An improvement on the technique of transosseous tunnels for the fixation of lower pole patella fracture: Cerclage around the patella.

The technique of transosseous tunnels is widely used in lower pole patella fracture (LPP). Though the extensor mechanism can be restored, distal avulsion fragments are usually unstable. The subsequent fracture gap and nonunion impair the extensor mechanism in turn. Cerclage is a promising method for treating the condition. The aim of this prospective cohort study is to determine if cerclage around the patella combined with the technique of transosseous tunnels stabilizes the avulsion fragments and brings out improved outcomes on LPP.Twenty two patients with LPP were treated with the hybrid tec

Atraumatic avulsion of the distal iliopsoas tendon: an unusual cause of hip pain.

While uncommon, isolated avulsion fractures of the lesser trochanter occur in children and adolescents prior to the fusion of this apophysis as a result of athletic activities. In the elderly, isolated fractures of the lesser trochanter are rare but can occur as a result of trauma. They have been identified in patients with primary or secondary bone malignancies, which were previously considered pathognomonic for metastatic disease. In the absence of trauma, weakening of the bone due to systemic disorders such as osteoporosis or osteomalacia chronica renal failure may also be responsible. Diag

Traumatic Abducens Nerve Palsy Due to Avulsion in its Cisternal Segment: Illustrative Cases.

BACKGROUND Traumatic abducens nerve palsy can be caused by fractures around the Dorello's canal, direct injury at the dura entry point or within the Dorello's canal, or ischemia due to posterior meningeal artery damage. Recent imaging studies have also identified avulsion of the abducens nerve at its cisternal segment as a significant cause. CASE REPORT We present 2 cases of traumatic abducens nerve palsy due to avulsion at the cisternal segment. Preoperative thin-slice T2 magnetic resonance imaging indicated unclear visualization of the nerve at the cisternal segment in both cases. Surgical e

Epidemiology, classification, treatment and mortality of Talus fractures: An observational study of 1794 talus fractures from the Swedish Fracture Register.

BACKGROUND: Epidemiological data on talus fractures from large nationwide and multicenter studies are rare. This study aims to describe the epidemiology, fracture classification and treatment regimens of talus fractures in a large adult Swedish population. METHODS: This observational study is based on data from the Swedish Fracture Register (SFR) including talar fractures in patients ≥18 with a sustained fracture between 2012 and 2021. Epidemiological data on sex, age, injury date, injury mechanism and type (high or low energy trauma), fracture classification (side, type), initial treatment a

Extensor carpi radialis longus avulsion: a literature review and case report.

Extensor Carpi Radialis Longus (ECRL) avulsion is a rare injury which follows resisted wrist hyperflexion. Treatment of this condition with open reduction and internal fixation is not previously described in the literature, and treatment with plaster immobilisation or k-wire fixation requires a prolonged period of immobilisation. We believe that open reduction and internal fixation of these fractures with early mobilisation will result in the best possible wrist function. We describe a sign to raise the index of suspicion for this injury: a palpable bone lump on the dorsum of the hand in the p

The tooth as a foreign body in soft tissue after head and neck trauma.

Thorough evaluation of dentition is important in the management of craniofacial trauma. Avulsed or fractured teeth in the pulmonary and gastrointestinal tracts can lead to serious complications that are well described. However, the penetration of avulsed teeth into soft tissues of the head and neck is unusual and may not be recognized. Two cases in which a tooth became a foreign body in these soft tissues are illustrative. One patient had bronchoscopy for possible aspiration of an avulsed maxillary canine; it was later found embedded in the premaxillary tissues. The other patient had a right m

Management of the distal radioulnar joint and ulnar styloid fracture.

Fractures of the distal radius and ulnar styloid have the potential to disturb the normal function of the distal radioulnar joint (DRUJ), resulting in loss of motion, pain, arthritis, or instability. The DRUJ can be adversely affected by several mechanisms, including intra-articular injury with step-off, shortening, and angulation of an extra-articular fracture; injury to the radioulnar ligaments; ulnar styloid avulsion fracture; and injury of secondary soft tissue stabilizers. This article discusses the management of the DRUJ and ulnar styloid fracture in the presence of a distal radius fract

A shearing osteochondral fracture of the humeral head following an anterior shoulder dislocation in a child.

A dislocation of the shoulder joint is rare in children with an open physis. The fractures associated with an anterior dislocation generally reported in the literature have been Hill-Sachs lesions, avulsions of the greater tuberosity and glenoid fractures. We present a case of a previously unreported shearing osteochondral fracture, which is distinct from a classic Hill-Sachs lesion of the humeral head, in 12-year-old boy. The patient suffered a traumatic anterior shoulder dislocation with a spontaneous reduction along with this associated fracture. The fracture subsequently healed with no fur

[Salter-Harris type IV epipyseal fracture of the lateral malleolus. A rare injury in childhood].

