OBJECTIVE: Spinal cord infarction (SCI) is a rare complication of vertebral artery dissection (VAD). Its clinical features and outcomes have not yet been well documented.
METHODS: In addition to reporting a case with bilateral SCI caused by left VAD, we performed a systematic review of the literature conducted through a PubMed search.
RESULTS: A total of 17 cases were reviewed (nine men and eight women). The average age was 40·5±14·6 years. In addition to neck pain or headache (88%), patients with VAD-associated SCI often presented with a sensory level (76%) or Brown-Séquard syndrome (53%). …

BACKGROUND: Vertebral artery dissection (VAD) is one of the most important etiologies in young stroke patients. VAD causes ischemic stroke by embolism and transcranial Doppler (TCD) monitoring can detect microemboli originating from the dissection point as high intensity transient signals (HITS). We developed a simple but novel method of TCD monitoring at the vertebrobasilar junction in VAD patients.
METHODS: We placed a Welder TCD headband upside down on the patient's head and rotated it by 90°. Then we fixed a pulsed-wave 2-MHz TCD probe to the headband and put it on the suboccipital parame…

BACKGROUND AND PURPOSE: CT angiography (CTA) is widely used and may be the only vascular imaging technique ordered for emergent evaluation of neurovascular disease. With thin-section multisection CTA, the resolution of vessel wall imaging has improved. We describe cases of acute vertebral artery dissection (VAD) in which the only abnormality on CTA was a characteristic thickening of the wall of the V3 portion of the vertebral artery (VA). The arterial lumen at the dissection site was normal in caliber. This type of dissection is easily overlooked if only lumen-opacifying studies such as contra…

The dissection of the intracranial vertebral artery (VAD) is a common cause of young age brain stem stroke. VAD can be detected by conventional angiography, but there is yet no agreement on the most effective tool to use for the detection of VAD. Here, we report a patient with VAD, who was diagnosed with an intimal flap within the left vertebral artery by the magnetic resonance angiography (MRA) source images. Transcranial Doppler (TCD) showed a reversed flow in the basilar artery. After 4 months, TCD and transcranial color-coded Doppler (TCCD) confirmed a normal anterograde flow of the verteb

BACKGROUND: Vertebral artery dissecting aneurysms (VADAs) involving the posterior inferior cerebellar artery (PICA) origin present a unique therapeutic challenge due to high rupture risk, complex anatomy, and the critical need to preserve brainstem perfusion. Despite the increasing use of endovascular strategies, no prior meta-analysis has specifically evaluated outcomes for this clinically high-risk subgroup.
OBJECTIVE: To systematically review and quantitatively synthesize available data on the endovascular management of VADAs involving the PICA origin.
METHODS: A comprehensive literature

BACKGROUND AND PURPOSE: Endovascular trapping of the vertebral artery dissecting aneurysms (VADAs) carries a risk of medullary infarction due to the occlusion of the perforating arteries. We evaluated the detectability and anatomical variations of perforating arteries arising from the vertebral artery (VA) using three-dimensional DSA.
METHODS: In 120 patients without VA lesions who underwent rotational vertebral arteriography, the anatomical configurations of perforating arteries from the VA were retrospectively evaluated on the bi-plane DSA and reconstructed images to reach the consensus bet

Cervical spine manipulation (CSM) is a commonly spinal manipulative therapies for the relief of cervical spine-related conditions worldwide, but its use remains controversial. CSM may carry the potential for serious neurovascular complications, primarily due to vertebral artery dissection (VAD) and subsequent vertebrobasilar stroke. Here, we reported a rare case of locked-in syndrome (LIS) due to bilaterial VAD after CSM treated by arterial embolectomy.A 36-year-old right-handed man was admitted to our hospital with numbness and weakness of limbs after treating with CSM for neck for half an ho…

BACKGROUND AND PURPOSE: Surgical treatment of VA-PICA dissecting aneurysms is difficult and complication of LCNP is common. These lesions can be approached easily with endovascular technique, but complete obliteration can only be achieved in a small number of cases. Our aim was to report the clinical outcomes of VA-PICA dissecting aneurysms treated by endovascular embolization.
MATERIALS AND METHODS: Between 2001 and 2007, the authors treated 22 consecutive patients (15 men and 7 women; ranging in age from 12 to 59 years; mean age, 43 years) with VA-PICA dissecting aneurysms. Diagnosis of VA-

A 57-year-old man presented with subarachnoid hemorrhage caused by a dissecting aneurysm of the vertebral artery close to the origin of the posterior inferior cerebellar artery (PICA). The aneurysm was treated successfully with two fenestrated clips preserving the efferent artery with anterograde blood flow without PICA anastomosis. The postoperative course was uneventful. Postoperative angiography indicated disappearance of the aneurysm and anterograde blood flow of the PICA.

A 54-year-old hypertensive woman was admitted with severe interscapular back pain. A chest radiograph showed marked widening of the mediastinum. Aortography demonstrated a DeBakey type III, a thoracic aortic dissection and an anomalous right subclavian artery which was associated with an isolated left vertebral artery. The patient underwent aortic arch replacement with 5 branches and made an uneventful recovery. As far as we can determine, this is the first reported occurrence of these anomalies together with acquired disease of the aorta.

INTRODUCTION: Intracranial vertebral artery dissecting aneurysms (VADA) may manifest as ischemic stroke. Although some individuals stabilize with conservative management, aneurysmal enlargement and recurrent embolic events may still occur, especially in partially thrombosed lesions. The role of flow diversion (FD) in ischemia-onset VADA remains unclear. This study reports the first focused experience evaluating FD in VADA with ischemic onset.
METHODS: Among 119 FD procedures performed between 2022 and 2025, four male individuals were treated for ischemia-onset VADA. All lesions were in the V4

Ruptured vertebral artery dissecting aneurysms (rVADAs) are life-threatening lesions with a high risk of rebleeding and brainstem infarction. This study describes our institutional experience with open surgical management of rVADAs, with a focus on perforator bypass strategies. We retrospectively analyzed 17 patients with rVADAs treated surgically between 2012 and 2025. Clinical data, imaging characteristics, surgical details, and outcomes were reviewed. Special attention was given to cases with perforator involvement and the use of bypass reconstruction. Clinical outcomes were assessed using

INTRODUCTION: Ruptured vertebral artery dissections frequently exhibit pearl-and-string signs, a characteristic also observed in many unruptured dissections. This study examines the natural course of 50 unruptured acute vertebral artery dissections presenting with pearl-and-string signs and compares them to 10 ruptured dissections of the same morphological pattern.
METHODS: We reviewed 633 radiology reports of head and neck imaging studies that included the keyword 'dissection'. From these, 60 cases displaying pearl-and-string signs were identified from a total of 322 vertebral artery dissect…
