A prospective study of ventriculo-peritoneal shunting for idiopathic normal pressure hydrocephalus.

Idiopathic normal pressure hydrocephalus (INPH) is a potentially treatable form of dementia but its diagnosis is difficult and the effectiveness of shunting remains controversial. This study investigates the clinical outcomes of ventriculo-peritoneal shunting in a controlled trial of 33 consecutive patients with INPH. Mean age was 77.2 years (range 58-92 years) and the duration of symptoms was 4.6 years (3 months-14 years). Nineteen patients underwent shunt surgery. At 3-4 months follow-up, patients who had undergone shunt surgery, compared to those who had not (controls), had significantly be

Upper extremity motor measures of Tap Test response in Normal Pressure Hydrocephalus.

OBJECTIVE: The Tap Test (TT) is a commonly used method for predicting shunt responsiveness in patients with Normal Pressure Hydrocephalus (NPH). The present study investigates whether measures of upper extremity motor function are useful for assessing response to spinal fluid drainage. METHODS: 42 subjects undergoing evaluations for idiopathic NPH (iNPH) participated in this study. A standardized gait evaluation, a neuropsychological battery, and objective tests of upper extremity motor functions were administered. A Neurologist skilled in NPH assessment independently rated patients as TT Res

The triad of idiopathic normal-pressure hydrocephalus a clinical practice case report.

An 89-year-old white male presented with memory impairment, slowness in responsiveness, and frequent falls over a two-year duration. Six months earlier, the patient was believed to have had a "dementia with parkinsonian features," but showed no response to incrementing doses of both donepezil and carbidopa-levodopa. Urinary urgency was believed to have been due to prostate hypertrophy. A head CT with contrast revealed moderate ventriculomegaly in the setting of mild diffuse cortical atrophy. A diagnosis of idiopathic normal-pressure hydrocephalus (INPH) was made.

Endoscopic third ventriculostomy in idiopathic normal pressure hydrocephalus: an Italian multicenter study.

OBJECTIVES: The aim of the report is to define the indications and results of endoscopic third ventriculostomy (ETV) in idiopathic normal pressure hydrocephalus and to discuss the physiopathological mechanism of this procedure. METHODS: The cases of 110 patients with idiopathic normal pressure hydrocephalus who underwent ETV in four Italian neurosurgical centers were retrospectively reviewed. The postoperative outcome was correlated with patient age, length of clinical history, preoperative clinical score, symptoms of clinical onset, type of hydrocephalus, and intraoperative findings. Adjustable valves in normal-pressure hydrocephalus: a retrospective study of 218 patients.

OBJECTIVE: We sought to assess the value of adjusting shunt valve opening pressure, complications, and outcomes with the use of an adjustable shunt valve in the treatment of patients with normal-pressure hydrocephalus (NPH). METHODS: In a single-center retrospective study, 231 adjustable valves (range, 30-200 mm H2O) were the first shunt implantations in 147 patients with idiopathic NPH (INPH) and 71 patients with secondary NPH (SNPH). The effect of adjustment on gait disturbance, cognitive impairment, urinary incontinence and other symptoms were evaluated, and an improvement index was create

Frequency analyses of CSF flow on cine MRI in normal pressure hydrocephalus.

Our objective was to clarify intracranial cerebrospinal fluid (CSF) flow dynamics in normal-pressure hydrocephalus (NPH). Frequency analyses of CSF flow measured with phase-contrast cine MRI were performed. The CSF flow spectra in the aqueduct were determined in patients ( n=51) with NPH, brain atrophy or asymptomatic ventricular dilation (VD), and in healthy volunteers (control group; n=25). The changes in CSF flow spectra were also analyzed after intravenous injection of acetazolamide. Moreover, a phase transfer function (PTF) calculated from the spectra of the driving vascular pulsation and

[Magnetic resonance imaging of aqueductal flow in patients with normal hydrocephalus pressure].

The treatment of patients with the so-called Normal Pressure Hydrocephalus (NPH) consists of ventricular shunting, but only in some cases is there clinical improvement. The selection of patients for ventricular shunting is usually based on the response to the evacuating lumbar punctures of the cerebrospinal fluid (CSF), but new MRI techniques of flow measurement have been used successfully. In this study, a group of 58 patients, 28 of which with NPH, is presented. Flow quantification of the aqueductal CSF using the MR Phase Contrast technique was performed in all of them. The authors conclude

Normal pressure hydrocephalus and basilar artery aneurysm. Case report and pathogenetic consideration.

