Evidence that an internal schema adapts swallowing to upper airway requirements.

KEY POINTS: To swallow food and liquid safely, airway protection is essential. Upward and forward movements of the hyoid and larynx in the neck during swallowing vary in magnitude between individuals. In healthy human adults, hyoid and laryngeal movements during swallowing were scaled by differences in initial upper airway area before swallowing. Individuals increased laryngeal elevation during swallowing in response to increased airway opening before swallowing. We show that when upper airway protection requirements change, individuals use an internal sensorimotor scaling system to adapt move

Anatomical Directional Dissimilarities in Tri-axial Swallowing Accelerometry Signals.

Swallowing accelerometry is a noninvasive approach currently under consideration as an instrumental screening test for swallowing difficulti … mfortable self-selected volume from a cup or a controlled volume by the examiner from a 5-ml spoon. Swallows were grouped based on the viscosity of swallows and the participant's stroke history. Then, a comprehensive set of features was extracted in multiple signal domains from 881 swallows. The results highlighted inter-axis dissimilarities among tri-axia …

Effects of wearing and removing dentures on oropharyngeal motility during swallowing.

Wearing dentures and dysphagia are common in older individuals; however, it is still unknown how dentures affect oral and pharyngeal swallowing. The purpose of this study was to reveal the effects of wearing and removing dentures on oropharyngeal movements during pharyngeal swallowing in the feeding sequence of solid food. Participants were 25 edentulous volunteers (nine men, 16 women; mean age 76·2 years) who wore complete dentures. The test food was minced agar jelly containing barium sulphate. Subjects were instructed to feed and swallow the test food with or without dentures during observa

Tongue pressure production during swallowing in patients with mandibular prognathism.

UNLABELLED: Abnormalities of swallowing in patients with mandibular prognathism have not been evaluated quantitatively. The aim of this study was to compare tongue pressure production for bolus transfer between volunteers with normal occlusion and patients with mandibular prognathism. The control group had 10 female volunteers with normal occlusion, and the patient group had 10 women with mandibular prognathism. Tongue pressure was measured by a palatal sensor sheet at five sites on swallowing 4 mL of a tasteless and odourless jelly. RESULTS: The tongue pressure waveform differed between the

[The study of tongue pressure during swallowing liquid in healthy adults].

OBJECTIVE: To investigate the tongue pressure (TP) produced by tongue-hard palate contact in the process of normally swallowing liquid in healthy adults. METHODS: Thirteen adult male subjects were recruited to perform a single swallow of 5 ml water when sitting with upright position. The tongue pressure sensor sheet was used to monitor TP as a result of tongue-hard palate approximatation in the anteriomedian, midmedian, posteriomedian and circumferential parts, and the swallowing sound was recorded by microphone. The temporal sequence of TP at each measured part was obtained after setting the

Propagation Curve and Velocity of Swallowing Pressure in Healthy Young Adults.

The objective of this study is to construct a propagation curve and determine propagation velocities in young healthy adults examined using a 2.64-mm-diameter high-resolution manometry catheter with 36 circumferential sensors; to explore data reproducibility; and to determine whether the swallowing pressure (SP) propagation velocity correlated with bolus volume. Repeated measures with subjects serving as their own controls. Thirty healthy subjects (average age 25.3 years) swallowed saliva and 2, 5, and 10 mL of cold water to determine the maximum SP from the soft palate to the cervical esophag

Surgical resection of cervical schwannoma and paraganglioma: Speech and swallowing outcomes.

We conducted a retrospective study (1999 to 2009) at our tertiary care institution to evaluate speech and swallowing outcomes after the resection of cervical schwannoma or paraganglioma. Of 6 patients treated for schwannoma, 5 (83.3%) had immediate dysphonia and dysphagia. All patients with deficits received primary reinnervation (n = 2) or subsequent medialization laryngoplasty (n = 3). At 6 months, 4 patients (66.6%) still had dysphonia and dysphagia. At final follow-up (median: 10 months; range: 8 to 12 months), 4 patients (66.7%) had dysphonia and 2 (33.3%) had dysphagia. Of 10 patients tr

Two cases of small bowel obstruction secondary to a swallowed potato.

