Exploring the electrophysiology of Parkinson's disease with magnetoencephalography and deep brain recordings.

Aberrant information processing in the basal ganglia and connected cortical areas are key to many neurological movement disorders such as Parkinson's disease. Investigating the electrophysiology of this system is difficult in humans because non-invasive methods, such as electroencephalography or magnetoencephalography, have limited sensitivity to deep brain areas. Recordings from electrodes implanted for therapeutic deep brain stimulation, in contrast, provide clear deep brain signals but are not suited for studying cortical activity. Therefore, we combine magnetoencephalography and local fiel

[Anesthesia considerations for deep-brain stimulation in a patient with type-2 pantothenate kinase deficiency (Hallervorden-Spatz disease)].

Neurodegeneration associated with pantothenate kinase deficiency is an autosomal recessive condition caused by mutations in the pantothenate kinase 2 gene (PANK2). Clinical characteristics include progressive motor impairment and dementia. Medical treatment is limited and the dystonia tends to be refractory, making stereotactic surgery with placement of deep-brain electrodes an option that is being adopted with greater frequency in these patients. We report the case of a 32-year-old woman with severe dystonia associated with PANK2 protein deficiency. The patient was scheduled for stereotactic

Electrical pelvic floor stimulation: a possible alternative treatment for reflex urinary incontinence in patients with spinal cord injury.

The present study demonstrates the clinical experience of pelvic floor stimulation using percutaneous implantable electrodes and implantable electrical stimulator for the treatment of reflex urinary incontinence in patients with spinal cord injury. Pelvic floor stimulation was carried out on six paraplegic patients who had urinary incontinence from an overactive bladder. After the percutaneous implantation of a pair of electrodes, chronic stimulation was carried out by employing an implanted receiver or an external pulse regulator. Within 4 to 16 weeks of electrical stimulation urinary inconti

Artificial reflex arc: a potential solution for chronic aspiration. III: Stimulation of implanted cervical skin as a functional graft triggering glottic closure in the canine.

Stroke often impairs the ability of the vocal cords to close during deglutition, resulting in aspiration. The current study is a further advance on the application of the concept of an artificial reflex arc during swallowing. The second and third cervical branches were bilaterally isolated in four dogs and baseline sensory nerve traffic was detected via tripolar perineural electrodes. Cervical skin pressure-induced suprathreshold signals were used to trigger vocal cord contraction via bipolar electrodes passed around the recurrent laryngeal nerves. Skin subtended by the cervical nerves was imp

Alpha 2-noradrenergic modulation of hippocampal theta activity.

The purpose of the present study was to determine the role of norepinephrine in the generation of hippocampal theta activity. Experiments were performed on urethane-anesthetized rats, implanted with recording electrodes in the dentate gyrus and stimulating electrodes in the dorso-medial posterior hypothalamus. The effects of norepinephrine on hippocampal theta activity was studied by directly infusing norepinephrine and other noradrenergic agents into the hippocampus. Norepinephrine microinfusion produced a decrease in the amplitude of theta activity as observed in the polygraph chart record.

The relationship of temperature profiles to frequency during interstitial hyperthermia.

Regional hyperthermia is currently being investigated as a potential adjuvant to radiation therapy treatment of malignant disease. Since tumor response is directly related to treatment temperature, thermal distributions within tumors and surrounding normal tissue must be predictable under various conditions. Normal canine muscle was heated to approximately 42 degrees C with radiofrequency current fields over the frequency range of 500 kHz to 300 MHz. With two rows of four interstitially implanted needles acting as electrodes, thermal profiles show that temperature increases occurred between th

[Cardiac stimulation in children. 20 years' experience].

Between July, 1967 and December, 1987, 71 children and adolescents (43 boys, 28 girls) aged from 9 days to 20 years (mean 7.8 years) underwent pacemaker implantation. In 91 p. 100 of the cases this treatment was performed for complete atrioventricular block (CAVB). Surgical CAVB was the reason for 59 p. 100 of implantations (correction of tetralogy of Fallot and, more recently, of complex cardiopathies), the second main reason (16.9 p. 100) being nonsurgical CAVB associated with heart disease; the children in this group were young (mean age 4.2 years), and the prognosis mainly depended on the

Subdural recordings from an awake human brain for measuring current intensity during transcranial direct current stimulation.

Transcranial direct current stimulation (tDCS) is an emerging method, used for non-invasively stimulating the brain in normal healthy subjects and in patients with neurological disorders. However, the pattern of the spatial distribution of the current intensity induced by tDCS is poorly understood. In this study, we directly measured the spatial characteristics of the current intensity induced by tDCS using an intracranial strip electrode array implanted over the motor cortex in patients with Parkinson's disease undergoing deep brain stimulation lead placement surgery. We used a bilateral stim

Positron Emission Tomography Imaging Reveals Auditory and Frontal Cortical Regions Involved with Speech Perception and Loudness Adaptation.

