Clinical Outcomes of Fiber-Tape Flexible Fixation for Chronic Lisfranc Injuries.

BACKGROUND: The optimal surgical procedures for chronic Lisfranc injuries are controversial. This study aimed to demonstrate the clinical outcomes of fiber tape fixation for chronic Lisfranc injuries under early full weightbearing protocols. METHODS: This study included 11 feet from 10 patients who underwent fiber tape fixation for chronic Lisfranc injuries (undiagnosed for more than 6 weeks from injuries). All patients were allowed full weightbearing with normal shoes 4 weeks after the surgery. Clinical outcomes were evaluated with the Self-Administered Foot Evaluation Questionnaire. Radiogr

Nonunion of intertrochanteric fractures of the femur following open reduction and internal fixation. Results of second attempts to gain union.

A retrospective review was performed on 20 patients treated at the authors' hospital between 1961 and 1981 with noninfected nonunions following primary open reduction and internal fixation (ORIF) of intertrochanteric femoral fractures. All patients were available for follow-up examination. Each patient was interviewed at an average follow-up period of 6.6 (0.8-11.9) years. The group consisted of 15 women and five men with an average age at the time of fracture of 63 (36-86) years. Only one fracture was "stable" while 19 were "unstable" with loss of medial support. The initial fixation device w

Open ankle arthrodeses via an anterior approach.

BACKGROUND: In open ankle arthrodesis, debate remains as to which surgical approach and fixation devices should be used. The purpose of this study was to identify union, complication, and patient satisfaction rates of ankle fusions performed at our institution, using the plane between extensor hallucis longus and tibialis anterior with medial tibiotalar screw internal fixation. MATERIALS AND METHODS: A retrospective review was performed of all isolated primary fusions between 2005 and 2009. Eighty-two ankles were identified in 73 patients. All patient records were reviewed, and 57 patients (6

[Results of 30 keratoprostheses with retrocorneal fixation].

Keratoprosthesis is the last solution for corneally blind patients who cannot benefit from corneal grafts. The indications are limited to blind patients with monophthalmia and bilateral problems that are otherwise untreatable. After implanting the Choyce and Strampelli's devices for more than 10 years, we abandoned these anterior fixation techniques and now use a keratoprosthesis in which the sole mechanical fixation consists of a posterior support which is subsequently sealed by the fibrosis produced by the endothelial-Descement tissues. This new prosthesis is made of two pieces, an optic and

Investigating the Radiological Outcomes of Syndesmosis Injuries in Ankle Fracture Patients After Suture Button Fixation.

Concomitant syndesmotic injury occurs in 10% of ankle fractures. Anatomic reduction and maintenance of this reduction is critical in ensuring ankle stability and preventing long-term complications. This is a retrospective cohort study aimed at evaluating the mid-term radiological outcomes of syndesmotic injuries in ankle fracture patients after surgical fixation with suture button device. The study group included 33 patients. Plain radiographs including anteroposterior, lateral and mortise views of the affected ankle were performed preoperatively, postoperatively and at 3-month follow-up. Ante

Endoscopic Hematoma Evacuation for Acute Subdural Hematoma in the Elderly: A Preliminary Study.

AIM: Endoscopic surgery for acute subdural hematoma (ASDH) has recently been performed in elderly patients. Unlike other reported procedures, we used the endoscope's fixation device to ensure hemostasis because the surgeon can then use a surgical instrument with each hand. To date, there have no reports that prove lesser invasiveness or noninferiority of endoscopic surgery based on direct comparison with a large craniotomy. In this study, we investigated whether our endoscopic procedure was effective and safe. MATERIALS AND METHODS: We performed 6 cases of endoscopic surgical procedures in el

Rapid Implementation of a 3-Dimensional-Printed Patient-Specific Titanium Sacrum Implant for Severe Neuropathic Spinal Arthropathy and Guide to Compassionate US Regulatory Approval.

BACKGROUND AND OBJECTIVE: Rapid design and production of patient-specific 3-dimensional-printed implants (3DPIs) present a novel opportunity to restore the biomechanically demanding integrity of the lumbopelvic junction. We present a unique case of a 61-year-old patient with severe neuropathic spinal arthropathy (Charcot spine) who initially underwent a T4-to-sacrum spinal fusion. Massive bone destruction led to dissociation of his upper body from his pelvis and legs. Reconstruction of the spinopelvic continuity was planned with the aid of a personalized lumbosacral 3DPI. METHOD: Using high-r

Open Reduction and Internal Fixation of Fractures of the Proximal Part of the Humerus.

INTRODUCTION: We describe the surgical technique for open reduction and internal fixation (ORIF) of proximal humeral fractures with a locking plate. STEP 1 PREOPERATIVE PLANNING: To choose the right candidate, obtain a full understanding of the patient's fracture pattern, activity level and demands, and bone quality; be aware of predictors of complications and poor outcomes. STEP 2 PATIENT POSITIONING: Place the patient in the beach-chair position with the arm draped free or in a hydraulic device with good access for the image intensifier. STEP 3 APPROACH: The deltopectoral approach is gene

A New Stem Guide and a Tuberosity Anchoring "Lasso" System for Shoulder Hemiarthroplasty in the Treatment of Complex Proximal Humerus Fractures.

Hemiarthroplasty implantation for proximal humerus fracture is challenging. In case of young demanding patients with functional rotator cuff, humeral head replacement is an option if open reduction internal fixation is not a viable solution. The criteria for success in terms of clinical outcomes and longevity are anatomic healing of the tuberosities and stem positioning (restoring correct height, version, and axis). The purpose of this article is to describe a technique including a new stem guide and a tuberosity anchor "lasso" system for the implantation of hemiarthroplasty in complex proxima

O-arm Navigation-assisted Extreme Lateral Interbody Fusion Utilizing SureTrak Device.

Currently, intraoperative guidance during conventional extreme lateral interbody fusion relies primarily on fluoroscopy, which can expose both surgical staff and the patient to radiation. The use of O-arm navigation during extreme lateral interbody fusion and percutaneous pedicle screw fixation is anticipated to enhance accuracy and substantially reduce radiation exposure. To date, there have been no published studies investigating the application of O-arm navigation in extreme lateral interbody fusion procedures. Thirty-three extreme lateral interbody fusion and percutaneous pedicle screw fix
