Update on the evaluation and treatment of osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a heritable bone fragility disorder that presents with a wide clinical phenotype spectrum: from perinatal lethality and severe deformities to very mild forms without fractures. Most cases of OI are due to autosomal dominant mutations of the type I collagen genes. A multidisciplinary approach with rehabilitation, orthopedic surgery, and consideration of medical therapy with bisphosphonates underpins current management. Greater understanding of the pathogenesis of OI may lead to novel, therapeutic approaches to help improve clinical symptoms of children with OI in

Renal safety in patients treated with bisphosphonates for osteoporosis: a review.

Bisphosphonates are widely used for the treatment of osteoporosis and are generally well tolerated. However, the United States Food and Drug Administration safety reports have highlighted the issue of renal safety in bisphosphonate-treated patients. All bisphosphonates carry labeled "warnings" or a contraindication for use in patients with severe renal impairment (creatinine clearance <30 or <35 mL/min). Data from pivotal trials and their extension studies of bisphosphonates approved for the management of osteoporosis were obtained via PubMed, and were reviewed with support from published arti

Potential anticancer properties of bisphosphonates: insights from preclinical studies.

Bone is a common site of metastasis from advanced cancers, and metastasis to bone accounts for the majority of distant recurrences from breast and prostate cancers. Bone metastases are characterized by increased rates of bone turnover. Bisphosphonates are extensively used in the treatment of metastatic bone disease to reduce the rates of osteolysis and the risk of skeletal-related events. In addition, bisphosphonates have demonstrated direct and indirect anticancer potential in preclinical studies. These activities include induction of apoptosis, inhibition of invasion, synergistic cytotoxicit

Systemic metabolic radiopharmaceutical therapy in the treatment of metastatic bone pain.

Bone pain due to skeletal metastases constitutes the most common type of chronic pain among patients with cancer. It significantly decreases the patient's quality of life and is associated with comorbidities, such as hypercalcemia, pathologic fractures and spinal cord compression. Approximately 65% of patients with prostate or breast cancer and 35% of those with advanced lung, thyroid, and kidney cancers will have symptomatic skeletal metastases. The management of bone pain is extremely difficult and involves a multidisciplinary approach, which usually includes analgesics, hormone therapies, b

[Femur fractures caused by long-term bisphosphonate therapy].

There is increasingly more evidence to suggest that spontaneous femur fractures may be an adverse effect of long-term bisphosphonate use. Patients typically present with prodromal thigh pain, cortical thickening on conventional radiographs or present at A&E with a spontaneous transverse subtrochanteric femur fracture with a medial spike. Discontinuation of bisphosphonate therapy is advisable in patients presenting with a spontaneous femur fracture with specific features while using bisphosphonates. The way in which bisphosphonates alter the mechanical properties of bone is as yet unknown.

Osteonecrosis of the jaw and bisphosphonate treatment for osteoporosis.

A potential side effect associated with bisphosphonates, a class of drugs used in the treatment of osteoporosis, Paget's disease and metastatic bone disease, is osteonecrosis of the jaw (ONJ). The incidence of ONJ in the general population is unknown; this rare condition also may occur in patients not receiving bisphosphonates. Case reports have discussed ONJ development in patients with multiple myeloma or metastatic breast cancer receiving bisphosphonates as palliation for bone metastases. These patients are also receiving chemotherapeutic agents that might impair the immune system and affec

Bisphosphonate-associated osteonecrosis of the jaw. A proposal for conservative treatment.

The use of bisphosphonates (BPs) has proven effective in the treatment of bone-related diseases, despite the potential risk of developing osteonecrosis of the jaw (ONJ). So far, results for the treatment of ONJ have not been satisfactory. In our study, we have treated two patients diagnosed with ONJ. In addition to local treatments a hydrogel was applied, with promising results. The fistulae disappeared 2-3 weeks into the treatment. After a six-month follow-up period there has been no sign of recurrence. The extent of maxillary bone exposure has diminished notably, although not entirely disapp

[Bisphosphonates in oncology: update 2007].

