Occurrence of pneumothorax during noninvasive positive pressure ventilation through a helmet.

A 79-year-old woman presented with hypoxemic acute respiratory failure secondary to pneumonia and was started on continuous noninvasive positive pressure ventilation (NPPV) by helmet. Patient improved over first two days of NPPV, but worsened suddenly on the third day because of development of a pneumothorax. Pneumothorax may have been caused by barotrauma from desynchronization between patient and ventilator.

Pediatric all-terrain vehicle-related injuries in Ohio from 1995 to 2001: using the injury severity score to determine whether helmets are a solution.

OBJECTIVE: The goal was to identify regions in Ohio with severe pediatric all-terrain vehicle-related injuries and to determine whether helmet usage was associated with lower injury severity scores. METHODS: We performed a retrospective review of data for all patients entered into the registries of Ohio's major pediatric trauma centers for the period of January 1, 1995, to December 31, 2001. RESULTS: Seven hospitals participated. A total of 285 children were admitted; 2 patients died, and 13 required rehabilitation. The mean age was 11.1 years, with 76.1% of patients being male and 88.1% whi

Helmet use and risk of head injuries in alpine skiers and snowboarders.

CONTEXT: Although using a helmet is assumed to reduce the risk of head injuries in alpine sports, this effect is questioned. In contrast to bicycling or inline skating, there is no policy of mandatory helmet use for recreational alpine skiers and snowboarders. OBJECTIVE: To determine the effect of wearing a helmet on the risk of head injury among skiers and snowboarders while correcting for other potential risk factors. DESIGN, SETTING, AND PARTICIPANTS: Case-control study at 8 major Norwegian alpine resorts during the 2002 winter season, involving 3277 injured skiers and snowboarders report

Cleidocranial dysplasia in a mother and her daughter within the scope of neurosurgery. Report of two cases.

Cleidocranial dysplasia is an autosomal-dominant disorder characterized by late closure or nonclosure of the anterior fontanelle, late ossification of cranial sutures, defective clavicle, and delayed eruption of permanent teeth. In this article, two cases of cleidocranial dysplasia involving a mother and her daughter are reported, and a case management policy is suggested. The 1-year-old daughter was macrocephalic and brachycephalic, and had midface hypoplasia and hypertelorism. Plain radiographs revealed aplasia of the clavicles. Three-dimensional computerized tomography scanning demonstrated

Infant cranial molding deformation and sleep position: implications for primary care.

Infant positional plagiocephaly, a cranial molding deformity expressed by asymmetrical head shape, is on the rise. The increase correlates with the recommendation by the American Academy of Pediatrics that infants be placed on their backs to sleep to reduce the risk of sudden infant death syndrome. The majority of misshapen heads develop because infants are placed to sleep in the same supine position, without head rotation. Infant molding deformities will generally improve with repositioning and cranial growth, but permanent deformation can occur, especially without early treatment. This artic

Helmet use among competitive cyclists.

The United States Cycling Federation (USCF) decision in 1986 to mandate helmet use in all sponsored races marked a major initiative in cycling safety. Confirming earlier reports about the effectiveness of helmets in preventing injuries, this study also examines the attitudes of 554 USCF members toward the policy and about helmet use in both racing and nonracing situations. Although 64% of the racers reported some hardshell helmet use in training before the policy, 80% used helmets in training after the ruling. Most cyclists favored the USCF policy, although only 19% favored requiring helmet us

Cyclists' clothing and reduced risk of injury in crashes.

INTRODUCTION: A majority of cyclists' hospital presentations involve relatively minor soft tissue injuries. This study investigated the role of clothing in reducing the risk of cyclists' injuries in crashes. METHODS: Adult cyclists were recruited and interviewed through hospital emergency departments in the Australian Capital Territory. This paper focuses on 202 who had crashed in transport related areas. Eligible participants were interviewed and their self-reported injuries corroborated with medical records. The association between clothing worn and injury was examined using logistic regres

Individualized 3D-Printed Tissue Retraction Devices for Head and Neck Radiotherapy.