The authors describe the case of a 60-year-old patient with basilar artery aneurysm who in time developed normal pressure hydrocephalus (NPH). Clinical examinations and laboratory tests did not reveal other data from which the pathology could be attributed to causes other than the vascular malformation already considered. The mechanical obstruction exerted by the aneurysm lodged on the floor of the third ventricle represents, according to the authors, the physiopathogenetic mechanism by which a temporal delay is developed between the endoventricular pulsation and that of the cerebral veins so

Normal pressure hydrocephalus in diabetic patients with recurrent episodes of hypoglycemic coma.

The pathophysiology of brain damage induced by severe hypoglycemia is still unknown. We experienced a case with type 1 diabetes and recurrent severe hypoglycemic coma who showed a central brain atrophy and an abnormal cerebrospinal fluid flow, suggesting normal pressure hydrocephalus. Following this case, the CSF flow was studied using 111In-DTPA cisternography in six consecutive diabetic patients admitted for repeated episodes of hypoglycemic coma. All the patients showed the central brain atrophy on computed tomography and four of them (67%) had the ventricular reflux, with delayed clearance

Endoscopic ventriculostomy versus shunt operation in normal pressure hydrocephalus: diagnostics and indication.

In contrast to the shunt operation the indication for an endoscopic ventriculostomy in patients diagnosed for normal pressure hydrocephalus is not scientifically established. Between September 1997 and December 1999 we operated on 48 patients diagnosed for normal pressure hydrocephalus. The diagnosis was established by means of the intrathecal lumbar or ventricular infusion test, the cerebrospinal fluid tap test and MRI-CSF flow studies pre- and postoperatively. In 37 patients (77%) we have implanted a ventriculo-peritoneal shunt, and in 11 patients (23%) we performed the endoscopic assisted t

Quantitative analysis of CSF flow dynamics using MRI in normal pressure hydrocephalus.

In order to clarify the flow dynamics of cerebrospinal fluid (CSF) in normal pressure hydrocephalus (NPH), a phase-contrast cine magnetic resonance (MR) imaging technique with retrospective cardiac gating was used to measure the quantitative flow velocity of CSF in the aqueduct in patients with NPH after subarachnoid hemorrhage (SAH-NPH group, n = 17), idiopathic NPH (1-NPH group, n = 2), asymptomatic ventricular dilatation or brain atrophy (VD group, n = 7) and healthy volunteers (control group, n = 19). Intracranial pressure (ICP) and pressure volume response (PVR) were also measured during

To what extent has the pathophysiology of normal-pressure hydrocephalus been clarified?

The causes and pathophysiology of idiopathic normal-pressure hydrocephalus (NPH) still remain poorly understood. We reviewed the recent pertinent literature on NPH from the past 3 years (1994- 1996) in this journal [Crit Rev Neurosurg (1997) 7:263-275] and here review further manuscripts that appeared mainly in 1997. Recent investigations suggest that idiopathic NPH may be a primary cerebral parenchymal disease rather than a disorder of cerebrospinal fluid (CSF) absorption. The pathophysiology of idiopathic NPH has a wide spectrum ranging from purely hydrodynamic problems owing to a CSF absorp

The differential diagnosis of Alzheimer's disease. Cerebral atrophy versus normal pressure hydrocephalus.

This article summarizes the neuroradiology of Alzheimer's disease (AD) and details the radiologic features that permit the identification of patients with normal pressure hydrocephalus (NPH). Patients with presumed AD show a characteristic atrophy pattern with specific involvement of the temporal lobes and hippocampus. These findings have prognostic implications. Patients with NPH typically show severe motoric and gait deficits and initially mild cognitive impairment. Marked improvement is shown in select patients after ventricular shunting.

Motor performance in normal pressure hydrocephalus assessed with an optoelectronic measurement technique.

The motor performance of 10 patients with normal pressure hydrocephalus (NPH) was clinically assessed with the aid of a computer assisted optoelectronic movement analysis system (Mac Reflex). We used the Posturo Locomotion Manual (PLM) method that gives a quantitative estimation of the postural, locomotor and manual performance in freely moving humans. Measurements were done before intervention, after CSF tap test and 3 months post installation of a CSF ventriculo peritoneal shunt. Comparison was made to data from 10 matched normal subjects and 10 patients with Parkinson's disease (PD). Shunt

Regional cerebral blood flow in normal pressure hydrocephalus: diagnostic and prognostic aspects.

Relative regional cerebral blood flow (rrCBF) was measured by single-photon emission tomography (SPET), using technetium-99m-d,l-hexamethylpropylene amine oxime (HMPAO) as flow tracer, in 23 patients with normal pressure hydrocephalus (NPH). 1000 MBq 99mTc-HMPAO was given intravenously and the rrCBF calculated as regional/cerebellar count level ratios. The patients were examined before and 3-12 months after ventriculoperitoneal shunt surgery. rrCBF was also determined in ten healthy aged matched volunteers who served as controls. The NPH patients had decreased rrCBF in the hippocampal regions
