[Is primary, secondary or tertiary prevention of coronary heart disease possible by means of oestrogens or gestagens?].

Clinical trials in primary prevention of coronary atherosclerosis have demonstrated that estrogen application in postmenopausal women may reduce the relative risk by approximately 50 % when initiated at the time of decreasing endogenous estradiol production. Multifactorial estrogen actions (i. e. vasodilatation, inhibition of vascular smooth muscle cell proliferation) could explain these observations. Based on some investigations in secondary and tertiary prevention of coronary atherosclerosis in postmenopausal women it may be concluded that oestrogens alone or in combination with certain prog…

Prevalence and detection of HIV risk behavior in primary care: implications for clinical preventive services.

Despite their potential to reduce the incidence of HIV infection through primary prevention, family physicians report low levels of routine identification of patients at risk and counseling. This may reflect perceptions that few of their patients are at risk, that patients at risk will self-disclose during consultations, or that a physician-initiated approach is unacceptable to patients presenting for non-HIV-related problems. Our aim was to determine the prevalence of risk factors for HIV infection and HIV testing among patients in general practice, the acceptability to patients of opportunis…

Temporal evolution of clinical event risk after primary-prevention ICD implantation: Chagas vs. non-Chagas dilated cardiomyopathy.

Chagas cardiomyopathy evolves through progressive myocardial injury that may alter the temporal distribution of arrhythmic risk compared with other dilated substrates. We analysed a two-centre registry of consecutive primary-prevention implantable cardioverter-defibrillator (ICD) recipients in Buenos Aires (2008-2025), excluding cardiomyopathies in which ICD indication is not based on ventricular dysfunction. The final cohort comprised 194 patients: 50 with Chagas cardiomyopathy (26%), 83 with ischaemic (43%) and 61 with non-ischaemic dilated cardiomyopathy (31%). Median LVEF was 30% in both g

Predictors and Prognostic Significance of Appropriate Implantable Cardioverter-Defibrillator Therapy in Primary Prevention Patients with Ischemic Cardiomyopathy.

Background: In the population of patients with ischemic cardiomyopathy (IC) and reduced left ventricular ejection fraction, the benefits of prophylactic implantable cardioverter-defibrillator (ICD) therapy are not uniform. Identifying predictors of ventricular arrhythmias to estimate the risk of appropriate therapy is crucial. Methods: Patients with IC and an ICD for primary prevention implanted between 2006 and 2019 were retrospectively analyzed for appropriate therapy (ATh). The primary objective was to assess predictors of ATh development. The secondary objective was to assess the impact of…

Lower-Cost Nonsurgical Primary Prevention Strategies for Women at High Risk of Breast Cancer: A Scoping Review.

BACKGROUND: A systematic mapping of nonsurgical, lower-cost primary prevention strategies for women at high risk of breast cancer remains limited. OBJECTIVE: To identify and summarize lower-cost (i.e., lower economic cost relative to surgical and pharmaceutical alternatives; not dependent on expensive equipment, patented drugs, or complex medical procedures; implementable through community, family, or self-management models), nonsurgical primary prevention strategies for women at high risk of breast cancer, providing empirical evidence to inform clinical practice, health education, and public…

Cost-Effectiveness of Prehospital Delay Reduction Versus Primary Stroke Prevention in US Adults With Type 2 Diabetes.

BACKGROUND: Much attention has focused on reducing prehospital delay among patients with acute ischemic stroke (AIS) to facilitate delivery of highly effective but time-sensitive treatments. The relative benefits and cost-effectiveness of minimizing treatment delays versus more fully implementing primary prevention measures for AIS are unclear in high-risk populations, including US adults with type 2 diabetes. METHODS: We used data from the National Health and Nutrition Examination Survey (2015-2018) to identify US adults ≥45 years of age with type 2 diabetes and no prior stroke (N=1232). Str…

Network intervention analysis of a school-based gaming disorder prevention program in Hong Kong primary school students.

