USCAP Specialty Conference: case 1-type I pleuropulmonary blastoma.

Pleuropulmonary blastoma (PPB) was defined in 1988 by Manivel et al. in a series describing 11 intrathoracic pulmonary neoplasms in young children. The PPB is a unique peripheral pulmonary or pleural-based tumor of childhood that is characterized in its earliest form as a bland-appearing multiloculated cyst with small foci of tumor cells and in later forms as mixed and predominantly primitive, overtly malignant neoplasms. Prior to the introduction of the PPB as a distinct entity, this tumor had been reported in the literature as pulmonary blastoma, sarcoma arising in mesenchymal cystic hamarto

Transesophageal endoscopic resection of mediastinal cysts (with video).

BACKGROUND AND AIMS: Primary mediastinal cysts are infrequent lesions derived from a variety of mediastinal organs or structures. Complete surgical resection is the treatment of choice even in asymptomatic patients to prevent severe adverse events (AEs) and to establish the diagnosis. Transesophageal endoscopic resection of benign mediastinal tumors has been proven feasible. The aim of this study was to evaluate the feasibility, safety, and efficacy of transesophageal endoscopic surgery for mediastinal cysts. METHODS: From January 2016 to May 2021, patients with mediastinal cysts who underwen

Magnetic resonance imaging of the thorax in childhood. Work in progress.

Magnetic resonance (MR) imaging was performed in 10 children with intrathoracic disease. Entities imaged included mediastinal lymphoma and neuroblastoma, pulmonary metastases from hepatoblastoma and osteogenic sarcoma, hemorrhagic pleural effusion, and nonmalignant disease such as abscess, cystic fibrosis, and bronchogenic cyst. MR imaging was particularly valuable in delineating mediastinal and parenchymal masses from adjacent vascular structures without the need for contrast material enhancement. However, MR was insensitive to lesion calcification. Since pulmonary vessels give very low signa

CT characteristics of expansions in the middle and posterior mediastinum.

Chest radiographs and computed tomographic (CT) findings in 42 patients with pathology of the middle or posterior mediastinum, confirmed by thoracotomy, were examined retrospectively. The possibility of using contrast-enhanced CT to distinguish between cysts and solid tumors on the one hand and benign and malignant tumors on the other was assessed. All the expansions were detectable on chest radiographs. It was only by using CT, however, that the extent of the tumors could be established. Simple cysts could be identified because of their water density even without administration of contrast me

Endoscopic ultrasound-guided fine-needle aspiration in the diagnosis of foregut duplication cysts: the value of demonstrating detached ciliary tufts in cyst fluid.

BACKGROUND: The management of foregut duplication cysts is controversial, especially in asymptomatic patients. The safety and accuracy of endoscopic ultrasound (EUS) and EUS-fine-needle aspiration EUS-FNA) in confirming the nature of cysts by using electron microscopy (EM) has not been reported. In this study, the authors describe the utility of demonstrating detached ciliary tufts (DCTs) in the diagnosis of foregut duplication cysts with EUS-FNA. METHODS: Consecutive patients with suspected mediastinal masses or mediastinal cysts on imaging studies were evaluated prospectively by EUS and EUS

Mediastinal Cystic Teratoma in a Patient From Papua: A Rare Case Report.

Mediastinal teratomas are rare benign germ cell tumours that develop between the lungs, representing approximately 1%-5% of all mediastinal tumours. This case report presents a 41-year-old male patient from Papua with a history of chronic cough for 3 months and fatigue for about a month. Physical examination revealed decreased breath sounds in the right upper lung field, while laboratory tests were normal. A chest CT scan identified a benign cystic mass in the right anterior inferior mediastinum. Trans-thoracic CT-guided fine-needle aspiration with ROSE cytology suggested an infected cyst cont

[Tracheobronchial and pulmonary parenchymatous congenital abnormalities requiring surgical treatment in adults].

Most tracheobronchial and parenchymatous congenital abnormalities of the respiratory system are diagnosed in early life. However, some lesions may be initially silent and diagnosed only in adulthood. These cases included congenital abnormalies of the tracheobronchial tract (tracheal and/or bronchial stenosis, bronchogenic cysts, bronchial atresia, oesotracheal fistula, oesobronchial fistula, and tracheal diverticulum), and lung parenchyma itself (pulmonary sequestration, congenital cystic adenomatoïd malformation, lobar emphysema, lobar or lung hypoplasia). To avoid dreadful complications, the

[Congenital pulmonary malformations: clinical, radiological and treatment features].

BACKGROUND: Congenital broncho-pulmonary malformations (CBM) are rare, essentially presented by congenital lobar emphysema, bronchogenic cysts, pulmonary sequestrations and cystic adenomatoid malformations. The diagnosis can be in prenatal. In postnatal, symptoms are variable. Radiological investigations lead to diagnosis in all cases. AIM: To study the principal clinic, radiologic and therapeutic of the congenital broncho-pulmonary malformations through ten cases. METHODS: Retrospective study of 10 cases of congenital bronchopulmonary malformations diagnosed between 2003 and 2010 in our ins

Lateral supratentorial endodermal cyst: case report and review of literature.

