Endoscopic annular chondroperichondrial tympanoplasty, technical description.

PURPOSE: Tympanoplasty is a surgical procedure performed to cure middle ear infections and restore normal middle ear function. It is one of the most common procedures in otological surgery. Since Wullstein described tympanoplasty, the microscope has been a widely used surgical tool in otological surgery. Transcanal endoscopic ear surgery (TEES) has come to the forefront as an alternative option due to the various limitations of microscopic surgery and the development of otological surgical techniques. When reviewing the types and tecniques of grafts used in tympanoplasty in the current literat

Benefits of Cartilage Conduction Hearing Aids for Speech Perception in Unilateral Aural Atresia.

Severe conductive hearing loss due to unilateral aural atresia leads to auditory and developmental disorders, such as difficulty in hearing in challenging situations. Bone conduction devices compensate for the disability but unfortunately have several disadvantages. The aim of this study was to evaluate the benefits of cartilage conduction (CC) hearing aids for speech perception in unilateral aural atresia. Eleven patients with unilateral aural atresia were included. Each participant used a CC hearing aid in the atretic ear. Speech recognition scores in the binaural hearing condition were obta

Otoplasty: evaluation, technique, and review.

Prominent ear deformity is the most common abnormality of the external ear. Over two hundred different techniques have been described to correct this deformity. Many of these techniques have proven successful in their ability to achieve high patient satisfaction despite the significant variations in these techniques. From this perspective otoplasty is a privileged procedure allowing the surgeon great latitude in his approach and ability to achieve patient satisfaction. Despite high patient satisfaction, each technique has inherent strengths and weaknesses. The art of otoplasty is in the abilit

Modification of the cartilaginous framework for autologous ear reconstruction: Construction of a stable complete ring framework with grander highs and lows.

BACKGROUND: Successful microtia reconstruction involves fabrication of a framework with well-defined ear subunits. Tragal definition and deep conchal bowl are key elements to produce a natural well-defined and contoured ear. We describe a modification in the cartilage framework with the purpose of increasing framework stability, tragal definition and conchal bowl depth. METHODS: The tragus is placed on two cartilaginous bars (L-shaped), which are fixed to the framework base block creating a complete ring. These increase tragal projection and conchal depth. The tragus is carved angled posterio

Separating the helix from the antihelix: a new concept in prominent ear correction.

BACKGROUND: For over a century, various otoplasty techniques for correction of the prominent ear have been attempted. Nevertheless, cartilage memory strength, mainly in the thick cartilage (and consequently the recurrence of the prominence), still remains a problem. An additional difficulty relates to the antihelix irregularities caused by attempts to weaken the cartilage spring, which are time-consuming and in some cases lead to an unnatural-looking result. OBJECTIVE: The technique reported in this article represents a new approach to otoplasty that deconstructs the auricular pavilion with a

Reconstruction of Microtia Using a Single Expanded Postauricular Flap without Skin Grafting: Experience of 683 Cases.

BACKGROUND: Auricular reconstruction of congenital microtia patients is a huge challenge for plastic surgeons worldwide. Current techniques usually use a retroauricular fascial flap with skin grafting, often proving to be color mismatched, complex, and with visible scarring. An uncomplicated strategy that results in minimal scarring and a good color match is still required, especially for Asian patients. In the present work, the authors developed a modified technique for auricular reconstruction using a single expanded flap combined with a cartilage framework. METHODS: Auricular reconstructio

Round window membrane rupture following blunt force trauma.

Round window rupture following barotrauma such as diving and air travel is well documented. However, we describe a case of round window rupture following a slap to the ear, which has not been described previously. A 12-year-old boy was slapped by his teacher in rural Nepal over his left ear and immediately noted hearing loss. There was no dizziness or tinnitus. He presented to our unit 21 days after the trauma and examination showed a perforation of the tympanic membrane in the anterior inferior quadrant with an otherwise normal ear examination and no nystagmus seen. Audiogram showed a left pr

Otoplasty: sequencing the operation for improved results.

: After studying this article, the participant should be able to: 1. Understand the anatomy and embryology of the external ear. 2. Understand the anatomic causes of the prominent ear. 3. Understand the operative maneuvers used to shape the external ear. 4. Be able to sequence the otoplasty for consistent results. 5. Understand the possible complications of the otoplasty procedure. Correction of prominent ears is a common plastic surgical procedure. Proper execution of the surgical techniques is dependent on the surgeon's understanding of the surgical procedure. This understanding is best found

Secondary surgery for the unsatisfactory auricle after auricular reconstruction.

OBJECTIVES: Complications, including framework exposure, infections, and reconstructed auricle deformation, may occur after auricular reconstruction. However, reports on surgical methods for cases with unsatisfactory outcomes after auricular reconstruction using an autologous costal cartilage are insufficient. Herein, we summarized retreatment casesfor poor ear morphology in patients who had undergone auricular reconstruction in our department for 5 years and discussed other techniques. METHODS: Between September 2014 and September 2019, 24 ears of 24 patients with poor morphology, unsatisfac

Small spheroids for head and neck cartilage tissue engineering.

The demand for cartilage reconstruction in the head and neck region arises frequently due to trauma, malignancies, and hereditary diseases. Traditional tissue engineering produces cartilage from a small biopsy by combining biomaterials and expanded cells. However, this top-down approach is associated with several limitations, including the non-uniform distribution of cells, lack of physiological cell-cell and cell-matrix interactions, and compromised mechanical properties and tissue architecture. The capacity of cells to aggregate into microtissues enables an alternative bottom-up approach to