This is a case presentation of a 9-year-old boy who sustained a rare Salter-Harris type IV distal fibular fracture including an avulsion fracture of the anterior inferior tibiofibular ligament at the fibular attachment. Treatment consisted of open reduction and internal fixation by Kirschner wire and cerclage. Possible posttraumatic growth disturbances and the major implications are highlighted.

[Experiences with primary osteosynthesis in radial head fractures].

Displaced fractures of the radial head are indications for early surgery. The resection is unsatisfactorily leading often to an instable elbow in valgus deformity and to painful troubles in the distal radio ulnar joint. Prosthesis implantation has its own problems. Reconstructive surgery is obviously the best solution. 21 internal fixations in 20 patients were performed using small screws (and 2 plates) between 1968 and 1976. The functional and radiological results are excellent in 15 out of 19 controlled patients. 3 patients have a moderate reduction of the mobility, 1 patient a non-union. -W

Surgical Management of Bilateral Triceps Muscle Rupture: A Case Report and Literature Review.

INTRODUCTION: Triceps tendon avulsion is a rare tendinous injury that can be easily overlooked. If left untreated, such injuries can lead to a weakening of a patient's elbow extension and thereby cause significant disability; therefore, early identification and appropriate surgical intervention are important. CASE REPORT: We report bilateral triceps tendon avulsion injuries in a 49-year-old, right-hand dominant airline pilot. The patient fell while running and injured both his elbows. X-rays of both elbows showed displaced olecranon avulsion fractures, confirming triceps tendon injuries. Bone

Isolated Displaced Chaput Fracture (Rammelt Type III) With Significant Intra-articular Displacement: A Case Report.

The distal tibiofibular syndesmosis, which is essential for preserving ankle stability, is often involved in ankle injuries. Avulsion fractures of the anterior inferior tibiofibular ligament, also referred to as Chaput fractures, if left untreated, may go undetected on plain radiographs and result in chronic instability. For precise evaluation and treatment planning, advanced imaging techniques such as computed tomography (CT) are frequently necessary. We present the case of a 23-year-old man who fell from a height of 2 m and suffered a twisting injury to his right ankle and was seen at King H

Acquired maxillofacial defects from motor vehicle accidents: statistics and prosthodontic considerations.

Trauma from motor vehicle accidents is one of the leading causes of death in the United States; moreover, the costs related to personal harm are only second to cancer. Head and neck injuries predominate and involve contact with the vehicle's interior (80%), contact with the vehicle's exterior (12%), or noncontact with the vehicle (8%). The patient with maxillofacial defects resulting from motor vehicle accident will have numerous soft tissue and hard tissue injuries ranging from neurologic involvement to fractures and/or avulsions of the temporomandibular joint, maxillae, mandible, teeth, and

[Clinical picture of anterior instability of the shoulder joint].

The authors have studied the clinical picture of 360 patients with primary traumatic dislocations and the long-term results have been observed in 286 patients. The analysis of 50 arthrotomies of the shoulder performed in cases of anterior instability has been made. A direct relationship between the clinical course of instability and the severity of the intra-articular lesions appearing during the first traumatic dislocation has been established. Such lesions include ruptures or avulsions of the anterior portion of the cartilage wall, ruptures of the joint capsule, avulsion of the capsule from

[Traumatic muscle and tendon ruptures of the lower extremities in sport: adductor muscles, M. rectus femoris and M. biceps femoris].

Ruptures of the adductor muscles, the M. rectus femoris or the M. biceps femoris are sports injuries which need quick and reliable diagnostic management. Treatment of muscle injuries is mostly conservative; complete tendon ruptures or avulsion fractures of the tendons are treated operatively according to the dislocation and the functional loss.

Proximal interphalangeal joint dislocations and fracture-dislocations.

Although proximal interphalangeal joint dislocations are generally straightforward to treat, fracture-dislocations are among the most difficult hand injuries to manage. Fracture patterns range from simple to treat palmar plate avulsion fractures to complex, unstable pilon fractures of the base of the middle phalanx, where achieving adequate reduction and fixation can be extremely difficult. Moreover, these fractures may present sub-acutely or chronically, which greatly adds to the complexity of the case. It is therefore no surprise that clinical results vary and are often difficult to predict.

Roentgen examination of groin and hip pain in the athlete.

Stress fractures and reactions of the pubic ramus, pubic symphsitis, gracilis syndrome, pelvic avulsion injuries, femoral stress fractures, degenerative osteoarthritis, discogenic pain, and spondylolysis are among the multiple conditions causing groin pain in the athlete. These conditions occasionally have uncertain etiologies and are contributed to by poor training techniques and ill-repaired running shoes, combined with minor congenital anomalies that may have been silent prior to the demands of athletic competition. Roentgen documentation of the specific injury enables early appropriate tre