OBJECTIVE AND IMPORTANCE: Epithelial cysts of the central nervous system are unusual entities, with the majority reported to occur in the spinal canal. More unusual is the intracranial presentation, which shows a predilection for midline localization in the posterior fossa, brainstem, and suprasellar regions. This report discusses the differential diagnosis, pathogenesis, radiographic presentation, and therapeutic considerations of a laterally positioned cerebral convexity endodermal cyst. CLINICAL PRESENTATION: A 49-year-old right-handed man presented with approximately a 30-month history of

Congenital midline cervical cleft. Case report and review.

Congenital midline cervical cleft is a rare disorder with a wide range of presentations. To date, there have been fewer than 35 cases reported in the English-language literature. A rarer finding is the association of bronchogenic cysts or respiratory epithelium with the midline cleft. The clinical presentation of congenital midline cervical cleft with a cephalic skin tab, atrophic cleft, and caudal sinus may distinguish it from other conditions of the midline neck. We present an 18-month-old girl with congenital midline cervical cleft to illustrate its clinical presentation and the proper trea

Rare gastroesophageal junction tumors or cysts of bronchial origin: A case report.

RATIONALE: Bronchogenic cysts (BCs) are rare congenital anomalies that often present without distinct clinical symptoms, complicating their diagnosis. However, as these cysts enlarge, they can cause symptoms due to compression of adjacent tissues and organs. In February 2024, a 32-year-old male was admitted to the Central Hospital of Suining City, Sichuan Province, China, with a preliminary diagnosis of a "gastrointestinal mesenchymal tumor." A thorough pathological evaluation ultimately confirmed the diagnosis as a BC. The cyst was successfully excised through surgical intervention. PATIENT

A congenial cutaneous bronchogenic sinus of the abdominal wall in an infant.

Cutaneous bronchogenic anomalies are very rare, and we report a child who presented with mucoid discharge from a small opening on her epigastrium that was present since birth. Diagnosis and treatment of congenital bronchogenic anomalies are important because surgical excision is needed to prevent the potential risk of infection and malignant degeneration. To the best to our knowledge, this is the first report of a congenital bronchogenic sinus of the abdominal wall.

[Lymphoepithelial cyst of the thyroid coexistent with amyloid goiter].

A case of amyloid goiter in a 73-year-old man with systemic amyloidosis complicating long lasting rheumatoid arthritis is reported. The thyroid gland was massively infiltrated with amyloid and contained foci of fat tissue and epithelial solid cell nests. Incidentally, a cyst lined by squamous epithelium, with small amount of lymphocytes in the subepithelial stroma was found. This cyst fulfilled the criteria for the diagnosis of a lymphoepithelial cyst presumably of branchiogenic origin. The herein described case of simultaneous occurrence of branchiogenic cyst and amyloid goiter represents, to

[Two cases of the bronchial cyst located in the anterior mediastinum].

Bronchial cysts are common cystic tumors around the tracheobronchial tree in the middle and posterior mediastinum and rarely locate in the anterior mediastinum. We reported two cases of the bronchial cyst located in the anterior mediastinum. One case was a 57 year-old-female. A thymic cyst was suspected and the extended total thymectomy was performed through the mediansternotomy. The microscopic examination showed bronchial epithelium and cartilage in the cystic wall. The another case was 71 year-old-male operated by thoracoscopic surgery for the cystic tumor in the anterior mediastinum. Micro

Primary cervical hydatid cyst: A rare case report.

INTRODUCTION AND IMPORTANCE: Hydatid cyst is a zoonotic parasitic disease caused by Echinococcus granulosis. Occurrence in the head and neck is quite uncommon even in endemic areas. The diagnosis of an isolated cystic neck mass is still a challenge due to the presence of similar congenital cystic lesions and benign tumors in the neck. Imaging is useful, but sometimes they cannot identify a definitive diagnosis. The treatment of choice is exclusively surgical excision, combined with chemotherapy. Histopathology confirms the definitive diagnosis. CASE PRESENTATION: We present a case of an 8-yea

Imaging of cystic masses of the mediastinum.

Cystic masses of the mediastinum are well-marginated round lesions that contain fluid and are lined with epithelium. Major cystic masses include congenital benign cysts (ie, bronchogenic, esophageal duplication, neurenteric, pericardial, and thymic cysts), meningocele, mature cystic teratoma, and lymphangioma. Many tumors (eg, thymomas, Hodgkin disease, germ cell tumors, mediastinal carcinomas, metastases to lymph nodes, nerve root tumors) can undergo cystic degeneration-especially after radiation therapy or chemotherapy-and demonstrate mixed solid and cystic elements at computed tomography (C

[Congenital Malformations of the Lung - an Overview].

Congenital pulmonary malformations comprise a heterogenous group of rare developmental diseases. The most common malformations are the tracheal bronchus, bronchial atresia, bronchogenic cyst, pulmonary sequestration, congenital lobar emphysema, and congenital pulmonary airway malformation. Due to their space-consuming effect, patients suffer early postnatal respiratory distress which generally requires immediate surgical resection. The management of asymptomatic lesions remains subject to debate, but early elective surgery is generally recommended to avoid respiratory and infectious complicati