Bowel obstruction due to bezoars (compaction of ingested material within the gastrointestinal tract) is a rare, but well documented occurrence. In this paper, we present two cases of potato-induced small bowel obstruction. Both patients were approximately 50 years old and had cerebral palsy and learning disabilities, respectively. They presented with abdominal pain and bilious vomiting, with no medical or surgical history. Diagnosis of small bowel obstruction was confirmed by CT prior to taking the patients to the operating theatre, where whole potatoes were found to be obstructing each patien

Speech and swallow rehabilitation following partial glossectomy: a systematic review.

PURPOSE: Tongue cancer is known to negatively affect both speech and swallowing function; however this is the first review to report speech-language pathology (SLP) intervention for both functions following partial glossectomy. METHOD: Using a PRISMA approach, systematic screening of nine databases was undertaken. Original studies reporting SLP rehabilitation for speech and/or swallowing dysfunction with participants following partial glossectomy as primary cancer treatment were included. RESULT: These studies are discussed in terms of SLP assessment and intervention trends as well as qualit

One swallow does not a summer make: safety and renal denervation.

Renal denervation is increasingly being adopted as a treatment option in patients with resistant hypertension. The long-term safety of this procedure is unknown. Though the procedure interrupts the sympathetic nerves at the renal level, it also has effects on other organ beds, notably the heart and vasculature. These effects have been purported to be clinically beneficial and thus formed a rationale for examining the role of renal denervation in other conditions, including heart failure, arrhythmia, obstructive sleep apnoea and the metabolic syndrome. There is a theoretical concern that attenu

Acoustic study of pharyngeal swallowing as a function of the volume and consistency of the bolus.

Cervical auscultation is a noninvasive technique for the exploration of swallowing and has been used since the 1960s. The aim of our study was to describe how the volume and consistency of the bolus affect swallowing acoustic sound characteristics in healthy subjects. Twenty-three subjects aged from 20 to 59 years were included (13 women and 10 men). A microphone mounted on a stethoscope chest piece, positioned on the skin on the right side in front of the posteroinferior border of the cricoid cartilage, was used; it was connected to a computer for acoustic recordings. Each subject swallowed 2

Effect of the order of test diets on the accuracy and safety of swallowing studies.

OBJECTIVE: To assess whether the order of test diets influences the results of swallowing studies with regard to their accuracy and safety. METHODS: Subjects with suspected dysphagia underwent a videofluoroscopic swallowing study (VFSS) and/or a fiberoptic endoscopic evaluation of swallowing (FEES) and repeated the study on the same day or within a week. The order of test diets comprised of two different sets: trial 1 with the fluid first and trial 2 with the semi-solid food first. Main outcome measurements were the modified penetration-aspiration scale (mPAS) and the pharyngeal residue sever

Biomechanics of human tongue movement during bolus compression and swallowing.

We evaluated the effects of gel consistency and bolus volume on ingestion in humans. Eight healthy men were asked to ingest liquids, and sample foods of different gel consistencies and volumes, as usual. Tongue pressure against the hard palate was recorded at five points, and bolus flow was recorded using videoendoscopic images. The number of squeezes increased as gel consistency and volume increased. The integrated magnitude of tongue pressure during squeezing increased with increasing gel consistency. Bolus propulsion into the pharynx was affected by bolus characteristics, and location of th

Nursing swallow screens: why is testing water only not enough?

UNLABELLED: The speech-language pathologist (SLP) standardized a Nursing Bedside Swallowing Screen (NBSS) tool for all patients admitted to the hospital. The adults engaged in the NBSS before oral intake (i.e., medication included) as part of the Brain Attack Pathway for patients with neurological symptoms. If the patient failed the NBSS in the emergency department (ED), then the screen was repeated again after the patient had been admitted before the SLP dysphagia evaluation. Fifty-three male and female patients ranging from 34 to 96 years old with an initial diagnosis of stroke or transient
