Bilaterally subluxed diffractive intraocular lenses: big expectations and even bigger comorbidities.

A 78-year-old woman with an ocular history of cataract surgery with a diffractive intraocular lens (IOL) in each eye has developed fluctuating vision, greater in the right eye than the left eye, after 4 years. She has a history of inactive central serous retinopathy and a vision potential of 20/25 + 2 in the right eye and 20/25 in the left eye. She has well-controlled diabetes, hypertension, and hypercholesterolemia. She has enjoyed her spectacle independence for some time and wishes to have her vision restored. On examination, her uncorrected distance visual acuity (UDVA) was 20/50 in the rig

The Eye That Hid Its Wound: A Case Report of Corneal Perforation Despite a Negative Seidel Test.

A 58-year-old man presented with pain, redness, watering, and sudden diminution of vision in the right eye following blunt trauma from a stone. Slit-lamp examination revealed mild corneal edema, a central corneal defect, a traumatic cataract with probable anterior lens subluxation, and a distinct air bubble within the anterior chamber. Seidel's test was negative, and no iris prolapse or obvious corneal laceration was identified. The presence of intracameral air raised a strong suspicion of prior communication between the anterior chamber and the external environment, suggestive of a self-seale…

Sutureless intrascleral pocket technique of transscleral fixation of intraocular lens in previous vitrectomized eyes.

In this case series, we assessed a new technique, the intrascleral pocket procedure of transscleral fixation (TF) of the intraocular lens (IOL) in post-vitrectomized eyes. We performed the transscleral fixation of IOL in four aphakic patients who underwent pars plana vitrectomy. Two points 180° apart were marked at the limbus. A 2-mm-sized intrascleral pocket was created by lamellar dissection using a crescent blade without conjunctival dissection. A 2.8-mm clear corneal incision (CCI) was made using a keratome. Prolene sutures were exteriorized through the CCI pocket and a three-piece foldabl

Ocular Clues in Musculoskeletal Disorders: A Narrative Review for Orthopaedic Specialists.

Musculoskeletal disorders (MSDs) are primarily characterized by joint, bone, and connective tissue involvement, but they can also present with ocular manifestations that carry diagnostic and clinical significance. These findings such as uveitis, blue sclera, lens subluxation, and scleritis may serve as early markers of systemic disease or indicate ongoing inflammation that affects orthopaedic decision making. Despite their relevance, ocular symptoms are frequently underrecognized in orthopaedic settings, potentially delaying comprehensive care. This narrative review aims to explore the spectru…