Bisphosphonates prevent bone complications induced by cancer. Their low toxicity promoted their extensive use supported by different international recommendations. However the prescription of these therapies is now seriously questioned because of their late and severe toxicity, the osteonecrosis of the jaw, and the growing efficiency of the oncologic therapies. No monitoring method is now available for bisphosphonates therapy. Therefore only a stricter selection of the patients, based on established and well-proven indications, as well as limitation of the administration durations could mainta

Diagnosis and treatment of vertebral column metastases.

The vertebral column is recognized as the most common site for bony metastases in patients with systemic malignancy. Patients with metastatic spinal tumors may present with pain, neurologic deficit, or both. Some tumors are asymptomatic and are detected during screening examinations. Treatment options include medical therapy, surgery, and radiation. However, diversity of patient condition, tumor pathology, and anatomical extent of disease complicate broad generalizations for treatment. Historically, surgery was considered the most appropriate initial therapy in patients with spinal metastasis

Bisphosphonate actions on cancer.

Bisphosphonates (BPs) suppress cancer cell colonization in bone associated with cancers such as breast cancer and multiple myeloma. The mechanism of the suppressive action of BPs is thought to be due to an inhibition of osteoclastic bone resorption which releases bone-stored growth factors that feed cancer cells colonizing bone. Recently, data are accumulating that BP suppresses growth and induces apoptosis in cancer cells in culture, suggesting that BP directly influences survival of cancer cells in an osteoclast-independent manner. These results raise the possibility that BP inhibits cancer

The pharmacology and therapeutic utility of bisphosphonates.

Bisphosphonates have clinical application in diseases associated with increased bone turnover that inhibit osteoclast-mediated bone resorption by direct and indirect actions on osteoblasts and macrophages. Bisphosphonates are the treatment of choice for Paget's disease of bone, in which they return to normal the increased rate of bone turnover and slow radiographic disease progression. The agents reduce hypercalcemia associated with malignancy, and may reduce bone pain and prevent radiographic progression of metastatic bone disease. In patients with postmenopausal osteoporosis, they prevent fu

Pharmacological management of osteoporosis.

Pharmacological interventions for the prevention of fractures in patients with osteoporosis aim mainly at correcting the bone remodelling imbalance by either reducing bone resorption and bone turnover, or stimulating bone formation. A number of agents are currently available but only a few were evaluated under controlled conditions in clinical trials whose primary efficacy end-point was the prevention of fractures. In addition, this information was obtained in patients with postmenopausal osteoporosis mainly with prevalent vertebral fractures, and most studies published to date had the occurre

[Clinical, humoral and scintigraphic assessment of a bisphosphonate as potential treatment of diaphyseal dysplasia: Ribbing and Cammurati-Engelmann diseases].

Cammurati-Engelmann's Disease or Progressive Diaphyseal Dysplasia (PDD), is a rare autosomal dominant disorder, sometimes non hereditable, which begins in childhood, and is characterized by symmetrical excess of osseous apposition in diaphysis and metaphysis of long bones. In severe cases skull and vertebrae could be involved. Clinically, patients refer limb pain, muscular weakness and atrophy, easy fatigability and waddling gait. Later on S. Ribbing described an illness that he thought was a separate entity with sclerosis and enlargement of diaphysis of femora and tibiae, which begins after p

Pamidronate: an unrecognized problem in gastrointestinal tolerability.

Pamidronate (aminohydroxypropylidine bisphosphonate, APD) is an effective agent for treatment of Paget's disease of bone, and it has also been thought to be effective for treatment of osteoporosis. We desired to study a newer, time-release preparation of pamidronate, and carried out a placebo-controlled, double-masked study of postmenopausal osteoporosis. The original formulation was in a rapidly dissolving gelatin capsule. We encountered four episodes of esophagitis in 49 enrolled patients. We therefore discontinued treatment with this preparation and later began the study again using a stand
