Reconstructive surgery can restore appreciable function of the upper limb in tetraplegic patients. We report two cases of carpometacarpal dislocation of the four long fingers. They were diagnosed in emergency and treated by open reduction and stabilization by intramedullary pinning. They were followed for an average of 30 months. The results were rated good, and the two patients had an excellent grip strength, no deformity, no instability, no disabling or limited prehension. Thirty-six patients with stage II or III SNAC and SLAC wrists were treated by midcarpal arthrodesis and complete scaphoid excision. When assessed at a mean follow-up of 25 months, pain was significantly reduced both under resting and stress conditions. The active range of motion was 54% of the contralateral wrist and grip strength was 65% of the non-operated hand. We have reviewed the results in 34 patients (39 operations) following simple excision of the trapezium for osteoarthritis of the basal joint of the thumb. The average age at operation was 57 years and the average follow-up was 6 years. All the patients were graded clinically and radiologically and were asked their opinion of the procedure. The authors reviewed 20 lengthening procedures according to Matev performed between 1982 and 1985 at SOS Main Strasbourg. They developed a special device which has proved to be simple and easy to use. Indications are limited to a few congenital malformations and traumatic mutilations. In children a free vascularized epiphysis transfer has been used to allow normal growth. Results are analysed and short-comings are outlined: lengthening is limited (47.8%); frequency of complications mainly trivial account for 25%; treatment period is lengthy (2.8 m in children and 4.2 m in adults).

Thirty-four total trapezio-metacarpal prostheses have been fitted since 1971. Twenty-eight have had a follow-up for more than six months (up to a maximum of five years) and allow a valid assessment of the situation. Singleton-Merten syndrome (SMS) is a rare disease with a phenotype of dental dysplasia. Currently, the underlying mechanism of this disease is unknown. In order to investigate the functional mechanism of the SMS tooth phenotypes, we isolated dental pulp tissue and established SMS primary pulp cells.