BACKGROUND AND AIMS: The growing prevalence of gaming disorder has prompted the emergence of early prevention programs. While previous evaluations assessed overall efficacy, the specific symptom-level changes following program participation remain unknown. This study addressed the gap using network intervention analysis (NIA) to investigate how a newly validated prevention program, the Digital Netizen Alliance, was associated with changes in the network components of gaming disorder symptoms. METHODS: In a school-clustered design, a sample of 600 Chinese Primary 4 to 6 students (48% female, a

Understanding the Rising Incidence of Early-Onset Colorectal Cancer: Life-Course Determinants and Opportunities for Primary Prevention.

Background/Objectives: Early-onset colorectal cancer (EOCRC), diagnosed before age 50 years, is increasing in many countries, but its causes … sk factors are plausible explanations but remain incompletely tested. Evidence supports life-course primary prevention, but no single factor should determine screening eligibility in isolation. Prospective studies with repeated exposure measurement are needed.

Polygenic Risk Identifies Older Adults Who May Benefit From Aspirin for the Primary Prevention of Ischemic Stroke.

BACKGROUND: Low-dose aspirin is no longer recommended for routine primary prevention in older adults due to bleeding risks outweighing vascular benefits. We hypothesized that an integrative polygenic score (iPGS) could identify a subgroup of older individuals who derive net benefit from aspirin for the primary prevention of ischemic stroke. METHODS: We performed post hoc analysis of the ASPREE randomized, placebo-controlled trial (Aspirin in Reducing Events in the Elderly) of daily 100-mg aspirin, in 12 031 genotyped participants of European ancestry aged >70 years without prior cardiovascula…

Refining risk stratification for primary prevention ICD therapy: Insights from cardiac MRI in the DO-IT registry.

BACKGROUND: Implantable cardioverter-defibrillators (ICDs) are recommended for the primary prevention of sudden cardiac death in patients with reduced left ventricular ejection fraction (LVEF). However, reliance on LVEF alone may inadequately identify patients at the highest risk. OBJECTIVE: This study aimed to evaluate the prognostic value of cardiac magnetic resonance imaging (CMR), particularly late gadolinium enhancement (LGE), in patients with ischemic cardiomyopathy (ICM) and nonischemic dilated cardiomyopathy (NIDCM). METHODS: This multicenter substudy of the Dutch Outcome in ICD Ther…

Lipoprotein(a) Atherosclerotic Cardiovascular Disease Risk Score Development and Prediction in Primary Prevention From Real-World Data.

BACKGROUND: Lipoprotein(a) [Lp(a)] is a predictor of atherosclerotic cardiovascular disease (ASCVD); however, there are few algorithms incorporating Lp(a), especially from real-world settings. We developed an electronic health record (EHR)-based risk prediction algorithm including Lp(a). METHODS: Utilizing a large EHR database, we categorized Lp(a) cut points at 25, 50, and 75 mg/dL and constructed 10-year ASCVD risk prediction models incorporating Lp(a), with external validation in a pooled cohort of 4 US prospective studies. Net reclassification improvement was determined among borderline-i

Primary, secondary and tertiary prevention of long-term benzodiazepine receptor agonists use in Belgium: a policy Delphi.

BACKGROUND: The long-term use of benzodiazepine receptor agonists (BZRAs) poses a significant public health challenge in Belgium because of the associated risks of physical and psychological dependence. Despite guidelines recommending short-term use, BZRAs are frequently prescribed beyond the recommended duration, leading to chronic use and associated harm. To address this issue, a policy Delphi study was conducted to assess targeted strategies for preventing long-term BZRA use through the lenses of primary, secondary, and tertiary prevention. METHODS: The study involved a panel of experts, i

Primary Prevention Strategy for Non-Communicable Diseases (NCDs) and Their Risk Factors: The Role of Intestinal Microbiota.

Non-communicable diseases (NCDs) are the leading cause of morbidity and mortality worldwide. These conditions have numerous health consequences and significantly impact patients' lifestyles. Effective long-term treatment is essential since NCDs are irreversible. Therefore, primary healthcare must be both exclusive and of the highest quality, ensuring comprehensive care. The primary goal should be to improve quality of life with a focus on patients, families, and communities, as most of these diseases can be prevented and controlled, although not cured. Several factors have been linked to indiv