Considerable progress has been made in the treatment of hearing loss with auditory implants. However, there are still many implanted patients that experience hearing deficiencies, such as limited speech understanding or vanishing perception with continuous stimulation (i.e., abnormal loudness adaptation). The present study aims to identify specific patterns of cerebral cortex activity involved with such deficiencies. We performed O-15-water positron emission tomography (PET) in patients implanted with electrodes within the cochlea, brainstem, or midbrain to investigate the pattern of cortical

Sculpting the temporal bone: an easy reversible cochlear implant electro-array stabilization technique.

PURPOSE: Since the beginning of cochlear implant (CI) surgery, several techniques to fixate the electrode array at the cochleostomy and stabilize it have been described; however, most techniques use autologous tissues such as fascia, muscle, fat or fibrin glue. We describe a new surgical technique aimed to stabilize the electrode array of a CI without using autologous tissues or artificial materials. MATERIALS AND METHODS: The surgical technique described consists in creating three stabilizing channels in the temporal bone for the electrode array. The first one in a partially opened aditus, t

A new approach towards a minimal invasive retina implant.

The possibility of using retina implants ('retinal prostheses') for the restoration of basic orientation in blind patients suffering from distal retinal diseases is presently under investigation by at least 18 independent project groups worldwide. It is a common feature of all implants to bypass degenerated retinal layers and to transfer visual information into the retinal network either by direct electrical stimulation or by neurotransmitter release. Contemporary implant designs are differing in the position of stimulating electrodes (epiretinal, subretinal, external) and the anatomical arran

Unveiling patterns of peri-lead edema after deep brain stimulation: a retrospective review of clinical and demographic factors.

OBJECTIVE: Postoperative peri-lead edema (PLE) is an increasingly recognized complication of deep brain stimulation (DBS), a therapeutic intervention commonly used for neurological conditions such as Parkinson's disease (PD), essential tremor (ET), intractable focal epilepsy, and dystonia. In this study, we conducted a retrospective chart review to evaluate the incidence of PLE and explore potential clinical and demographic risk factors. METHODS: A single physician performed DBS electrode placements. To check for complications, postoperative computed tomography (CT) scans were conducted on th

Implantable bioelectrodes: challenges, strategies, and future directions.

Implantable bioelectrodes for regulating and monitoring biological behaviors have become indispensable medical devices in modern healthcare, alleviating pathological symptoms such as epilepsy and arrhythmia, and assisting in reversing conditions such as deafness and blindness. In recent years, developments in the fields of materials science and biomedical engineering have contributed to advances in research on implantable bioelectrodes. However, the foreign body reaction (FBR) is still a major constraint for the long-term application of electrodes. In this paper, four types of commonly used im

Influence of Cochlear Anatomy on Intraoperative Electrically Evoked Compound Action Potentials.

Objective: The electrically evoked compound action potential (ECAP) is an objective measure to indirectly assess spiral ganglion neurons. The ECAP provides inputs about the prognoses of cochlear implant (CI) recipients. Several factors such as cochlear morphology can affect ECAP measurements. This study aims to investigate the variation effect of cochlear parameters on intraoperative ECAP thresholds. Methods: This is a retrospective study on patients who underwent CI surgery with normal inner ear morphology at our center between 2017 and 2023. Cochlear anatomical parameters, including diameter

Auditory brainstem stimulation with a conformable microfabricated array elicits responses with tonotopically organized components.

Auditory brainstem implants (ABIs) restore hearing to deaf individuals not eligible for cochlear implants. Speech comprehension in ABI users is generally poor compared to that of cochlear implant users, and side effects are common. The poor performance may result from activating broad areas and multiple neuronal populations of the cochlear nucleus, however detailed studies of the responses to surface stimulation of the cochlear nucleus are lacking. A conformable electrode array was microfabricated to fit on the rat's dorsal cochlear nucleus (DCN). It hosts 20 small electrodes (each 100 μm diam

Extensive monitoring during auditory brainstem implant surgery.

In patients with reduced auditory nerve function, for example due to tumour removal or an accident, hearing rehabilitation can be elicited by an auditory brainstem implant (ABI). The electrode array of the ABI manufactured by Cochlear Ltd., Sydney, consists of 21 circled contacts in a silicon carrier. This is inserted in the lateral recess of the fourth ventricle. Since 1996, in Hannover eight patients have been implanted with a cochlear ABI Nucleus 21 + 1. All of them were profoundly deaf on both sides due to neurofibromatosis type 2 (NF2). To find the optimal electrode position during surger

Assessment of neural and MAP level asymmetries in a large cohort of children with bilateral cochlear implants.

Bilateral cochlear implants (BCIs) are provided to children who are deaf in both ears to restore binaural/spatial hearing, but interaural stimulation mismatches can limit potential benefits. This study aimed to: 1) investigate how the programming of stimulation parameters in BCI users differs between bilateral pairs of electrodes, and (2) evaluate the impact of sequential implantation and mismatched array types on asymmetries. A mixed effects modeling analysis assessed cochlear implants (CI) stimulation parameters and peripheral neural responses retrospectively collected (September 2003-July 2
