Use of implantable cardioverter defibrillator and cardiac resynchronization therapy: an Italian survey study on 220 cardiology departments.

OBJECTIVES: To evaluate the criteria for the use of implantable cardioverter defibrillator (ICD), cardiac resynchronization therapy (CRT) and other strategies in order to reduce the incidence of sudden death among adults at high risk and to identify the major barriers for the implementation of quality of care involving Italian cardiology departments in the context of 'Progetto Aritmie Area Scompenso Cardiaco ANMCO'. An additional aim was to evaluate how European Guidelines are applied in 'real-life' scenarios. METHODS: The clinical survey involved 220 centres. An 11-item questionnaire with pr…

Midterm 'super-response' to cardiac resynchronization therapy by biventricular pacing with fusion: insights from electro-anatomical mapping.

AIMS: Some authors recommend avoiding fusion with left ventricular (LV) intrinsic depolarization during cardiac resynchronization therapy (CRT). If fusion is still present during optimized biventricular (Biv) pacing and its long-term effects on the response to CRT are currently unknown. The aim of the study was to analyse the endocardial LV activation pattern induced by echocardiographically optimized Biv pacing and its influence on LV reverse remodelling. METHODS AND RESULTS: Contact electro-anatomical mapping was performed in 15 heart failure (HF) patients with left bundle branch block and …

Cardiac resynchronization therapy in patients with intrinsic and right ventricular pacing-induced left bundle branch block pattern.

We studied 95 consecutive patients, mean age 70 years, who received cardiac resynchronization therapy (CRT) for class III or IV heart failure with a left ventricular (LV) ejection fraction < or =35% and a QRS duration > or =120 ms. Sixty-seven patients had intrinsic left bundle branch block (LBBB) (group 1), and 28 patients had right ventricular pacing-induced LBBB (group 2). The time difference (TPW-TDI) between onset of QRS to the end of LV ejection by pulsed wave Doppler and onset of QRS to the end of systolic wave in the basal segment with greatest delay by tissue Doppler imaging was measu…

Cardiac resynchronization therapy may improve symptoms of congestive heart failure in patients without electrical or mechanical dyssynchrony.

AIMS: Cardiac resynchronization therapy (CRT) has reportedly not been effective in the absence of electrical or mechanical dyssynchrony. We present six patients with severe left ventricular (LV) dilation, mitral regurgitation (MR), and non-ischaemic cardiomyopathy who underwent CRT. We assessed the effects of CRT on LV ejection fraction (EF), LV dimensions, mitral valve regurgitant fraction (RF), pulmonary arterial pressures (PAP), and serum levels of B-natriuretic peptide (BNP). METHODS AND RESULTS: All patients had severe LV dilation (>/=6.8 cm) and no electrical or mechanical dyssynchrony.…

Cardiac resynchronization therapy with sequential biventricular pacing: impact of echocardiography guided VV delay optimization on acute results.

BACKGROUND: Cardiac resynchronization therapy (CRT) improves left ventricular synchrony as evaluated by tissue Doppler imaging (TDI), leading to improved left ventricular performance and reverse remodeling. New CRT devices enable programming of left and right VV delay. The aim of this study was to determine whether sequential biventricular (BiV) pacing by echo-guided programming of VV delay would enhance the response to CRT. METHODS: 15 consecutive patients with severe heart failure and left bundle branch block underwent CRT by BiV device implantation. They were studied with conventional and …

Improvements in left ventricular diastolic function after cardiac resynchronization therapy are coupled to response in systolic performance.

OBJECTIVES: To determine the short-term effects of cardiac resynchronization therapy (CRT) on measurements of left ventricular (LV) diastolic function in patients with severe heart failure. BACKGROUND: Cardiac resynchronization therapy improves systolic performance; however, the effects on diastolic function by load-dependent pulsed-wave Doppler transmitral indices has been variable. METHODS: Fifty patients with severe heart failure were evaluated by two-dimensional Doppler echocardiography immediately prior to and 4 +/- 1 month after CRT. Measurements included LV volumes and ejection fracti…

Cardiac resynchronization therapy homogenizes myocardial glucose metabolism and perfusion in dilated cardiomyopathy and left bundle branch block.

