Visualization of Ebstein's anomaly of the tricuspid valve by two-dimensional and standard echocardiography.

Seven patients with Ebstein's anomaly were studied by two-dimensional echocardiography, with and without stop-action technique, and by standard echocardiography. Sagittal two-dimensional echocardiograms visualized a small functional right ventricle and a large atrialized right ventricle. These two parts of the chamber were separated by the anterior tricuspid leaflet and its chordae tendineae. The echocardiograms corresponded to the findings of the right ventriculogram. In horizontal two-dimensional echocardiograms a huge anterior tricuspid leaflet, atrialized right ventricle, and a displaced s

Prenatally diagnosed Ebstein's anomaly and tricuspid valve dysplasia: associated anomalies and predictors of outcome in a multicenter cohort study.

PURPOSE: To describe associated anomalies and outcomes of prenatally diagnosed Ebstein's anomaly (EA)/tricuspid valve dysplasia (TVD) in a multicenter cohort and to identify predictors of adverse outcome (intrauterine or postnatal death). MATERIALS AND METHODS: In this retrospective study, cardiothoracic ratio, aortic valve diameter (AVD), pulmonary and atrioventricular valve measurements, ventricular dimensions, vena contracta, chamber areas, Celemajer index, peak tricuspid regurgitation velocity (TR_max), and pulmonary flow direction between survivors and non-survivors were compared across

Outcomes of the Fontan operation in patients with Ebstein anomaly: An Australia and New Zealand Fontan registry study.

OBJECTIVES: We sought to review the outcomes of patients with Ebstein anomaly (EA) after the Fontan operation. METHODS: Patients with EA were identified from a large binational registry about the Fontan operation. Data were collected from hospital records, registry data, and clinical correspondence. RESULTS: Of the 1601 patients who underwent a contemporary Fontan operation from 1991 to 2023, 34 patients had EA. Seven patients (21%) had concomitant congenitally corrected transposition of great arteries. Prior Starnes procedure was performed in 18 (53%) patients. Survival after Fontan operati…

Echocardiographic characteristics of 14 cases with uncommon manifestation of Ebstein anomaly in the fetus: a case series report.

PURPOSE: This study was conducted to investigate the diagnostic characteristics of uncommon manifestation of Ebstein Anomaly (EA) during the fetal period, to improve the accuracy of prenatal diagnosis. METHODS: This retrospective analysis was conducted with data from fetuses diagnosed with an uncommon manifestation of EA between January 2020 and October 2024. Maternal and fetal information, prenatal ultrasound data, and information on pregnancy termination/delivery and postnatal surgical treatment were collected. RESULTS: The sample comprised 13 with isolated posterior leaflet displacement a…

Assessing Left Ventricular Pathology in Patients with Ebstein Anomaly Using Cardiovascular Magnetic Resonance: Looking Past the Right Heart.

Ebstein Anomaly (EA) is a malformation of the right heart, but there is data to suggest that the left ventricle (LV) can suffer from intrinsic structural and functional abnormalities which affect surgical outcomes. The LV in patients with EA is hypertrabeculated with abnormalities in LV function and strain. In this retrospective single-center study, patients with EA who underwent pre-operative cardiac MRI (CMR) between the periods of 2014-2024 were included along with a group of healthy-age-matched controls. Left ventricular and right ventricular volume, function and strain analyses were perfo…

Pharmacological resynchronisation with flecainide in an infant with Ebstein anomaly and Wolff-Parkinson-White pattern: a case report.

BACKGROUND: Conduction abnormalities are frequently encountered in patients with Ebstein anomaly. The following case describes the safe use of flecainide in an infant with accessory-pathway mediated left ventricular dysfunction in the setting of Ebstein anomaly. CASE SUMMARY: An infant with an antenatal diagnosis of Ebstein anomaly developed progressive left ventricular dilatation and dysfunction over the first 2 months of life. ECG demonstrated persistent Wolff-Parkinson-White pattern with delta-wave polarity suggesting a right-sided septal accessory pathway. In the absence of SVT, accessory…

Impact of fetal haemodynamics on surgical and neurodevelopmental outcomes in patients with Ebstein anomaly and tricuspid valve dysplasia.

OBJECTIVES: To evaluate the impact of fetal haemodynamics on surgical and neurodevelopmental outcomes in severe Ebstein anomaly and tricuspid valve dysplasia. METHODS: Thirty-four fetuses with Ebstein anomaly/tricuspid valve dysplasia were referred from 2013 to 2019 for fetal echocardiography and clinical management. Nineteen fetuses with Ebstein anomaly/tricuspid valve dysplasia and 30 controls underwent cardiovascular magnetic resonance to quantify the fetal blood flow and to calculate cerebral oxygen delivery (cDO2) and consumption (cVO2). The 3D steady-state free precession acquisition wa…

Examination of pathologic features of the right atrioventricular groove in hearts with Ebstein anomaly and correlation with arrhythmias.

BACKGROUND: Catheter ablation of accessory pathways (APs) in patients with Ebstein anomaly (EA) has a higher recurrence rate than in subjects with normal hearts. Anatomic features could account for suboptimal ablation outcomes. OBJECTIVE: The purpose of this study was to examine the right atrioventricular (AV) groove in autopsy hearts with EA, correlate with clinical data, and identify features relevant for catheter ablation. METHODS: Thirty-three specimens with EA from our Cardiac Registry were examined. The right AV groove was inspected for gross anatomic features. Limited microscopy was p…

Percutaneous closure of an atrial septal defect in a patient with Ebstein anomaly and right-to-left shunt.

Ebstein anomaly is a congenital disease frequently associated with atrial septal defects, which can generate a right-to-left shunt, leading to systemic desaturation and right ventricular failure. We describe the case of a 68-year-old man with central cyanosis due to Ebstein anomaly and a patent foramen ovale. An atrial septal occluder was initially implanted after having performed prolonged test occlusion of the interatrial communication. In this case, device embolization occurred due to high right pressure. Percutaneous closure of atrial septal defects in the presence of a right-to-left shunt…

Detection of right ventricle thrombosis in patient with Ebstein anomaly of tricuspid valve after Fontan procedure by CT.

A case of a 9-year-old boy with a severe form of Ebstein anomaly who underwent a fenestrated Fontan procedure and exclusion of the tricuspid valve is reported. CT demonstrated the presence of the right ventricular thrombus which was first found on echocardiography and confirmed perioperatively.

Cone reconstruction of the tricuspid valve in Ebstein anomaly with or without one and a half ventricle repair.

OBJECTIVE: We sought to review our institutional experience and midterm results with cone reconstruction in Ebstein anomaly with or without one and a half ventricle repair to evaluate these 2 important surgical methods for the treatment of Ebstein anomaly. METHODS: From January 1, 2004, to October 1, 2009, 30 consecutive patients with Ebstein anomaly underwent cone reconstruction. All patients underwent cone reconstruction, and 20 patients with severe Ebstein anomaly had a bidirectional cavopulmonary shunt constructed. The median age was 60 months (range, 2-192 months). Our criteria to define…
