An overdosage of alfathesin was accidentally administered to a 4 kg infant with multiple congenital anomalies during an operation for craniosynostosis. The overdose was a consequence of a mistake in adjusting the rate of administration of a constant flow infusion pump. The total dosage corresponded to eight times the necessary dose administered over a period of 30 minutes. The only notable effect was a delayed awakening, which necessitated assisted ventilation. The high therapeutic index of alfathesin is responsible for the absence of serious consequences.

Implantation of an intrathecal infusion pump is now accepted as a valuable technique to treat chronic pain. The most frequently reported complications associated with it are catheter fractures or migrations, usually resulting in a recurrence of pain and the onset of withdrawal symptoms. Recently, the neurologic complication of development of a catheter-tip mass has been observed. The unusual complication of intradural catheter migration responsible for sciatica is presented here. Treatment consisted of catheter replacement, leading to immediate disappearance of the sciatica.

AIMS/HYPOTHESIS: Methacholine (MC) is a nitric oxide vasodilator, but unlike other vasodilators, it potentiates insulin-mediated glucose uptake by muscle. The present study aimed to resolve whether this action was the result of a vascular effect of MC leading to increased muscle perfusion or a direct effect of MC on the myocytes.