OBJECTIVES: We investigated whether cardiac resynchronization therapy (CRT) affects myocardial glucose metabolism and perfusion in dilated cardiomyopathy (DCM) and left bundle branch block (LBBB). BACKGROUND: Patients with DCM and LBBB present with asynchronous left ventricular (LV) activation, leading to reduced septal glucose metabolism. Cardiac resynchronization therapy recoordinates LV activation, but its effects on myocardial glucose metabolism and perfusion remain unknown. METHODS: In 15 patients (10 females; 61 +/- 13 years) with DCM and LBBB (QRS width 165 +/- 15 ms), gated (18)F-flu…

Left ventricular extracellular volume on computed tomography is significant predictor of patient prognosis after cardiac resynchronization therapy.

BACKGROUND: Cardiac resynchronization therapy (CRT) is useful for patients with severe heart failure (HF); however, almost 30% of patients are non-responders. PURPOSE: This study aimed to evaluate the utility of left ventricular (LV) extracellular volume (ECV) on computed tomography (CT) for the prediction of prognosis in CRT candidates. METHODS: This study included 52 patients who underwent cardiac CT before CRT device implantation between 2008 and 2024. The primary outcomes were cardiac death, hospitalization for HF, and fatal arrhythmia after CRT device implantation. CRT responders were d…

Personalized Test Bolus MSCT Protocol for Optimal Coronary Sinus Venous Visualization in Candidates for Cardiac Resynchronization Therapy.

Background/Objectives: A thorough understanding of the anatomy of the coronary sinus (CS) and its tributaries provides valuable information for selecting the optimal left ventricular lead and may even prompt reconsideration of the endovascular implantation strategy when planning cardiac resynchronization therapy (CRT). Currently, there is no personalized multislice computer tomography (MSCT) protocol for CS veins visualization that is suitable for all diverse candidates. Methods: a single-center prospective study included 74 various adult patients with recommendation class I and IIA for CRT. P…

Left Bundle Branch Area Pacing Versus Biventricular Pacing for Cardiac Resynchronization Therapy: A Meta-Analysis of RCTs.

BACKGROUND: Left bundle branch area pacing (LBBAP) has emerged as an alternative to conventional biventricular pacing (BiVP) for cardiac resynchronization therapy (CRT). While observational studies have suggested potential advantages of LBBAP in some domains. However, recent randomized controlled trials (RCTs) have reported conflicting results, and contemporary evidence has not been yet synthesized. OBJECTIVE: To compare the efficacy and safety of LBBAP/conduction system pacing (CSP) versus conventional BiVP for CRT using currently available randomized controlled trial data. METHODS: A syste…

Demographic Profile of Cardiac Resynchronization Therapy Recipients in a Saudi Tertiary Center: A Descriptive Single-Center Study.

OBJECTIVE: Women remain underrepresented in landmark cardiac resynchronization therapy (CRT) trials, despite evidence suggesting they may derive substantial clinical benefit. Age at implantation also influences outcomes and long-term management. However, demographic and clinical data on CRT recipients in Saudi Arabia remain limited. This study aimed to describe the sex distribution and age profile of CRT recipients in a tertiary Saudi center and to contextualize these findings within international CRT trial and registry data. MATERIALS AND METHODS: We retrospectively reviewed consecutive CRT …

Presence of Ineffective Cardiac Resynchronization Therapy Pacing Provides Insights Into Hidden Causes and Therapeutic Targets of Nonresponder.

INTRODUCTION: An effective cardiac resynchronization therapy (CRT) algorithm classifies each left ventricular (LV) pace as effective or ineffective based on electrogram morphology. Loss of capture, latency, pseudo-fusion during sinus rhythm, and rapid ventricular responses due to atrial fibrillation (AF) and premature ventricular conductions (PVCs) are potential causes of ineffective CRT pacing. However, little is known about the relationship between percentage of ineffective CRT pacing (%i-CRT) and CRT response. METHODS AND RESULTS: Among 136 consecutive CRT recipients, we evaluated 44 patie…

Echocardiographic response from left bundle branch area pacing optimized cardiac resynchronization therapy (LOT-CRT) vs traditional CRT.

