This is a rare case report of medial femoral condyle fracture with irreducible incarcerated patella dislocation. Following the literature search, only a few cases have been reported in the literature. In this case, the patient had undergone open reduction and screw fixation of the femoral condyle, augmented with a distal femoral locking plate (LCP). Postoperatively patient was immobilised with an above knee backslab for a month. After removing the backslab, physiotherapy was commenced to improve the range of motion and strengthening the quadriceps muscle. After 18 months of follow-up, the pati

PURPOSE: The surgical technique, medial patellar retinaculum plasty, can almost restore both static and dynamic stability and verge on anatomical repair for the treatment of habitual patellar dislocation in adolescents.
METHODS: In accordance with the injury patterns of the medial patellar retinaculum through knee MRI, we repaired different injury sites with this surgical procedure. We reviewed this technique in 16 patients with an average age of 15 years. Retrospective review of charts and radiographs immediately after the surgery up to the latest follow-up (range 12-36 months) was undertake

Ellis-van Creveld Syndrome (EVC) is a rare autosomal recessive disorder. In 1940 Richard W. Ellis and Simon van Creveld first reported on a rare skeletal dysplasia, which to emphasize the main clinical characteristics, was termed "chondro-ectodermal dysplasia". The ectodermal involvement includes the skin, hair and nails while the chondrodysplastic characteristics involve the cartilage and bones, primarily in the forearms and lower legs. For the orthopaedic surgeon progressive valgus knee deformity accompanied by patella dislocation is the main problem in EVC. This study reports a ten year fol

BACKGROUND: Patellofemoral cartilage deterioration and osteoarthritis are reported to be associated with recurrent patellar dislocation. However, the association between first-time traumatic patellar dislocation and cartilage deterioration is unknown.
PURPOSE: The aim of this study was to assess long-term cartilage deterioration in the patellofemoral and tibiofemoral joint after conservatively treated traumatic lateral patellar dislocation.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: Twenty patients (mean age, 25 years) who sustained first-time traumatic lateral patellar dislo

BACKGROUND: About 10% to 16% of patients with patellar instability have bilateral involvement. The risk factors for future contralateral patellar instability in patients who have ipsilateral patellar instability are not known. Knowledge related to the possibility of future contralateral patellar instability would be helpful, as previous studies have shown suboptimal outcomes and increased complication rates in patients with bilateral patellar instability.
PURPOSE: The purpose of the current study was to identify the risk factors for future contralateral patellar instability and develop a pred

Acute lateral patellar dislocation is relatively common in younger age group and most likely caused by indirect trauma. About 10% of acute patella dislocations are the result of a direct blow to the medial side. We report a case of irreducible lateral patellar dislocation which is of exceptional interest in that lateral patellar dislocation (extra-articular) occurred in comparatively older age group with arthritic knee, there was no direct trauma involved and it was associated with some rotation along long axis, impaction of patella on lateral femoral condyle and was locked underneath osteophy

A 63-year-old white woman was evaluated for a chronically dislocating right patella 6 months following total knee arthroplasty. At the time of our initial evaluation, her knee would not extend beyond 35 degrees and her patella was dislocated and irreducible. At arthrotomy, when the patella was everted, the patellar component was oriented with the facet ridge rotated 90 degrees to the trochlear groove of the femoral component. The patellar template guide indicated that the fixation pegs had been drilled properly, and thus, the patellar component had been assembled improperly during manufacturin

The present paper reports a case of osteo-onychodysplasia (nail-patella syndrome). There are very few descriptions of this syndrome in the orthopaedic literature, possibly because it is rare for the morphological changes in the knee and the associated abnormalities to require or be amenable to treatment. In the present case an accidental sprain of the right knee caused recurrent dislocation of the patella that necessitated surgical intervention.

Patellar instability is the pathologic condition where the patella is prone to recurrent lateral dislocation. The clinical results on large series of patients and long-term outcome of Goldthwait techniques have not been described in the literature. The aim of this systematic review is: (1) to analyze and summarize the available literature focused on Goldthwait procedure in the treatment of recurrent patellar dislocation and (2) to evaluate the clinical and functional outcomes of patients treated with this surgical procedure. A systematic review of the literature was performed to investigate th

PURPOSE: Our purpose was to study the 1- to 7-year results of a new arthroscopic technique for recurrent lateral patellar dislocation or subluxation.
METHODS: The study group included 38 knees in 37 patients treated over a period of 7 years; 36 knees in 35 patients were available for review (2 were lost to follow-up). There were 26 female and 9 male patients. All patients were evaluated at a mean follow-up of 51 months (range, 1 to 7 years). The indications for surgery were recurrent dislocation in 23 knees and recurrent subluxation in 13 knees. Arthroscopic evaluation and lateral release was

