Antioxidant Support for the Corneal Endothelium: Evidence from Vitamin C Supplementation in Hard Cataract Phacoemulsification.

BACKGROUND: Phacoemulsification is one of the most frequent surgeries in the world. However, prolonged use of phacoemulsification machines produces reactive oxygen species which will damage corneal endothelial cells. Ascorbic acid has an antioxidant capacity to neutralize oxidative stress in the anterior chamber. This study will investigate the protective effect of ascorbic acid on corneal endothelial cells in patients with hard nuclear cataracts. METHODS: This study is a double-blinded randomized controlled trial. Samples will be divided into three groups, and 500 mg vitamin C three times da…

Comparative Analysis of Corneal Endothelial Cell Loss After Phacoemulsification in Microcornea versus Normal-Sized Corneas.

BACKGROUND: Phacoemulsification is inevitably accompanied by loss of corneal endothelial cells, and microcornea eyes with crowded anterior segments may be more susceptible to this damage. PURPOSE: This study aimed to compare endothelial cell loss (ECL) after cataract phacoemulsification surgery between eyes with microcornea and those with normal-sized corneas. METHODS: This retrospective study included 50 eyes from 50 patients with microcornea and 120 eyes from 120 patients with normal-sized corneas who underwent phacoemulsification. Demographic data and biometric parameters were evaluated p…

Bilateral Rhegmatogenous Retinal Detachment After Uneventful Phacoemulsification Cataract Surgery: A Case Report and Literature Review.

A 72-year-old man developed a non-simultaneous bilateral rhegmatogenous retinal detachment (RRD), three months after bilateral sequential uneventful phacoemulsification cataract surgery performed through a 2.75 mm clear cornea incision by the same high-volume cataract surgeon. The patient was of Indian background and a high myope. Meticulous retinal evaluation had been performed preoperatively, and no retinal holes, tears, or other predisposing retinal abnormalities were discovered. Notably, posterior vitreous detachment (PVD) was absent bilaterally prior to cataract surgery but developed post…

Full-thickness macular hole after uneventful phacoemulsification cataract surgery: Case report and review of literature.

A 61-year-old healthy female patient underwent an uneventful left phacoemulsification procedure under local anesthesia for a left visually significant senile cataract. One month postoperatively, her decimal visual acuity improved from 0.06 preoperatively to 0.5. However, 3 months postoperatively, the best-corrected visual acuity (BCVA) had decreased to 0.3. Fundus examination revealed the development of a full-thickness macular hole (FTMH). Diagnosis was confirmed by optical coherence tomography of the macula, which demonstrated FTMH. The patient subsequently underwent pars plana vitrectomy wi…

Subconjunctival injection of microcrystalline prodrug of dexamethasone for long-acting anti-inflammation after phacoemulsification surgery.

Long-acting injectable formulations of dexamethasone with minimal invasiveness are highly desired to manage chronic ocular inflammatory conditions. Here, we applied microcrystals (MCs) of a hydrophobic acetone-based ketal-linked prodrug of dexamethasone (SKD) to treat postoperative ocular inflammation. We compared the efficacy and safety of SKD MCs through subconjunctival (SC) injection with that of Maxidex (a topical suspension of dexamethasone MCs) through SC injection and eye drops in the phacoemulsification combined with intraocular lens implantation (Phaco-IOL) rabbit model. In Phaco-IOL …

Aphakic Pupillary Block by an Intact Anterior Vitreous Membrane after Total Lens Extraction by Phacoemulsification.

An 85-year-old Japanese woman with acute primary angle closure in her right eye underwent cataract extraction. Because of the weakness of the Zinn's zonules, all of the lens tissue including the lens capsule was removed by phacoemulsification. Because of the absence of vitreous prolapse into the anterior chamber, vitrectomy was not performed. Nine days postoperatively, acute angle closure due to pupillary block by an anterior vitreous membrane developed. To resolve the pupillary block, anterior vitrectomy was performed on the same day. Postoperatively, her symptoms resolved, the anterior chamb…
