Infantile haemangioma is the most common tumour of infancy, yet the pathogensis of this lesion remains unknown and the predictable life cycle is poorly understood. Though much new information on infantile haemangioma has emerged over the past decade, researchers continue to debate the fundamental features; including cells of origin, nonrandom distribution, and mechanisms regulating the sometimes explosive growth and slow involution. The development of biomarkers has shed light on the pathogenesis and management of infantile haemangioma. Several useful biomarkers and their suggestions as to the

The case of a boy is reported showing the typical symptoms of Down's syndrome, in whom the chromosome analysis revealed a mosaic karyotype: 50% 48,XYY,+21/50% 47,XY,+21. Findings of 92 cases from the literature are summarized to show the frequencies of double gonosome-autosome aneuploidies compared with single trisomies. Referring to the different chromosomes involved, the aneuploid cell formation, the frequencies of combinations, as well as the tendency to mosaic formation are analyzed. The age of parents at the time of birth and the life expectancy are described as well as the clinical sympt

A 43-year-old female patient having a known history of pulmonary sarcoidosis presented with a rapidly developing intracranial hypertension, followed by a state of drowsiness. Cerebral MRI revealed intense leptomeningeal contrast enhancement and thickening of the pituitary gland. The present report detailed the primary imaging abnormalities observed in the patient and highlighted the importance of considering a differential diagnosis within the context of the clinical picture The clinical approach supporting the inflammatory origin of the observed imaging abnormalities in this case is discussed

Behçet's disease is a multisystemic disease of unknown origin characterized by a recurrent bipolar aphtosis (oral and genital) associated with vascular, digestive or articular symptoms. Gynecologists can be faced to this disease at any time of the life of their patients, including during the pregnancy. Given that the first demonstrations of the disease can be genital, they are in the front line to evoke this diagnosis. They thus have to know the main characteristics of the disease to make the diagnosis and to organize a multidisciplinary management. During pregnancy, the treatment of the disea

Hemobilia is one of the causes of obscure gastrointestinal haemorrhage. Most cases of hemobilia are of iatrogenic or traumatic origin. Hemobilia caused by a hepatic artery pseudoaneurysm due to ascending cholangitis is very rare and its mechanism is unclear. We report a 74-year-old woman with a history of surgery for choledocholithiasis 30 years ago, suffering from a protracted course of life-threatening gastrointestinal bleeding. A small intestines series and endoscopic retrograde cholangiopancreatography revealed a chronic cholangitis with marked contrast reflux into the biliary tree. Angiog

Melorheostosis is a rare sclerosing bone dysplasia affecting children and adults. Concerning the etiology it seems to be a metameric abnormality of the tissue of mesodermal origin, which is initiated early in embryonic life prior to the formation of the extremity buds. The flowing hyperostosis is usually associated with limb deformity, soft tissue contractures, severe pain and stiffness of joints. Bone scanning complements standard radiologic studies and permits evaluation of the extent and activity of the disease. Conservative treatment is disappointing. Thus we recommend the excision of the

Behçet's disease is a systemic vasculitis of unknown origin. It mainly affects young men. Vascular involvement mainly affects the veins and may manifest as deep or superficial thrombosis. Arterial involvement is rare and serious. Arterial thrombosis or aneurysms/false aneurysms can be life threatening in case of rupture. All the arteries in the body can be affected, with widely varying frequencies. Involvement of the renal arteries is very rare. We report the case of a young patient followed for schizophrenia and known to have Behçet's disease, in whom a false aneurysm of the right renal arter

In the complex spectrum of coronary anomalies, the origin of the left coronary artery from the right sinus of Valsalva with intramural course could represent a catastrophic life-threatening condition leading to extensive myocardial infarction and sudden cardiac death, especially in young athletes. We report the case of a young female athlete with anomalous left coronary artery from the opposite sinus who survived a major non-ST-elevation myocardial infarction during the eighth kilometer of a running race. It was successfully treated by creating a neo-ostium of the left coronary artery in the l

Meckel diverticulum is the most common abnormality of the gastrointestinal tract arising from an incomplete obliteration of the vitelline duct during the intrauterine life. Although tumor development in Meckel diverticulum is not a common situation, it can occur due to the persistence of cellular islets with gastric, pancreatic or intestinal origin. The presence of a neuroendocrine tumor arising from Meckel diverticulum is even scarcer. We present the case of a 59-year-old patient in whom a Meckel diverticulum was found during surgery for inguinal hernia; the histopathological and immunohistoc

Little is known about the presence and role of neutralizing antibodies (NtAbs) in perinatal hepatitis C virus (HCV) infection. Using HCV pseudoparticles, NtAbs were studied longitudinally in 12 HCV-infected children with or without evidence of acute hepatitis during the first year of life. Broadly reactive NtAbs of maternal origin did not prevent vertical HCV transmission or progression to chronicity. NtAbs against homologous genotype or subtype appeared during the chronic phase and were more abundant and sustained in children with acute hepatitis. Cross-reactive NtAbs were present in both gro

