Evidence of Stem Cell Entrapment in AN69 Membranes During Continuous Renal Replacement Therapy.

BACKGROUND: Continuous renal replacement therapy (CRRT) is increasingly employed in pediatric intensive care units (PICUs) for managing both renal and non-renal conditions, including sepsis through cytokine removal. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a critical treatment for inherited bone marrow syndromes but poses challenges when performed alongside extracorporeal therapies like CRRT. This case explores the interaction between hematopoietic stem cells and AN69 membranes during CRRT in a pediatric patient undergoing allo-HSCT amid septic shock. METHODS: A 10-ye

Unraveling the impact of renal impairment and renal replacement therapy modalities on fluconazole pharmacokinetics.

BACKGROUND: Fluconazole is a triazole antifungal agent predominantly eliminated by renal excretion, making its pharmacokinetics (PK) highly susceptible to changes in renal function. PURPOSE: This study aimed to systematically investigate how varying degrees of renal impairment and the use of renal replacement therapy (RRT) modalities-including continuous veno-venous hemofiltration (CVVHF), hemodiafiltration (CVVHDF), and peritoneal dialysis (PD)-alter the PK behavior of fluconazole. By quantitatively comparing key PK parameters across healthy individuals, critically ill patients, and those un

Effect of Low-Sodium versus Conventional Sodium Dialysate on Left Ventricular Mass in Home and Self-Care Satellite Facility Hemodialysis Patients: A Randomized Clinical Trial.

BACKGROUND: Fluid overload in patients undergoing hemodialysis contributes to cardiovascular morbidity and mortality. There is a global trend to lower dialysate sodium with the goal of reducing fluid overload. METHODS: To investigate whether lower dialysate sodium during hemodialysis reduces left ventricular mass, we conducted a randomized trial in which patients received either low-sodium dialysate (135 mM) or conventional dialysate (140 mM) for 12 months. We included participants who were aged >18 years old, had a predialysis serum sodium ≥135 mM, and were receiving hemodialysis at home or

Early High-Volume Hemofiltration versus Standard Care for Post-Cardiac Surgery Shock. The HEROICS Study.

RATIONALE: Post-cardiac surgery shock is associated with high morbidity and mortality. By removing toxins and proinflammatory mediators and correcting metabolic acidosis, high-volume hemofiltration (HVHF) might halt the vicious circle leading to death by improving myocardial performance and reducing vasopressor dependence. OBJECTIVES: To determine whether early HVHF decreases all-cause mortality 30 days after randomization. METHODS: This prospective, multicenter randomized controlled trial included patients with severe shock requiring high-dose catecholamines 3-24 hours post-cardiac surgery

Improvement of Normothermic Ex Vivo Machine Perfusion of Rat Liver Grafts by Dialysis and Kupffer Cell Inhibition With Glycine.

Normothermic ex vivo liver machine perfusion might be a superior preservation strategy for liver grafts from extended criteria donors. However, standardized small animal models are not available for basic research on machine perfusion of liver grafts. A laboratory-scaled perfusion system was developed consisting of a custom-made perfusion chamber, a pressure-controlled roller pump, and an oxygenator. Male Wistar rat livers were perfused via the portal vein for 6 hours using oxygenated culture medium supplemented with rat erythrocytes. A separate circuit was connected via a dialysis membrane to
