Femoral stiffness after osteosynthesis of a subcapital osteotomy in osteoporotic bone: an in-vitro comparison of nine fixation methods.

Restitution of normal bone mechanics after osteosynthesis of a proximal femoral fracture in the elderly is assumed to be a prerequisite for optimal healing. To find the most appropriate fixation device for this purpose, the stiffness of 79 cadaver femora from donors with a history of osteoporosis was measured during axial loading before and after a subcapital osteotomy stabilized with one of nine different osteosynthesis devices. Only an osteotomy stabilized with an experimental device consisting of 2 von Bahr screws (Ericsson AB, Göteborg, Sweden) supported by acrylic cement gave similar mean

Biomechanical effect of medial cortical support and medial screw support on locking plate fixation in proximal humeral fractures with a medial gap: a finite element analysis.

OBJECTIVE: This finite element analysis aimed to examine the effect of medial cortical support and medial screw support on loads at the implant-bone interface of locking plate fixation of proximal humeral fractures with a medial gap. METHODS: An intact humerus from a healthy volunteer was used as the basis for a 3-dimensional (3D) computer-aided design (CAD) model. The 3D CAD model of the locking plate system was based on information in the manufacturer's catalogue. The proximal part of the humerus was osteotomized to create standard three-part fractures, which were then divided into a -MSC g

A Novel Viscoelastic Deformation Mechanism Uncovered during Vickers Hardness Study of Bone.

This study investigates the viscoelastic deformation mechanisms of bone as a response to Vickers hardness indentation. We utilized advanced high-resolution scanning electron microscopy (SEM) to investigate a distinct deformation pattern that originates from the indentation site within the bone matrix. The focus of our research was to analyze a unique deformation mechanism observed in bone tissue, which has been colloquially termed as "screw-like" due to its resemblance to a screw thread when viewed under an optical microscope. The primary goals of this research are to investigate the distincti

Effect of screw thread length on stiffness of proximal humerus locking plate constructs: A finite element study.

Plate-based treatment of proximal humerus fractures is associated with a high risk of complications such as screw perforation into glenohumeral joint. Smooth and threaded pegs were developed with the hope of minimising these risks. No consensus exists onto which threading profile achieves stiffest bone-plate construct. This study investigated the biomechanical effect of five percentages of threading on individual humeral head screws on a bone-plate construct. A finite element model simulating a two-part proximal humerus fracture treated with a Spatial Subchondral Support plate was developed an

Selection of fixation devices in proximal femur rotational osteotomy: clinical complications and finite element analysis.

OBJECTIVE: To compare the postoperative stability between large cancellous screws fixation and dynamic hip screws fixation in transtrochanteric rotational osteotomy. DESIGN: Finite element analysis was designed to validate the clinical outcomes of fixation failure in transtrochanteric rotational osteotomy. BACKGROUND: From 1997 to 2000, transtrochanteric rotational osteotomy had been done in 20 osteonecrotic hips. Fixation with large cancellous screws as suggested by Sugioka had been used in the first five hips. Unfortunately, fixation failures were encountered in all of the five cases. The

Transpedicular screw fixation.

Spinal fixation employing transpedicular screws has recently been the focus of increased attention at various institutions throughout the world, but concerns about the safety and efficacy of transpedicular screws linger. This study was undertaken to address some of these concerns. The study included evaluation of the internal and external morphology of the vertebral pedicles, which revealed that adequate bone stock is generally available at T2, T7, T12, and L1-L5 spinal levels to accept screws in the 4-7-mm diameter range. The pedicle was observed to be composed of abundant cancellous bone int

Roles of a nonvascularized fibular graft with and without fixation in the treatment of segmental tibial bone loss: A finite element analysis.

BACKGROUND: A nonvascularized fibular graft (NVFG) is considered to be an alternative option in managing segmental bone loss of the tibia. Nevertheless, there has been no consensus on optimal graft position and graft fixation technique. The purposes of the present study were to mechanically test the influences of various NVFG fixation techniques on the overall stability of the fixation construct by use of finite element analysis. METHODS: Seven FE models of tibias with segmental bone loss stabilized with various fixation techniques were developed including medial and lateral plate-screw, medi

Impact of Screw Diameter on Pedicle Screw Fatigue Strength-A Biomechanical Evaluation.

