Atopic dermatitis and ocular allergy: common mechanisms and uncommon questions.

PURPOSE OF REVIEW: Atopic dermatitis (AD) and ocular allergy aka allergic eye disease (AED) are two common conditions that often coexist in patients. However, molecular connections between these two conditions are incompletely understood. While common etiologic components including Th2 immune signaling have been suggested for AD and AED, the mechanism how current Th2-targetd therapies (dupilumab, tralokinumab) for AD can augment conjunctivitis is not well understood. RECENT FINDINGS: Differentially regulated genes and pathways relevant for AD disease manifestation are known. In contrast, simi

Reactivation of Alopecia Areata After Dupilumab Therapy for Atopic Dermatitis.

We present a case report of a 42-year-old woman with severe atopic dermatitis (AD) and history of alopecia areata (AA), which had been in remission for more than 5 years. She was treated with dupilumab for AD, and 4 months after initiating treatment, she developed a rapidly progressive reactivation of AA in an androgenic alopecia pattern. Alopecia resolved after a single intramuscular triamcinolone injection while the patient remained on dupilumab. This is the seventh report to date of dupilumab-associated alopecia but is the first case documenting reactivation of previous AA. Alopecia occurri

Allergy - Patients with Atopic Dermatitis Express Themselves Through a Questionnaire.

INTRODUCTION: Allergies are becoming more prevalent across the globe and can be linked to several skin diseases, particularly atopic dermatitis (AD). Disruption of the immune system in the skin can lead to inflammatory diseases such as atopic and contact dermatitis, skin infections, and allergies. This is especially evident in processes such as "atopic march", where in childhood, the development of atopic dermatitis can later lead to food allergies, allergic rhinitis and asthma. OBJECTIVE: The aim of this international online survey is to study the link between self-reported doctor-diagnosed

Atopic Dermatitis-Like Rash During Evolocumab Treatment of Familial Hypercholesterolemia.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a serine protease that targets the low-density lipoprotein (LDL) receptor for lysosomal degradation. PCSK9 impedes the receptor-mediated clearance of LDL-cholesterol, thereby increasing serum LDL-cholesterol levels. Evolocumab, a human monoclonal antibody against PCSK9, effectively reduces serum LDL-cholesterol levels. We report the first known case of a patient who developed an atopic dermatitis (AD)-like rash during evolocumab therapy. A 43-year-old Japanese man with heterozygous familial hypercholesterolemia was treated with subcutane

Crisaborole and atopic dermatitis skin biomarkers: An intrapatient randomized trial.

BACKGROUND: Crisaborole ointment 2% is a nonsteroidal phosphodiesterase 4 inhibitor for the treatment of mild-to-moderate atopic dermatitis (AD). The mechanism of action of crisaborole and its effects on lesional measures of disease severity are not yet well defined. OBJECTIVE: This phase 2a, single-center, vehicle-controlled, intrapatient study was designed to further characterize the mechanism of action of crisaborole through evaluation of clinical efficacy and changes in skin biomarkers in adults (n = 40) with mild-to-moderate AD. METHODS: Two target lesions were randomized in an intrapat

Histamine Modulates Sweating and Affects Clinical Manifestations of Atopic Dermatitis.

Many factors such as food or environmental allergens, bacteria, fungi, and mental stress aggravate the condition of atopic dermatitis (AD) eczema. Sweating can also exacerbate AD, and patients are aware of that. In the past, it has been reported that contamination of skin surface antigens by sweat induces acute allergic reactions and that sweating functions of AD patients via axonal reflexes are decreased. Histamine demonstrably inhibits acetylcholine-induced sweating in both mice and humans via histamine H1 receptor-mediated signaling. In sweat glands, acetylcholine inactivates glycogen synth

Synbiotics prevent asthma-like symptoms in infants with atopic dermatitis.

BACKGROUND: Infants with atopic dermatitis (AD) have a high risk of developing asthma. We investigated the effect of early intervention with synbiotics, a combination of probiotics and prebiotics, on the prevalence of asthma-like symptoms in infants with AD. METHODS: In a double-blind, placebo-controlled multicentre trial, ninety infants with AD, age <7\ months, were randomized to receive an extensively hydrolyzed formula with Bifidobacterium breve M-16V and a galacto/fructooligosaccharide mixture (Immunofortis(®) ), or the same formula without synbiotics during 12 weeks. After 1 year, the pr

Predicting long-term outcome in group treatment of atopic dermatitis.

BACKGROUND: Psychological group treatments are effective adjuncts to dermatological treatments in improving dermatological symptoms and quality of life. However, no data are available on variables that may predict which patients benefit from such group treatments. METHODS: On the basis of data from the Marburg group treatment study, we explored which variables are associated with successful treatment. Treatment outcome was defined by a marked reduction of dermatological symptoms at 1-year follow-up. RESULTS: Regardless of the treatment condition, a high pretreatment level of scratching, a lo

Atopic dermatitis: molecular mechanisms, clinical aspects and new therapeutical approaches.

Atopic dermatitis (AD) is a genetically determinated, chronic inflammatory skin disorder associated with cutaneous erythema and severe pruritus, affecting 10-15% of children with increasing incidence and socio-economical relevance. Frequently, AD is associated with development of allergic rhinitis and/or asthma later in childhood. In most of patients AD is associated with a sensitization to food and/or environmental allergens and increased serum-IgE, while only a fewer percentage missed links to the classical atopic diathesis. Currently investigated pathogenetic aspects of AD include imbalance
