Aging, motor skill, and the energy cost of walking: implications for the prevention and treatment of mobility decline in older persons.

BACKGROUND: Age-associated neural changes profoundly affect the biomechanics and energetics of walking, increase energy cost, and require novel approaches to exercise that focus on motor learning theory. METHODS: We present a conceptual framework for motor skill in walking, its effect on the energy cost of walking, and the influence of the aging brain. RESULTS: Motor learning theory and practice can be incorporated into interventions to promote skilled, energy efficient walking in older people. CONCLUSIONS: An extensive literature on motor skill and motor learning, derived from neuroscience

Efficacy of physical exercise intervention on mobility and physical functioning in older people with dementia: a systematic review.

Numerous trials have shown that physical activity and exercise training have beneficial effects in general older populations. However, few have studied its effectiveness among people with dementia. The aim of this systematic review is to examine the efficacy of trials using a rigorous randomised, controlled design and including physical activity or exercise as a major component of intervention on the physical functioning, mobility and functional limitations of people with dementia. We found 20 randomised controlled trials that included a total of 1378 participants. Of these, only three were of

Education predicts incidence of preclinical mobility disability in initially high-functioning older women. The Women's Health and Aging Study II.

BACKGROUND: To examine the impact of educational attainment on the incidence of preclinical mobility disability (PCD). METHODS: The Women's Health and Aging II Study is a prospective observational cohort study of 436 initially high-functioning community-dwelling women aged 70-79 years at baseline in Baltimore, Maryland. We measured the association of highest attained education level with preclinical mobility disability (PCD) over an 11-year period. PCD is defined as self-reported modification in any of four tasks without reporting difficulty in those tasks. The tasks were walking ½ mile, clim

Addressing the nonexercise part of the activity continuum: a more realistic and achievable approach to activity programming for adults with mobility disability?

Participation in physical activity is fundamental for the maintenance of metabolic health and the prevention of major chronic diseases, particularly type 2 diabetes and cardiovascular disease. A whole-of-day approach to physical activity promotion is increasingly advocated and includes not only increasing moderate-intensity physical activity but also reducing sedentary time and increasing light-intensity activity (the "nonexercise" part of the activity continuum). This whole-of-day approach to tackling the challenge of inactivity may be particularly relevant for adults with mobility disabiliti

Evaluation of sit-to-stand motion using a pressure distribution measurement system--effect of differences in seat hardness on sit-to-stand motion.

PURPOSE: This study examined differences between standing from an ordinary seat and standing from a low-repulsion mat using a pressure distribution measurement system (BIG-MAT) and identified parameters to determine sit-to-stand (STS) motion difficulty. METHOD: Ten healthy male volunteers (aged 30-38 years) participated. During STS motion from an ordinary seat and from a low-repulsion urethane mat, plantar surface pressure changes of both feet and the centre of pressure (COP) trajectory were recorded for 7  s. This series of tests was performed four times in each subject. Left and right press

Reconstruction of neglected Achilles' tendon defect with peroneus brevis tendon allograft: a case report.

This case report describes the use of a peroneus brevis allograft to reconstruct a neglected Achilles' tendon injury in a 75-year-old woman. She had difficulty walking, had stiffness, and was unable to perform a heel raise from a single-leg standing position. Magnetic resonance imaging confirmed a 6.8-cm wide defect 13 months after the initial injury. Surgical repair of the neglected rupture was performed using 4 strands of peroneus brevis allograft to bridge the defect. Early weight bearing and rehabilitation was allowed. At follow-up clinical examination 14 months postoperation, the patient

Progressive bony dysplasia in Apert syndrome.

Ten patients with Apert syndrome (type I acrocephalosyndactyly) were studied. Seven of these patients were observed from infancy for periods ranging from 2 1/2 to 22 years. The patients presented with limited joint mobility and were found to have multiple radiographic abnormalities, including subluxated or flattened humeral heads, irregularities of the glenoid cavity, and early fusion of the calcaneus to the cuboid bone. The limitation of motion and bony defects were progressive, suggesting that Apert syndrome is a generalized dysplasia.

Strain-induced localization as a universal mechanism toward high mobility in 2D semiconductors.

Two-dimensional (2D) semiconductors with atomic thickness have shown great potential for application in high-performance microelectronic devices due to the lack of short channel effects, but their carrier mobility is often limited by strong electron-phonon coupling (EPC) compared to that of three-dimensional (3D) semiconductors. Particularly, the hole mobility in 2D materials is typically much lower than the electron mobility, creating a critical bottleneck for realizing complementary electronics. In this work, we propose and validate a universal strategy to significantly enhance mobility by t

Assessing pressure injury risk using a single mobility scale in hospitalised patients: a comparative study using case-control design.

BACKGROUND: Pressure injury is known to cause not only debilitating physical effects, but also substantial psychological and financial burdens. A variety of pressure injury risk assessment tools are in use worldwide, which include a number of factors. Evidence now suggests that assessment of a single factor, mobility, may be a viable alternative for assessing pressure injury risk. AIMS: The aim of this study was to ascertain whether using the Braden mobility subscale alone is comparable to the full Braden scale for predicting the development of pressure injury. METHODS: This study, a retrosp

Prevention of postoperative shoulder stiffness after Rotator Cuff Repair through a structured self-rehabilitation protocol.

INTRODUCTION: Postoperative shoulder stiffness is a frequent complication after arthroscopic rotator cuff repair (RCR) and may delay early recovery. The influence of preoperative mobility, rehabilitation strategy, and immobilization position on postoperative stiffness and functional outcomes remains debated. HYPOTHESIS: Preoperative limitation of shoulder mobility and early postoperative stiffness influence mid-term functional recovery, and supervised self-rehabilitation may reduce early stiffness compared with classical physiotherapy. METHODS: A retrospective cohort of 362 patients undergoi
