Outcomes for Women With Diabetes at Late Preterm Gestation Who Received Antenatal Corticosteroids: A Population-Based Cohort Study.

BACKGROUND: The use of antenatal corticosteroids appears to be increasing despite limited and conflicting evidence about its efficacy among women with diabetes in pregnancy. AIM: To separately assess the effects of maternal diabetes and the effects of antenatal corticosteroid administration prior to late preterm birth on adverse maternal and neonatal outcomes. MATERIALS AND METHODS: Cohort study of women who gave birth ≥ 35 weeks gestation between September 2016 and March 2021 in two local health districts in New South Wales, Australia. Women were grouped into one of the following: (1) diabe

Is it a Match? Yawn Contagion and Smile Mimicry in Toddlers.

Automatic behavioral matching includes Rapid Facial Mimicry (RFM) and Yawn Contagion (YC) that occur when the facial expression of an individual acts as a 'mirror social releaser' and induces the same facial expression in the observer (within 1 s for RFM, and minutes for YC). Motor replication has been linked to coordination and emotional contagion, a basic form of empathy. We investigated the presence and modulating factors of Rapid Smile Mimicry (RSM) and YC in infants/toddlers from 10 to 36 months at the nursery 'Melis' (Turin, Italy). In February-May 2022, we gathered audio and/or video of

Facilitators and barriers to anemia prevention in the urban government childcare program for infants and young children in Peru.

BACKGROUND: Anemia is a major public health problem in children under 2 years old in Peru and other low- and middle-income countries. OBJECTIVES: We aimed to explore facilitators and barriers to anemia prevention through feeding strategies in the Cuna Mas day-care centers in urban Peru among infants and young children (IYC) aged 6-23 months. METHODS: Qualitative research in day-care services (n = 14) in Peru. We conducted direct observations and semi-structured interviews about consumption of animal source foods (ASF) and iron supplementation with day-care staff (technical managers, nursery

Influence of Maternal Region of Birth on Placental Pathology of Babies Born Small.

BACKGROUND: Placental pathology is a common antecedent factor in infants born small for gestational age. Maternal region of birth can influence rates of SGA. AIMS: To determine the association of maternal region of birth on placental pathology in babies that are born small, comparing a South Asian born population with Australia and New Zealand born women. MATERIALS AND METHODS: A retrospective cohort study was conducted at Monash Health, the largest public health service in Victoria. Mother-baby pairs above 34 weeks' gestation and birth weight less than 10th centile born in 2016 were include

A fatal case of acute encephalopathy in a child due to coxsackievirus A2 infection: a case report.

BACKGROUND: Certain types of enteroviruses, including coxsackieviruses, cause encephalitis, and other neurological complications. However, these pathogens rarely cause fatal infections, especially in immunocompetent infants. In this study, we present a rare case of acute encephalopathy caused by coxsackievirus A2 (CV-A2), which progressed rapidly in a previously healthy female child. CASE PRESENTATION: In June 2013, a 26-month-old female child from Kanagawa, Japan, was found unresponsive during sleep. She was healthy until that morning. Her temperature was 37 °C at 08:00. She was feeling fine

Utility of Decision Rules for Transcutaneous Bilirubin Measurements.

BACKGROUND: Transcutaneous bilirubin (TcB) meters are widely used for screening newborns for jaundice, with a total serum bilirubin (TSB) measurement indicated when the TcB value is classified as "positive" by using a decision rule. The goal of our study was to assess the clinical utility of 3 recommended TcB screening decision rules. METHODS: Paired TcB/TSB measurements were collected at 34 newborn nursery sites. At 27 sites (sample 1), newborns were routinely screened with a TcB measurement. For sample 2, sites that typically screen with TSB levels also obtained a TcB measurement for the st

Universal screening of newborns to detect hearing impairment--is it necessary?

BACKGROUND: In contrast to the recommendations of the Joint Committee on Infant Hearing, neonatal hearing screening programs are still not universally available. OBJECTIVES: To prospectively evaluate the presence of hearing loss in all newborn babies delivered in our hospital during study period using the transient evoked otoactoustic emission as the primary screening tool followed by confirmation with brainstem evoked response audiometry before six months of age and to determine risk factors responsible for hearing loss. DESIGN: A prospective study of nonrandomized cohort. SETTING: Nursery

Evaluation of the direct antiglobulin (Coombs') test for identifying newborns at risk for hemolysis as determined by end-tidal carbon monoxide concentration (ETCOc); and comparison of the Coombs' test with ETCOc for detecting significant jaundice.

OBJECTIVE: First, to determine the sensitivity, specificity, and positive predictive value (PPV) of the direct antiglobulin test (DAT) for significant hemolysis in the neonate, as referenced to end-tidal carbon monoxide, the criterion standard for estimating the rate of hemolysis; and second, to evaluate the predictive value of the two procedures for significant jaundice. DESIGN: Consecutive term newborns admitted to the nursery of an inner-city university hospital over a 15-week period. DAT screening by the Blood Bank was performed on all. End-tidal carbon monoxide levels were obtained at 12

Routine contrast enemas for diagnosing and managing strictures following nonoperative treatment of necrotizing enterocolitis.

