Spasm of the near reflex in a patient with multiple sclerosis.

A 33-year-old woman with a history of relapsing-remitting multiple sclerosis presented with complaints of episodic horizontal diplopia and blurred vision over the past two months. The patient gave a past history of similar episodes of diplopia that had occurred with previous clinical relapses of her multiple sclerosis. On eye movement examination, the patient exhibited intermittent spasms of convergence coupled with miosis. On upward gaze, the convergence and miosis became more marked. An auto-refraction revealed variable measurements in both eyes, consistent with accommodative spasms. A cyclo

Brainstem mediation of the stapedius muscle reflex in hydranencephaly.

Hydranencephaly is a rare condition in which the cerebral hemispheres are absent at birth and are replaced by membranous sacs in a cerebrospinal fluid-filled cranium. Surviving hydranencephalic patients have a functional brainstem and possible remnants of the cerebral cortex. This case report examines hearing function and the integrity of the brainstem mediated stapedius muscle reflex (SMR) in an adult with hydranencephaly, using middle ear impedance change measures. The brainstem mediated ipsilateral and cross-brainstem contralateral SMRs were elicited in the right ear at normal threshold lev…

Brainstem excitability is increased in subjects with palmomental reflex.

BACKGROUND/PURPOSE: The palmomental reflex (PMR) is a brief contraction of the mentalis muscles caused by a scratch over the thenar eminence, i.e. a brainstem reflex to afferents of upper limb. Using electrophysiologic methods, we studied the characteristics of brainstem excitability in PMR subjects. METHODS: Ten healthy PMR subjects were included in the study. Brainstem excitability was assessed with electrical stimulation at the trigeminal nerve, median nerve, ulnar nerve, and sural nerve with recordings at the mentalis muscles. A comparison was made by the probability between the mechanica…

Orthostatic hypertension: when pressor reflexes overcompensate.

Orthostatic hypertension--a rise in blood pressure upon assuming upright posture-is an underappreciated and understudied clinical phenomenon. There is currently no widely agreed-upon definition of clinical orthostatic hypertension, the current definitions being operational within the context of particular studies. The underlying pathophysiology is thought to involve activation of the sympathetic nervous system, but the actual etiology is poorly understood. Orthostatic hypertension is observed in association with a variety of other clinical conditions, including essential hypertension, dysauton

Genioglossus premotoneurons and the negative pressure reflex in rats.

Reflex increases in genioglossus (GG) muscle activity in response to negative pharyngeal pressure are important for maintenance of upper airway patency in humans. However, little is known of the central circuitry that mediates this negative pressure reflex (NPR). We used two approaches to determine which GG premotoneurons relay negative pressure-related information to the hypoglossal motor nucleus. First, to identify GG premotoneurons, we injected pseudorabies virus (PRV152) into the GG muscle. We found that medullary GG premotoneurons were concentrated mainly in the reticular formation adjace…

Hyperresponsiveness of congestive nasal reflexes in allergic rhinitis.

BACKGROUND: Nasal secretory hyperresponsiveness is well documented in allergic rhinitis, and is mediated in part by neural mechanisms. In contrast, reflex-mediated congestion is poorly documented in both normal and allergic subjects. OBJECTIVE: To characterize congestive responses to unilateral nasal bradykinin challenge in normal and allergic subjects, and to investigate whether congestive hyperresponsiveness is present in allergic rhinitis. METHODS: Normal subjects (n = 13), and subjects with out-of-season seasonal allergic rhinitis (SAR) (n = 16) underwent a unilateral nasal challenge pro…

Differential modulation of withdrawal reflexes by a cannabinoid in the rabbit.

Inhibition of spinal and trigeminal withdrawal reflexes by morphine and by the cannabinoid agonist HU 210 has been studied in anaesthetized and in decerebrated rabbits. In intact, pentobarbitone-anaesthetized animals, the jaw-depressor reflex (JDR) evoked by stimulation of the tongue, and the reflex elicited in the ankle flexor tibialis anterior (TA) by stimulation of the toes were inhibited to the same extent by morphine (1-30 mg kg(-1) i.v. cumulative). In spinalized, anaesthetized rabbits morphine depressed the JDR to the same level as in non-spinal preparations, but the effect of the opioi…

Primitive reflexes and postural reactions in the neurodevelopmental examination.

The primitive reflexes and the postural reactions comprise one of the earliest, simplest, and most frequently used tools among child neurologists to assess the central nervous system integrity of infants and young children. Infants with cerebral palsy have been known to manifest persistence or delay in the disappearance of primitive reflexes and pathologic or absent postural reactions. The clinical significance of asymmetric tonic neck reflex, Moro, palmar grasp, plantar grasp, Galant, Babinski, Rossolimo, crossed extensor, suprapubic extensor, and heel reflex, alone or in combination, as well…

Control of volitional and reflexive saccades in Tourette's syndrome.