BACKGROUND: Traditional cardiac resynchronization therapy (CRT) with biventricular pacing (BVP) may be less effective in patients with nonspecific intraventricular conduction delay (NIVCD). Left bundle branch area pacing (LBBAP) combined with left ventricular (LV) coronary venous lead pacing (LOT-CRT) may be more effective in these patients. OBJECTIVE: We assessed the echocardiographic response of LOT-CRT in patients with left bundle branch block (LBBB) or NIVCD and compared it with a propensity-matched BVP cohort. METHODS: Patients with conventional CRT indications and preferentially NIVCD …

QRS morphology and electrical dyssynchrony in patients before and after cardiac resynchronization therapy and after device optimization.

INTRODUCTION: QRS morphology and duration (QRSd) impact electrical dyssynchrony and the likelihood of response to cardiac resynchronization therapy (CRT). METHODS: Four groups of heart failure patients (n = 181) with CRT and left ventricular conduction delay were studied: Strauss left bundle branch block (Strauss LBBB; n = 73), LBBB not meeting Strauss criteria (non-Strauss LBBB; n = 29), interventricular conduction delay (IVCD; n = 54), and right ventricular pacing (RVp; n = 25). Electrical dyssynchrony was measured (using a novel proprietary technology) as systolic area under the curve (AUC…

Predicting cardiac resynchronization therapy response: development and validation of a single photon emission computed tomography-based nomogram.

BACKGROUND: Cardiac resynchronization therapy (CRT) is an effective treatment for patients with drug-refractory heart failure. However, more than thirty percent of patients do not benefit from CRT. This study aimed to develop and validate a novel model based on single photon emission computed tomography (SPECT) phase analysis features to predict CRT response. METHODS: We identified 163 CRT patients who received gated resting SPECT myocardial perfusion imaging (MPI) between 2010 and 2020 at The First Affiliated Hospital of Nanjing Medical University. All variables were first processed by univa…

Resolution of oral anticoagulation-resistant left atrial appendage thrombus by cardiac resynchronization therapy and atrioventricular nodal ablation.

UNLABELLED: We present a case of congestive heart failure with reduced ejection fraction complicated by atrial tachycardia and an oral anticoagulation-resistant left atrial appendage (LAA) thrombus. The LAA thrombus prevented sinus rhythm restoration, and the ventricular rate control was also difficult. Cardiac resynchronization therapy (CRT)-defibrillator implantation followed by atrioventricular nodal ablation was performed, and the patient's congestive heart failure improved. Furthermore, the resolution of the LAA thrombus was achieved three months after the implantation. The atrial tachyca…

Cardiac Resynchronization Therapy in Single-Ventricle Physiology: A Case Report of cc-TGA Managed With Fontan Strategy.

Cardiac resynchronization therapy (CRT) has been widely used in adult patients with heart failure and ventricular dyssynchrony, but its application in pediatric patients with congenital heart disease (CHD), particularly those with single-ventricle physiology, remains limited. We report the case of a male patient with congenitally corrected transposition of the great arteries (cc-TGA) and single-ventricle physiology who developed complete atrioventricular block and underwent single-chamber epicardial pacing at the age of four. A dual-chamber pacemaker was later implanted at the time of Glenn su…

Left bundle branch pacing versus biventricular pacing for cardiac resynchronization therapy: A systematic review and meta-analysis.

INTRODUCTION: Cardiac resynchronization therapy (CRT) reduces heart failure (HF) hospitalization and all-cause mortality in HF patients with left bundle branch block (LBBB). Biventricular pacing (BVP) is the gold standard for achieving CRT, but about 30%-40% of patients do not respond to BVP-CRT. Recent studies showed that left bundle branch pacing (LBBP) provided remarkable results in CRT. Therefore, we conducted a meta-analysis aiming to compare LBBP-CRT versus BVP-CRT in HF patients. METHODS: We systematically searched the electronic databases for studies published from inception to Decemb…
