivity (CO2R) and cerebral autoregulation using transcranial Doppler ultrasonography (TCD). METHODS: Twenty male patients, with pre-existing brachial plexus injury, and undergoing SGB for the treatmen

CT-guided injection of the stellate ganglion: description of technique and efficacy of sympathetic blockade.

Stellate ganglion blockade, typically performed by using palpable osseous landmarks, is an accepted method in the diagnosis and treatment of sympathetically maintained pain and other conditions. The authors present the results of stellate ganglion injection performed with computed tomographic (CT) guidance. Thirteen CT-guided procedures were performed in seven patients. Lidocaine, bupivacaine, and buprenorphine were used to achieve the blockade. Successful blocks were achieved in all 13 cases as evidenced by production of Horner syndrome and ipsilateral hand temperature elevations. The average

Epidural epinephrine and the systemic circulation during peripheral vascular surgery.

This study was designed to determine the haemodynamic effects of epidural epinephrine, 5 micrograms.ml-1, added to bupivacaine, 0.75 per cent, in elderly patients with cardiac disease undergoing peripheral vascular surgery (PVS). The effect of epidural epinephrine on the plasma concentration of bupivacaine was also measured. Twenty patients with a history and/or ECG evidence of myocardial ischaemia requiring PVS were randomly assigned to two groups. The patients were monitored with a modified V5 ECG, oscillometric BP monitor and a PA catheter. After control haemodynamic measurements, 12 ml of

[Changes in quantitative characteristics of mitochondria of the cortical tubules of the kidneys with compensatory hypertrophy in the blocking of autonomic mediation].

To elucidate the significance of neuromediators in ergotrophic provision of duct cells of the compensatory-hypertrophying kidney mitochondrial apparatuses of cortical nephron segments epithelium in a single kidney of Wistar rats have been morphometrically studied under conditions of continuous 90-day blockade of vegetative nerves by administration of both guanetidine and atropine. It is shown that distal duct mitochondria have undergone the most pronounced regressive changes in the experimental series that is accounted for by specificity and power capacity of the transport processes in this se

Mechanism of the postural vasoconstrictor response in the human foot.

1. The mechanism of postural vasoconstriction in the skin of the foot was examined in 102 healthy subjects by using laser Doppler flowmetry. 2. In 45 subjects, when one foot was lowered 50 cm below heart level and the other foot kept horizontal, blood flow was progressively reduced in the dependent foot (by 79%) with a concomitant, but less pronounced, reduction in flow in the horizontal foot (by 18%), indicating that a central mechanism is involved. After lumbar sympathetic blockade (in 10 patients with epidural anaesthesia), the flow in the horizontal foot remained virtually constant, indica