Multiple ligament injuries of the knee are rare, and severe injuries are often present following a knee dislocation. Appropriate initial evaluation and management is of critical importance for maximizing the potential outcome and avoiding disastrous consequences associated with these difficult injuries. While many of these injuries present acutely in an emergency department or clinic, presentation on a delayed basis is not uncommon. Injuries can present in a variety of situations, such as an isolated knee injury in an athlete or as part of a multisystem-injured trauma patient. In this publicat…

In knee arthroplasty, a lax flexion gap may cause flexion instability. Subsequently, the operated knee may subluxate or dislocate. We report on a case of recurrent subluxations with malrotation of the rotating platform (spinout) after primary LCS total knee arthroplasty. Potential causes for spinout, therapy procedures, and consequences for the primary surgical technique are discussed. In the LCS rotating platform knee arthroplasty, the flexion gap must not be laxer than the extension gap. In such an instance, we recommend a higher rotating platform and a more proximal distal transverse femora

BACKGROUND: Medial patellofemoral ligament reconstruction (MPFL-R) is the primary surgical intervention for recurrent lateral patellar dislocation (LPD). Isolated MPFL-R is recommended for patients without anatomic high-risk factors that predispose to further dislocations.
PURPOSE: To compare instability recurrence in patients with recurrent LPD without underlying anatomic risk factors treated with MPFL-R versus active rehabilitation.
STUDY DESIGN: Randomized controlled clinical trial; Level of evidence, 1.
METHODS: Patients aged 12 to 30 years with recurrent LPD and no underlying anatomic

PURPOSE: No outcome measurements have previously been designed to assess patient-perceived patellar instability. The purposes of this study were to address this limitation and to describe the development and validation of the Norwich Patellar Instability (NPI) scores, a self-administered 19-item questionnaire to assess perceived patellar instability.
METHODS: A previous study assessed activities that aggravated symptoms in individuals with patellar dislocation and instability symptoms. These reported activities were ranked in order of severity, and a weighting system was calculated. The NPI s

The factors involved in the mechanism leading to traumatic posterior dislocation of the hip are examined. In 47 adult patients who had previously suffered such a dislocation, ultrasound scans were used to measure femoral anteversion on both the affected and the uninjured side. In 36 normal adult volunteers, used as controls, similar measurements were made. Femoral anteversion on both the injured and uninjured side was significantly reduced in the patients compared with the volunteers. These findings are discussed in the light of previous work which indicates that medial rotation is a factor in

We report on the case of a 17-year-old boy with clinical features compatible with Lowry-Wood syndrome: microcephaly, short stature, multiple epiphyseal dysplasia, tapetoretinal degeneration, and mental retardation. Bilateral restricted elbow extension, knock knees and hip dislocation were also present. Radiographs showed evidence of radial dislocation due to the absence of the radial heads, lateral dislocation of both patellae, multiple epiphyseal dysplasia that was more severe at the proximal femoral epiphyses, and dislocation of both hips with severe hip dysplasia. The patient developed a be

Of 27 patients sustaining primary patellar dislocations, 20 were treated with immobilization and subsequent physiotherapy (including nine patients who underwent arthroscopy) and seven with immediate surgical stabilization and lateral release. The patients with predisposing factors such as patellofemoral malalignment, abnormal patellar configuration, and a history of prior symptoms of instability were more prone to recurrent dislocation and may benefit from operative intervention. Although the incidence of recurrence among those individuals can be decreased, at least 30% to 50% of all patients

BACKGROUND: This retrospective study aimed to analyze avulsion and osteochondral fractures associated with patellar dislocation using 3-dimensional (3D) computed tomography (CT).
METHODS: A retrospective analysis was conducted on patients admitted between 2015 and 2023 with acute or recurrent patellar dislocations. A musculoskeletal radiologist and 2 orthopedic surgeons evaluated CT scans of 148 patients (160 knees) to identify and categorize all avulsion and osteochondral fractures. The included fracture patterns were as follows: pattern I: medial patellofemoral ligament (MPFL) avulsion from

BACKGROUND: Habitual dislocation of the patella (HDP) is a rare condition. Many surgical procedures using proximal realignment have been reported to treat HDP in children, with around 80% satisfactory results. However, few articles have addressed the treatment of HDP associated with high-grade patellofemoral chondromalacia in adults.
HYPOTHESIS: A combination of proximal and distal realignment procedures of the patella will reduce pain and increase function in adult patients with HDP.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: Twelve patients with 13 symptomatic cases of HDP

Rotational dislocations of patella, which involve rotation of the patella around a horizontal or vertical axis are rare. These rotational dislocations of patella are difficult to reduce by close methods. These dislocations can have associated osteochondral and retinacular injury. We report a case of a 20-year-old male who presented with swelling and pain in the right knee following a motor cycle accident. Radiological evaluation using the computed tomography revealed a patellar dislocation with a concomitant Hoffa fracture. Patella was rotated around the vertical axis and was incarcerated into
