Impact of enhanced infection control and antimicrobial stewardship on infections by Clostridioides difficile, vancomycin-resistant enterococci, and third-generation cephalosporin-resistant Enterobacterales: a stepped-wedge cluster intervention study.

OBJECTIVES: Infection prevention and control (IPC) and antimicrobial stewardship (AMS) measures are critical to reducing transmission and infection by Clostridioides difficile (CDI) and other enteric pathogens. This study evaluated the impact of enhanced IPC and AMS on CDI and bloodstream infections (BSIs) caused by vancomycin-resistant enterococci (VRE) and third-generation cephalosporin-resistant Enterobacterales (3GCREB). METHODS: The study was conducted in five German university hospitals from January 2016 to July 2019. IPC and AMS interventions were sequentially enhanced in three departm…

The comparative pathology of enterocolitis caused by Clostridium perfringens type C, Clostridioides difficile, Paeniclostridium sordellii, Salmonella enterica subspecies enterica serovar Typhimurium, and nonsteroidal anti-inflammatory drugs in horses.

To determine if there were significant differences produced by 5 of the most prevalent causes of equine enterocolitis, we studied retrospectively the gross and microscopic pathology of 90 cases of enterocolitis submitted to the San Bernardino laboratory of the California Animal Health and Food Safety Laboratory. Included were cases caused by Clostridium perfringens type C (CP; n = 20), Clostridioides difficile (CD; n = 20), Paeniclostridium sordellii (PS; n = 15), Salmonella enterica subspecies enterica serovar Typhimurium (ST; n = 20), and NSAID intoxication (NS; n = 15). Grossly, necrotizing…

A competitive BB-ELISA for anti-toxin B neutralising antibodies in assessing risk of recurrent Clostridioides difficile infection: a retrospective cohort study from the MODIFY I and II trials.

BACKGROUND: Recurrent Clostridioides difficile infection (rCDI) is a major clinical challenge. Host neutralising antibodies (NAbs) against toxin B (TcdB) are key correlates of protection, but the absence of a simple, standardised assay to measure this functional immunity prevents risk-adapted patient management. We aimed to develop and clinically validate a rapid, bispecific antibody-based competitive ELISA for measuring anti-TcdB NAbs. METHODS: In this retrospective cohort study of the MODIFY I and II phase 3, randomised, placebo-controlled trials of bezlotoxumab (conducted at 322 sites in 3…

Twenty-Five-Year Trends in Mortality Associated with Clostridioides difficile Infection Among Patients with Inflammatory Bowel Disease in the United States: A Population-Based Analysis of Demographic and Geographic Disparities.

BACKGROUND: Individuals with inflammatory bowel disease (IBD) are at substantially increased risk for Clostridioides difficile infection (CDI), which leads to significantly higher morbidity and mortality compared to the general population. However, comprehensive national-level analyses of long-term mortality trends in this population remain limited. This study examines mortality trends associated with IBD and CDI in the United States from 1999 to 2023. METHODS: This descriptive study utilized the CDC WONDER Multiple Cause-of-Death database. Deaths involving IBD (ICD-10: K50, K51) and CDI (A04…

Epidemiology of Clostridioides difficile infection at one hospital 10 years after an outbreak of the epidemic C. difficile strain BI/027: changing strain prevalence, antimicrobial susceptibilities, and patient antibiotic exposures.

In contrast to the epidemiology 10 years earlier at our hospital when the epidemic restriction endonuclease analysis (REA) group strain BI accounted for 72% of Clostridioides difficile isolates recovered from first-episode C. difficile infection (CDI) cases, BI represented 19% of first-episode CDI isolates in 2013-2015. Two additional REA group strains accounted for 31% of isolates (Y, 16%; DH, 12%). High-level resistance to fluoroquinolones and azithromycin was more common among BI isolates than among DH, Y, and non-BI/DH/Y isolates. Multivariable analysis revealed that BI cases were 2.47 tim…

Fecal microbiota transplantation versus vancomycin or fidaxomicin in Clostridioides difficile infection first episode and first recurrence: a study protocol for a randomised, controlled, open-label, multicentre phase III clinical trial.

INTRODUCTION: Clostridioides difficile infection (CDI) is a leading cause of healthcare-associated diarrhoea in adults, with recurrence rates of 15%-25% after a first episode and up to 65% after multiple recurrences. Recurrent CDI leads to significant morbidity, diminished quality of life, prolonged antimicrobial exposure and increased healthcare utilisation. Faecal microbiota transplantation (FMT) demonstrates high efficacy for multiple recurrent CDI, but its benefit when used earlier-after the first episode in high-risk patients or first recurrence-remains insufficiently studied. Emerging ev…