We hypothesized that Tourette's syndrome (TS) patients would display abnormal control of saccades because of overlap between brain areas suspected in TS pathophysiology and those involved saccade control. Subjects were required to look toward (pro-saccade) or away from (anti-saccade) a visual target. Saccadic reaction times were elevated among TS subjects in all tasks. The occurrence of reflexive pro-saccades in the immediate anti-saccade task was normal, suggesting that the ability to inhibit reflexive saccades was not impaired in TS. However, timing errors (eye movements made prior to GO sig…

The orientating reflex: the "targeting reaction" and "searchlight of attention".

A concept of the orientating reflex is presented, based on the principle of vector coding of cognitive and executive processes. The orientating reflex is a complex of orientating reactions of motor, autonomic, and subjective types, accentuating new and significant stimuli. Two main systems form the orientating reflex: the "targeting reaction" and the "searchlight of attention:" In the visual system, the targeting reaction ensures that the image of the object falls onto the fovea; this is mediated by involvement of premotor neurons which are excited by saccade command neurons in the superior co…

Reflexes from airway rapidly adapting receptors.

Rapidly adapting receptors (RARs) occur throughout the respiratory tract from the nose to the bronchi. They have thin myelinated nerve fibres, an irregular discharge and adapt rapidly to a maintained volume stimulus, but often slowly to a chemical stimulus. They are polymodal, responding to mechanical and chemical irritant stimuli, and to many inflammatory and immunological mediators. RARs show very varied sensitivities to different stimuli, and diverse reflex responses. Those in the larynx are usually called 'irritant' receptors. They probably cause cough, the expiration reflex and other lary…

Electrophysiological recording of tendon reflexes in cervical myelopathy.

Twenty-six patients with unequivocal, clinically and radiologically confirmed cervical myelopathy were investigated electrophysiogically using needle electromyography (EMG), motor and sensory conduction velocity, F-wave latency and the electromyographic recording of the tendon reflexes (TR) of the biceps brachii, brachioradialis and triceps brachii muscles. Normal latency and amplitude values of the reflex evoked compound muscle potentials (T-wave) were calculated from 74 healthy volunteers. Pathological TR were found in 73.1% of the patients, while EMG--which was the next more effective metho…

The C fibre reflex of the cat urinary bladder.

1. Reflexes evoked in bladder parasympathetic neurones by electrical stimulation of bladder C afferent fibres were studied in cats anaesthetized with alpha-chloralose. The responses were compared with the ordinary micturition reflex evoked by low-threshold Adelta afferents from bladder mechanoreceptors and mediated by a spino-ponto-spinal reflex pathway. 2. The bladder was catheterized for fluid instillations and pressure recordings. Efferent reflex discharges were recorded from the cut central end of a small distal bladder branch of the pelvic nerve. The remaining bladder pelvic nerve branche…

[The normal value and clinical applications of the blink reflex].

OBJECTIVE: To study the normal value and clinical applications of blink reflex. METHODS: Using the clinical electrodiagnostic techniques, the orbital nerve was stimulated to elicit the blink reflex. RESULTS: In 20(40 sides) normal subjects, two components (R1 and R2) had been documented. The latencies were about 10.1 ms for R1, 30.4 ms and 30.7 ms separately for R2 and R2'. In 30 patients with trigeminal and facial nerve diseases and acoustic neuromas, the latencies on the affected sides were apparently longer or the reflex disappeared (P < 0.01). CONCLUSION: This study indicates that the s…

A brainstem network mediating apneic reflexes in the rat.

Apnea is an important protective response to upper airway irritation, but the central mechanisms responsible for eliciting sensory-induced apnea are not well understood. Recent studies have emphasized the Kölliker-Fuse nucleus in producing apnea and proposed a trigeminoparabrachial pathway for mediating these reflexes. However, in our earlier study of apneic responses produced by glutamate stimulation in the dorsolateral pons, we found that apnea was elicited from the area just ventral to the Kölliker-Fuse nucleus, rather than within it. Because this region was not known to be involved in resp…

Validity of the ipsilateral acoustic reflex as a screening parameter.

Historically, the ipsilateral acoustic reflex (IAR) has played a prominent role in middle ear screening protocols. However, in 1990, the American Speech-Language-Hearing Association's revised guidelines, "Screening for Hearing Impairment and Middle Ear Disorders," deleted the IAR as a screening parameter. This decision was based on studies that demonstrated that the inclusion of the IAR resulted in a high false-positive rate without adding to the hit rate and thus was a major contributor to over-referral. However, other investigations demonstrated the converse, that the IAR is a sensitive and …
