Lifetime Noise Exposure and Its Interaction With Ageing: Effects on Audiometric Thresholds, Speech-In-Noise Perception, and Tinnitus for Palestinian Workers.

Noise-induced and age-related hearing loss involve partially similar pathophysiology, but their combined effects remain poorly understood, especially in under-regulated occupational settings. This study investigated the independent and interaction effects of lifetime noise exposure and age on audiometric, speech-in-noise, and tinnitus outcomes in Palestinian workers. Ninety-eight adult workers aged 18 - 75 years completed standard (0.25 - 8 kHz) and extended high-frequency (10 - 14 kHz) audiometry, Arabic diotic and antiphasic digits-in-noise (DIN) testing, and an Arabic Noise Exposure Structu

Speech perception deficits and neural network alterations during cocktail-party listening in age-related hearing loss.

Age-related hearing loss (ARHL) is associated with difficulty understanding speech in complex listening environments, yet its neural basis remains unclear. We combined behavioral measures with functional near-infrared spectroscopy (fNIRS) to examine speech perception and neural responses during cocktail-party listening. Sixty-eight participants (Young-NH, Older-NH, ARHL) completed a spatialized speech-recognition task under different masker types and signal-to-noise ratios, together with resting-state and task-based fNIRS recordings. ARHL showed poorer speech perception and reduced self-report

Reaction times of cochlear implant users to a change in electrode or modulation frequency in the presence of a masker.

Spatial and temporal selectivity vary substantially between cochlear implant users, resulting in large differences in speech perception. Direct electrode stimulation offers precise control to capture this variability. However, most studies using direct stimulation have not incorporated interfering stimuli, which may not reflect the complex nature of speech and ambient noise. To address this gap, we introduce the Electrode Interaction Reaction Time (ELI-RT) paradigm, which uses amplitude-modulated, interleaved pulses on two closely spaced electrodes at clinical pulse rates. ELI-RT includes two

Attenuation and distortion components of age-related hearing loss: Contributions to recognizing temporal-envelope filtered speech in modulated noise.

Older adults with hearing loss may experience difficulty recognizing speech in noise due to factors related to attenuation (e.g., reduced audibility and sensation levels, SLs) and distortion (e.g., reduced temporal fine structure, TFS, processing). Furthermore, speech recognition may improve when the amplitude modulation spectrum of the speech and masker are non-overlapping. The current study investigated this by filtering the amplitude modulation spectrum into different modulation rates for speech and speech-modulated noise. The modulation depth of the noise was manipulated to vary the SL of

Cognitive Function Prediction of Performance During Dual-Tasks Across Adults With and Without Hearing Loss.

BACKGROUND: Dual-task procedures are commonly implemented to examine cognitive load and listening effort as individual differences in cognition often determine successful listening. However, which methods are most efficacious is unclear. Specifically, standardized, targeted assessment procedures for establishing cognitive function, and age-related changes that might account for changes in dual-task performance have yet to be established. Additional data are needed across aging populations, including middle-aged adults and older adults to establish the trend of performance changes throughout th

Impact of stimulus-related factors and hearing impairment on listening effort as indicated by pupil dilation.

Previous research has reported effects of masker type and signal-to-noise ratio (SNR) on listening effort, as indicated by the peak pupil dilation (PPD) relative to baseline during speech recognition. At about 50% correct sentence recognition performance, increasing SNRs generally results in declining PPDs, indicating reduced effort. However, the decline in PPD over SNRs has been observed to be less pronounced for hearing-impaired (HI) compared to normal-hearing (NH) listeners. The presence of a competing talker during speech recognition generally resulted in larger PPDs as compared to the pre

Cochlear neuropathy in human presbycusis: Confocal analysis of hidden hearing loss in post-mortem tissue.

Recent animal work has suggested that cochlear synapses are more vulnerable than hair cells in both noise-induced and age-related hearing loss. This synaptopathy is invisible in conventional histopathological analysis, because cochlear nerve cell bodies in the spiral ganglion survive for years, and synaptic analysis requires special immunostaining or serial-section electron microscopy. Here, we show that the same quadruple-immunostaining protocols that allow synaptic counts, hair cell counts, neuronal counts and differentiation of afferent and efferent fibers in mouse can be applied to human t

Agreement between psychophysical tuning curves and the threshold equalizing noise test in dead region identification.