BACKGROUND: Radiotherapy for head and neck cancer may cause various oral sequelae, such as radiation-induced mucositis. To protect healthy tissue from irradiation, intraoral devices can be used. Current tissue retraction devices (TRDs) have to be either individually manufactured at considerable cost and time expenditure or they are limited in their variability. In this context, a 3D-printed, tooth-borne TRD might further facilitate clinical use. METHODS: A novel approach for the manufacturing of TRDs is described and its clinical application is analysed retrospectively. The devices were virtu

Protective device to reduce aerosol dispersion in dental clinics during the COVID-19 pandemic.

AIM: To describe the use of a new protective device to reduce aerosol dispersion in dental clinics during the COVID-19 pandemic. METHODOLOGY: The device consists of a rigid translucent acrylic structure (methyl polymethacrylate), adjusted to the dental chair, involving the patient's head, neck and chest. There is also a piping system to generate negative pressure, for aspiration and filtering of the air inside the device chamber. The operator works through small holes in the acrylic structure, to reduce contact with the microparticles arising from aerosols during dental procedures. Simulated

[Design of a head auxiliary support device in prone position].

Prone position ventilation (PPV) is an important protective strategy for lung ventilation, widely used in clinical practice, especially since the novel coronavirus infection pandemic. Since PPV is a non-physiological position, improper implementation and management can lead to serious adverse events such as pressure injury, facial edema, unplanned extubation and (or) reintubation, and even asphyxia. At present, preventive and protective strategies are mainly used to manage PPV-related complications in clinical practice. These strategies not only increase the workload of medical staff and the u

Current helmet and protective equipment usage among previously injured ATV and motorcycle riders.

Injury recidivism among trauma patients may be related to an individual pattern of high risk behaviors. The extent to which an injury episode modifies this behavior pattern is unknown. A self report, voluntary, anonymous, cross sectional survey was administered to motorcycle and all-terrain vehicle (ATV) riders at a popular recreation site. Data included demographics, injury history, and current usage of helmet and protective gear. Two hundred eighty surveys were completed. History of ATV/motorcycle related minor and major injury were reported by 21% and 9%, respectively. Persons with a histor

Driver Cooling Strategies to Mitigate Heat Illness in Motorsports: Physiological Impacts and Recommendations.

INTRODUCTION: Each year, over 1 million individuals participate in automobile racing and are at an increased risk of heat illness due to the physical effort to pilot the vehicle while wearing a fire protective suit in a hot cockpit. To date, there has been no investigation into the various methods advertised to mitigate heat stress in drivers. PURPOSE: This study aimed to evaluate the effectiveness of three driver cooling strategies-cool shirt, helmet blower, and suit blower-during heat exposure, and to determine their impact on thermal stress in a laboratory setting. METHODS: Physically fit

Head Kinematics and Injury Metrics for Laboratory Hockey-Relevant Head Impact Experiments.

Investigating head responses during hockey-related blunt impacts and hence understanding how to mitigate brain injury risk from such impacts still needs more exploration. This study used the recently developed hockey helmet testing methodology, known as the Hockey Summation of Tests for the Analysis of Risk (Hockey STAR), to collect 672 laboratory helmeted impacts. Brain strains were then calculated from the according 672 simulations using the detailed Global Human Body Models Consortium (GHBMC) finite element head model. Experimentally measured head kinematics and brain strains were used to c

Soft-helmet skull remodeling in canine models: intracranial volume unchanged by compensatory skull growth.

OBJECT: Habitual sleeping positions in infants can produce occipital plagiocephaly, which causes strabismus as well as skull and facial asymmetry. This condition can be managed using a hard helmet, but maintaining an infant in such a device is often intolerable. The authors studied whether the shape of a young canine skull could reform while inside a soft helmet and whether intracranial volume would be preserved by the compensatory growth of the skull. METHODS: The authors tracked the head sizes of 14 1-week-old beagles who wore long, soft helmets (study group) and seven beagles who did not (
