The catheter and urinary tract infection.

Catheter-associated bacteriuria is the most common infection acquired in hospitals and nursing homes. This infection would be even more common but for the use of the closed catheter system. Most modifications have not improved upon the closed catheter itself. However, even with meticulous care, this system will not prevent bacteriuria forever. After bacteriuria develops, our ability to limit its complications is minimal. Additionally, the catheterized urinary tract becomes a reservoir of bacteria that can be transferred to other patients. Once a catheter is put in place, one must keep two impo

The rationale for bladder washouts in children with neurogenic bladder.

Children with a neurogenic bladder are at risk of developing recurrent urinary tract infections and long-term kidney failure. Due to an altered lower urinary tract, children may be overtreated for simple bacteriuria or undertreated for a potentially severe urinary tract infection. This group of patients represent high users of healthcare, and are at risk of colonization and development of antibiotic resistance. Bladder washouts with non-antibiotic electrochemically activated solutions are a potential new prophylactic option for patients with bladder dysfunction when clean intermittent catheter

When are indwelling urinary catheters appropriate in elderly patients?

Indwelling catheters are commonly used in elderly patients with urinary retention, incontinence, pressure ulcers, and cancer, ostensibly for the patient's comfort but sometimes to ease the burden of health care workers. Indwelling catheterization is overused; catheters should be inserted only for specific, well-documented indications. The best way to avoid catheter-associated infections and other complications is to avoid long-term catheter use whenever possible. Given the risks involved, catheterization is indicated only as a last resort, after other options have failed.

High-resolution imaging reveals microbial biofilms on patient urinary catheters despite antibiotic administration.

PURPOSE: Catheter-associated urinary tract infections (CAUTIs) are a significant cause of morbidity worldwide, as they account for 40% of all hospital-associated infections. Microbial biofilm formation on urinary catheters (UCs) limits antibiotic efficacy, making CAUTI extremely difficult to treat. To gain insight into the spatiotemporal microbe interactions on the catheter surface we sought to determine how the presence or absence of bacteriuria prior to catheterization affects the organism that ultimately forms a biofilm on the UC and how long after catheterization they emerge. METHODS: Thi

[Clean vesical intermittent catheterization: lubrication of vesical catheter through lidocaine or gentamicine?].

INTRODUCTION: The patients who undergo intermittent catheterization are at high risk of urinary tract infection. A prophylaxis method for this infection can be represented by the lubrication of urethral catheters with 0.1% gentamycin cream. This study compared the lubrication of urethral catheters with 0.1% gentamycin cream and 2% lidocaine jelly in relation to the quantitative results of the urine culture and incidence of symptomatic urinary tract infection and asymptomatic bacteriuria. MATERIAL AND METHODS: Sixteen patients, divided in groups A and B, were studied. Patients in Group A were

Temporary tube cystostomy as a treatment for urinary obstruction secondary to adrenal disease in four ferrets.

Adrenal neoplasia is a common problem in middle-aged to older ferrets. Male ferrets may present for stranguria and dysuria due to prostatic/paraurethral tissue enlargement secondary to elevation in androgens produced by the neoplastic tissue. Progressive urethral compression followed by complete urinary obstruction can result. Urinary obstruction can persist for days following surgery requiring urinary diversion. Four ferrets presenting with signs consistent with urinary obstruction secondary to adrenal disease were immediately treated with urethral catheterization or cystocentesis followed by

Augmentation cystoplasty in the management of neurogenic bladder disease and urinary incontinence.

Vesical augmentation procedures were performed on 15 patients for neurogenic bladder disease and urinary incontinence. Enterocystoplasty with ileum, cecum and sigmoid was used associated with different operations to prevent upper tract deterioration or urinary incontinence. The small bowel stored larger amounts of urine at a lower maximal detrusor pressure at capacity than the large bowel. All ureterointestinal and ureterovesical reimplantations were successful, including 5 ureteroileal with the Camey procedure. The 2 failures, characterized by persistent urinary incontinence, included a male

Continent urinary diversion in the epispadias-exstrophy complex.

Despite improvements in modern surgical reconstructive techniques, many patients with epispadias-exstrophy continue to experience urinary incontinence. Continent diversion is commonly performed to achieve urinary continence and improve quality of life. In this work we describe the population that can be considered for continent urinary diversion, consider the benefits and implications of concurrent augmentation and bladder neck closure, and review recent literature regarding continence outcomes and common complications. Even in this complex patient population, urinary continence can be reliabl

Acute urinary retention from hyperangulation of the urinary bladder neck secondary to uterine leiomyoma, a case report.

We present a case of acute urinary retention (AUR) with hyper-angulation of the urinary bladder neck secondary to uterine leiomyoma. Our patient is a 45-year-old female who presented with AUR and suprapubic pain requiring catheterization. CT images highlight the level of obstruction secondary to suspension of the urinary bladder rather than direct urethral compression. This case highlights this unique manifestation of AUR demonstrating the necessity for understanding its different mechanisms. Clinicians should maintain a high index of suspicion for AUR in patients with leiomyoma and lower urin

[Management of pregnancy and delivery after augmentation cystoplasty].

We report 2 cases of women who became pregnant and experienced vaginal delivery after augmentation cystoplasty. CASE 1: A 23-year-old woman with spina bifida became pregnant 3 years after augmentation sigmoidocystoplasty which had been performed to treat intractable urinary tract infection and urinary incontinence. During pregnancy, she developed febrile urinary tract infection twice which required antibiotics together with tight adherence to clean intermittent catheterization. At 36 weeks of gestation, she was safely delivered of a healthy baby. No deterioration of urinary continence level an

Medical interns' knowledge and training regarding urethral catheter insertion and insertion-related urethral injury in male patients.

BACKGROUND: Improper catheterization can lead to urethral injury. Yet research from four continents suggests training of junior doctors in c … ssociated with urethral injury. CONCLUSIONS: More thorough training of incoming medical interns in urinary catheterization may help to reduce the risk of complications and injury. Training should be universal and thought given to its timing within the curriculum. Training should include step by step instruction in the process, emphasis on history taking and awareness of …

Randomized multi-centre trial of the effects of a catheter coated with hydrogel and silver salts on the incidence of hospital-acquired urinary tract infections.

Catheters coated with hydrogel and silver salts have been proposed to prevent hospital-acquired urinary tract infections (UTI). We carried out a randomized, prospective, double-blind multi-centre trial to compare those catheters with classical urinary tract catheters. We included in the study 199 patients requiring urethral catheterization for more than three days: 109 in group 1 (classical catheter) and 90 in group 2 (catheter coated with hydrogel and silver salts). Urine from the patients was tested for 10 days after the insertion of the catheter (reactive dipsticks each day and diagnostic u