A study to determine the value of contrast enemas in diagnosing and managing intestinal strictures following nonoperative treatment of necrotizing enterocolitis was performed from 1978 through 1983. From 1974 through 1977, 17 patients survived nonoperative treatment of NEC and three developed symptomatic strictures, an incidence of 18% (3/17). Since then a total of 31 infants were treated for NEC; three patients survived operation for perforation and there were seven deaths, leaving 21 in the study group. Sixteen patients had contrast enemas three to six weeks after resolution of NEC, which re

Neonatal rotavirus infection and its relation to cord blood antibodies.

Among 274 neonates born at the maternity services of an urban hospital in India, 36.1% of the infants shed rotavirus in feces (as detected by ELISA) by 72 h of life. The excretion rate increased to 70.3% among the 120 infants who stayed for 5 days or more at the hospital. Diarrhoeal symptoms of mild and self-limited nature were observed only in 19.2% of the rotavirus excretors, the remaining being asymptomatic. Among the 98 infants who received supplement feeds, 49% acquired rotavirus infection as against 24.7% of the 150 exclusively breast fed infants (p less than 0.001). Viral RNA in the fec

Outbreaks of infectious intestinal disease in schools and nurseries in England and Wales 1992 to 1994.

We present data on outbreaks of infectious intestinal disease in schools and nurseries obtained from the surveillance scheme of all general outbreaks of infectious intestinal disease in England and Wales reported to the PHLS Communicable Disease Surveillance Centre between 1992 and 1994. A minimum set of data was received for 1280 outbreaks, 95 of which (7%) arose in schools and nurseries. The commonest pathogens were salmonellas, Shigella sonnei, and small round structured viruses. The mode of transmission was described as mainly from person to person in 55 outbreaks and mainly foodborne in 3

[The nuclear family and social protection of children].

The nuclear family is the consequence and reflection of rapid industrialization of the country. It requires a wider safeguarding of infancy in the framework of more extensive social and care service structures. Kindergartens and nursery schools are the basic answers to child care, coupled with the network of independent institutions, and hospitals, out-patients centres and school medical services, and the local health unit, which supervises services in its district. Mention is also made of the measures still needed to ensure that kindergartens and nursery schools are available and efficient, a

Non-pharmacological care for opioid withdrawal in newborns.

BACKGROUND: The prevalence of substance use, both prescribed and non-prescribed, is increasing in many areas of the world. Substance use by women of childbearing age contributes to increasing rates of neonatal abstinence syndrome (NAS). Neonatal opioid withdrawal syndrome (NOWS) is a newer term describing the subset of NAS related to opioid exposure. Non-pharmacological care is the first-line treatment for substance withdrawal in newborns. Despite the widespread use of non-pharmacological care to mitigate symptoms of NAS, there is not an established definition of, and standard for, non-pharmac

Outbreak of febrile illness caused by coxsackievirus A4 in a nursery school in Beijing, China.

BACKGROUND: Coxsackievirus A4 (CV-A4) is classified as human enterovirus A according to its serotype. CV-A4, an etiological agent of hand, foot, and mouth disease, affects children worldwide and can circulate in closed environments such as schools and hospitals for long periods. FINDINGS: An outbreak of febrile illness at a nursery school in Beijing, China, was confirmed to be caused by CV-A4. Phylogenetic analysis of the complete genome of the isolated strain showed that the virus belongs to the same cluster as the predominant CV-A4 strain in China. This outbreak was controlled by effective

Disorganized infant attachment classification and maternal psychosocial problems as predictors of hostile-aggressive behavior in the preschool classroom.

This study of 62 low-income families examined the relation between maternal and infant measures assessed at 18 months infant age and child behavior problems at age 5 as rated by preschool teachers. The infancy assessments included measures of mother-infant interaction, maternal psychosocial problems, infant cognitive development, and infant attachment security, including the disorganized/disoriented classification. The strongest single predictor of deviant levels of hostile behavior toward peers in the classroom was earlier disorganized/disoriented attachment status, with 71% of hostile presch

[Epidemiological studies after an outbreak of meningitis (author's transl)].

The outbreak of meningitis during the period from the end of August to the beginning of October 1974 in Kiel affected 45 children, particularly those in the age group between 4 and 9 years. An potential causative agents ECHO viri were identified in 15/45 cases and Coxsackie B viri in 2/45 cases. The patients presented the typical meningitic symptoms (fever, headache, stiffness of the neck). In all cases the disease ran a benign course. - Epidemiologically, it was possible to trace 32/45 cases back to three locally demarcated groups with demonstrable possibilities of contact for the diseased ch