OBJECTIVE: Loosening of pedicle screws is a frequently observed complication in spinal surgery. Because additional stabilization procedures such as cement augmentation or lengthening of the instrumentation involve relevant risks, optimal stability of the primarily implanted pedicle screw is of essential importance. The aim of the present study was to investigate the effect of increasing the screw diameter on pedicle screw stability. METHODS: A total of 10 human cadaveric vertebral bodies (L4) were included in the present study. The bone mineral density was evaluated using quantitative compute

A Novel Screwless Modification of the Khoury Plate Technique using an Innovative Bone Adhesive Formulated from Underwater Biomimetic Marine Proteins.

Over the past two decades, the Khoury plate technique has been widely utilized with much success to facilitate bone augmentation in critically deficient bone areas. Simply, the technique includes harvesting autogenous bone plates and utilizing them with external screw fixation. The rigidity of the bone plates enclosing the bony defect on both the buccal and lingual surfaces allows for tension-free bone remodeling to occur, favoring an ideal bone-forming environment. Following, a 4- to 6-month healing period, a flap is raised, screws may be removed, and implants are placed. With advancements ma

Comparative Assessment of the Mechanical Response to Different Screw Dimensions in Scaphoid Fracture Fixation.

The scaphoid is the most commonly fractured carpal bone. Headless compression screws became the gold standard for fixation, but the ideal screw diameter remains debated. This study investigates the relative benefit of using a larger screw diameter to improve stability in typical scaphoid fractures. It also examines the effects of preload and screw length on mechanical behaviour. A finite element (FE) model of a mid-waist scaphoid fracture was created. Screws from Medartis (1.7 mm, 2.2 mm, and 3.0 mm diameter; 23 mm length) were placed along the longitudinal axis. Boundary and loading condition

Complications using intermaxillary fixation screws.

PURPOSE: The most important part of facial bone fracture treatment is immobilization. Bandaging of the fractured mandible was the first immobilization device used in ancient Greece. Since then, various methods with different shapes and uses were introduced in the treatment of maxillofacial trauma patients. Intermaxillary fixation (IMF) with bone screws was first used in 1981. This technique has advantages, including quickness, ease of use, less trauma, and reduced risk of needle-stick injury. We analyze the advantages and disadvantages of this technique. METHODS AND MATERIALS: In a retrospect…

Headless Compression Screw Fixation for Proximal Phalanx Fractures: A Biomechanical Study.

Background: Proximal phalanx fractures are common injuries of the hand with multiple treatment options. Intramedullary (IM) screw fixation has become more widely used, and early outcomes are promising. However, biomechanical data regarding this type of fixation are sparse. Methods: Two methods of IM screw fixation of proximal phalanx fractures were tested in cadaver specimens. All specimens were treated with a single antegrade headless compression screw, with half getting the addition of a blocking screw. To test the most common deforming force of flexion-extension, each phalanx was subjected

Cement augmentation of calcar screws may provide the greatest reduction in predicted screw cut-out risk for proximal humerus plating based on validated parametric computational modelling: Augmenting proximal humerus fracture plating.

AIMS: Fixation of osteoporotic proximal humerus fractures remains challenging even with state-of-the-art locking plates. Despite the demonstrated biomechanical benefit of screw tip augmentation with bone cement, the clinical findings have remained unclear, potentially as the optimal augmentation combinations are unknown. The aim of this study was to systematically evaluate the biomechanical benefits of the augmentation options in a humeral locking plate using finite element analysis (FEA). METHODS: A total of 64 cement augmentation configurations were analyzed using six screws of a locking pl

Monocortical fixation for locking plate distal screws does not impair mechanical properties in open-wedge high tibial osteotomy.

BACKGROUND: The neurovascular bundle containing the deep peroneal nerve has a potential risk of injury during open-wedge high tibial osteotomy (OWHTO), particularly due to drilling for bicortical fixation at distal screw holes. Therefore, monocortical fixation is recommended for distal fixation of a long locking plate as long as good stability is ensured. The purpose of this study was to analyse the biomechanical properties of monocortical fixation of distal locking screws for OWHTO. METHODS: Three-dimensional models of bone and fixation materials simulating OWHTO were created using computed

Central versus eccentric internal fixation of acute scaphoid fractures.

PURPOSE: To accurately calculate the cross-sectional area of typical scaphoid fracture patterns and compare the amount of fracture surface area available for healing when a screw was positioned in the center of the scaphoid (central) versus perpendicular (eccentric) to the fracture plane. METHODS: We employed a laser scanning technique to create 3-dimensional models of the scaphoid and permit the precise calculation of area along any cross-sectional cut of the bone. We computed approximate bone apposition areas for typical acute Herbert and Fisher fractures for 10 dry bone specimens. Next, we