UNLABELLED: Apnoea is a common sign in respiratory syncytial virus (RSV) infections in young infants and can be the first presentation of an acquired RSV infection. We describe polysomnographic recordings of three infants revealing prolonged RSV-related apnoea before RSV infection was diagnosed. The apnoeas were of central origin. The caregivers had not noted any apparent life-threatening events (ALTE) prior to the polysomnography. Cardiorespiratory monitoring after the acute infection did not reveal any further apnoeas.
CONCLUSION: Central, prolonged apnoea can be the first sign of an acquir

Cyanosis of the digits may have several etiologies ranging from trauma to connective tissue disease; however, the most common cause of the so-called blue toe syndrome is atheroembolic disease or aneurysm and is frequently misdiagnosed on initial presentation. Pedal pulses are often palpable which may misdirect the physician from a diagnosis of vascular pathology. Furthermore, the proximal source of embolic shower may be far from the sight of symptoms. Noninvasive vascular testing, peripheral angiography, abdominal and popliteal ultrasonography, and echocardiography are all techniques that may

Numerous drugs may disturb hematopoiesis or damage the peripheral blood cells. These often potentially fatal complications are not frequent, but if they do occur the life of the patient depends on whether the attending physician recognizes the correct causes and immediately stops further administration of the drug responsible. The diagnosis must be differentiated from cytopenias of immunological origin in which autoimmune mechanisms are usually present. The most important isolated cytopenias of the three series of cells of the peripheral blood and pancytopenia are discussed. The fundamental me

URINARY INCONTINENCE IN CHILDREN. Urinary incontinence in children and adolescents is most often of functional origin. Questioning and clinical examination with a bladder diary should look for underlying urological or neurological causes. The type of incontinence, daytime or nocturnal, must be specified to adapt treatments. Renal and bladder ultrasound is recommended, and urodynamic studies are not routinely used. Lifestyle advice and bowel management are prerequisites for treatment. Depending on the type of symptoms, drug treatment, pelvic floor treatment, behavioral measures or stimulation o

The complicated diaphragmatic hernia of traumatic origin, is a life-threatening condition. In most cases, the diaphragmatic rupture is caused by blunt trauma that affects the chest or abdomen, but penetrating wounds may represent also an etiological factor. Traffic accidents are primarily involved, in time of peace. We report a case of a 59-year-old male patient, who had a traffic accident 16 years before his admission to the emergency department. In these years he produced vague dyspeptic symptoms and was treated several times for pleural effusions and COPD. At the time of his latest admissio

CASE: A 6-year-old patient with Ewing sarcoma at L4 who was treated with total en bloc spondilectomy (TES) through a posterior-only approach. During the dissection of the anterior L4 body, there was uncontrollable bleeding from an unknown origin, which needed packing, massive transfusion, and urgent laparotomy. A median sacral artery (MSA) rupture was identified as the cause of this massive bleeding. Eventually, the patient was successfully stabilized.
CONCLUSIONS: To our knowledge, this is the first reported intraoperative bleeding of the MSA related to a lumbar TES through posterior-only ap

Paraneoplastic pemphigus (PNP), a subset of pemphigus, is a unique autoimmune blistering condition that can affect multiple organs other than the skin. It is a life-threatening disease associated with an underlying malignancy, most commonly of lymphoproliferative origin. The clinical picture may resemble pemphigus, pemphigoid, erythema multiforme, graft-versus-host disease, or lichen planus. The earliest and most consistent finding is a painful, severe, chronic and often recalcitrant stomatitis. Treatment of PNP is difficult. Immunosuppressive agents are required to decrease blistering, and tr

The risk of zoonose transmitted from cats and dogs has increased as these animals have been more and more included in family life to become real household members. This paper reviews the main bacterial, parasitic, viral and fungal infections that could be transmitted to humans by these pet animals in different situations. Although the risk of human disease is relatively low for most of the presented infections, its recognition is important to prevent some exposures, to apply correct prophylactic measures after bites, and to include these zoonoses in the differential diagnosis in case of illnes

Several biophysical and biochemical changes are prerequisites for the adaptation of the newborn to extrauterine life. Enzyme induction is the mechanism on which this adaptation of metabolism is based. The theoretical fundamentals of enzyme induction are presented, and the perinatal origin of enzymes explained by means of the example of PEP carboxykinase. Among the mose frequent disturbances of the maturing of organs are hyperbilirubinaemias and the respiratory distress syndrome of newborn. The biochemical and physical concepts underlying these disease pattens are explained, and possibilities o

Cervicofacial actinomycosis is an uncommon, chronic, suppurative, and granulomatous bacterial infection. It is often of dental origin and tends to mimic other dental infections, granulomatous disorders, and cancers. The initial diagnostic workup, predicated upon imaging and tissue biopsies, is frequently nonspecific. A definitive diagnosis is usually rendered only after surgical excision and histologic examination of the cervicofacial mass. We propose a classification of three stages: localized infection without sinus involvement, localized infection with sinus involvement, and disseminated in
