Transjugular Left Atrial Appendage Occlusion in a Patient With Anomalous Inferior Vena Cava.

OBJECTIVE: We report a successful right transjugular Watchman device implantation for left atrial appendage (LAA) occlusion in a patient with anomalous inferior vena cava with azygos continuation, precluding standard transfemoral venous access. KEY STEPS: Ultrasound-guided right internal jugular venous access was obtained and upsized to accommodate the delivery system. Transseptal puncture was performed using an Agilis NxT medium-curve steerable sheath with microcatheter and wire exchanges, followed by staged balloon septal dilatation. The delivery sheath was successfully advanced into the LA…

Reduced-dose direct oral anticoagulant versus antiplatelet strategies after left atrial appendage occlusion.

BACKGROUND: The optimal antithrombotic therapy after left atrial appendage occlusion (LAAO) in patients with atrial fibrillation (AF) remains uncertain. Reduced-dose direct oral anticoagulation (DOAC) is increasingly used in clinical practice as an alternative to antiplatelet-based strategies, but comparative outcome data are limited. AIMS: We sought to compare the clinical outcomes associated with low-dose DOAC therapy versus dual antiplatelet therapy (DAPT) or single antiplatelet therapy (SAPT) after LAAO. METHODS: Using the TriNetX Global Network, we identified adult patients with AF who …

Late Infection of a Left Atrial Appendage Occlusion Device Detected by Transesophageal Echocardiography.

BACKGROUND: Left atrial appendage occlusion devices are used for stroke prevention in patients with atrial fibrillation who cannot tolerate long-term anticoagulation. Device-related infection is a rare complication and typically occurs early after implantation. CASE SUMMARY: A 73-year-old woman with paroxysmal atrial fibrillation who underwent Watchman implantation 6 years earlier presented with persistent methicillin-resistant Staphylococcus aureus bacteremia following debridement of infected right humeral hardware. Transthoracic echocardiography demonstrated preserved left ventricular systo…

Giant Right Atrial Appendage Aneurysm with Thrombus Causing Cardiac Compression in Pulmonary Tuberculosis.

Right atrial appendage aneurysm (RAAA) is a rare cardiac anomaly, predominantly considered congenital, resulting from dysplasia or localized absence of atrial myocardium leading to thinning and aneurysmal dilation. However, this case highlights the possibility of an acquired mechanism, particularly in the context of chronic infectious and inflammatory conditions such as tuberculosis (TB), which may have contributed to aneurysm formation in this patient. The patient's presentation with the left-sided chest pain and weight loss in the setting of active pulmonary TB raises clinical suspicion of a…

Percutaneous left atrial appendage closure for stroke prevention: current challenges and future perspectives.

Stroke accounts for significant morbidity and mortality worldwide. Accordingly, appropriate prevention is crucial to reduce the global burden of stroke. Percutaneous left atrial appendage closure (LAAC) is a viable approach for preventing cardioembolic stroke in patients with non-valvular atrial fibrillation who have a contraindication to long-term oral anticoagulation due to a non-reversible cause. In the last 2 decades, percutaneous LAAC has become a rapidly evolving field, with multiple devices approved or under clinical development. However, several issues remain, including limited high-qu…

Impact of early bleeding on long-term mortality after left atrial appendage occlusion.

BACKGROUND: Short-term antithrombotic therapy is recommended after left atrial appendage occlusion (LAAO) to reduce the risk of device-related thrombosis. However, patients referred for LAAO have excess bleeding risk. Yet, short-term bleeding events post-LAAO are not well characterized. OBJECTIVES: We aimed to evaluate the incidence of early bleeding post-LAAO and its association with long-term mortality risk. We also sought to identify predictors of early bleeding. METHODS: Patients who underwent LAAO at Mayo Clinic between 2018 and 2023 were included. Early bleeding was defined as bleeding…