OBJECTIVE: Identification of dead regions is currently based on the results of psychophysical tuning curves (PTC) and the threshold equalizing noise (TEN) test. Summers et al (2003) found poor agreement (56%) between PTC- and TEN test results. Kluk and Moore (2005) argued that these results are explained by beat- and/or combination tone detection. The purpose of this study was to examine the diagnostic dead region relationship between PTCs, modified for beats and/or combination tone detection, and the TEN test with levels calibrated in hearing levels (HL), i.e., TEN[HL]. DESIGN: Twenty-four h

Speech perception in tinnitus is related to individual distress level - A neurophysiological study.

Individuals suffering from tinnitus often complain about difficulties understanding speech in noisy environments even in the absence of a peripheral hearing loss. This EEG study aimed to investigate whether aspects of phonetic perception are affected by the experience of tinnitus. We examined a sample of individuals with chronic, subjective tinnitus (n = 30, age range 30-50 yrs.), who underwent behavioural screening (standard tinnitus questionnaires) and comprehensive audiometric testing that covered peripheral and central hearing abilities (pure tone audiometry, suprathreshold audiometry (fre

Growth of suppression using distortion-product otoacoustic emission measurements in hearing-impaired humans.

Growth of distortion-product otoacoustic emission suppression was measured in 65 subjects with mild-to-moderate sensorineural hearing loss (HI). Measurements were made at four probe frequencies (f(2)) and up to five L(2) levels. Eleven suppressor frequencies (f(3)) were used for each f(2), L(2) combination. These data were compared to data from normal-hearing (NH) subjects (Gorga et al., 2011a). In both NH and HI subjects, growth of suppression depended on the relation between f(2) and f(3), such that the slope was close to one when f(3) ≈ f(2), steeper than one when f(3) < f(2), and shallower

Individual differences in behavioral estimates of cochlear nonlinearities.

Psychophysical methods provide a mechanism to infer the characteristics of basilar membrane responses in humans that cannot be directly measured. Because these behavioral measures are indirect, the interpretation of results depends on several underlying assumptions. Ongoing uncertainty about the suitability of these assumptions and the most appropriate measurement and compression estimation procedures, and unanswered questions regarding the effects of cochlear hearing loss and age on basilar membrane nonlinearities, motivated this experiment. Here, estimates of cochlear nonlinearities using te

The effects of age and cochlear hearing loss on temporal fine structure sensitivity, frequency selectivity, and speech reception in noise.

Temporal fine structure (TFS) sensitivity, frequency selectivity, and speech reception in noise were measured for young normal-hearing (NHY), old normal-hearing (NHO), and hearing-impaired (HI) subjects. Two measures of TFS sensitivity were used: the "TFS-LF test" (interaural phase difference discrimination) and the "TFS2 test" (discrimination of harmonic and frequency-shifted tones). These measures were not significantly correlated with frequency selectivity (after partialing out the effect of audiometric threshold), suggesting that insensitivity to TFS cannot be wholly explained by a broaden

Comparison of phonological and whole-word treatments for two contrasting cases of developmental dyslexia.

This study compared the effectiveness of two reading treatment programmes for two contrasting developmental dyslexics. W.B. demonstrated "pure" phonological dyslexia (deficient nonword reading but normal irregular-word reading) and N.S. "pure" surface dyslexia (the converse pattern). Both participants completed: (a) a phonological programme, which targeted the sublexical reading procedure through repeated exposure to word "families" with the same grapheme-phoneme correspondence (GPC; e.g., frail, raid, bait); and (b) a whole-word programme, which targeted the lexical reading procedure through

Spatiotemporal Dynamics of Covert Attention With Different Degrees of Central Visual Field Defects: An ERP and sLORETA Study.

PURPOSE: The present study aimed to investigate the spatiotemporal dynamics of covert attention by simulating different degrees of central visual field defects in healthy subjects. METHODS: An electroencephalogram (EEG) was recorded while 40 normal-sighted subjects performed a target discrimination task. Target stimuli simulated different defect degrees of the central visual field by artificially central scotomas (5, 10, 20, and 30 degrees of visual angle) masked on the center of black-and-white checkerboards. Event-related potentials (ERPs) and standardized low-resolution brain electromagnet

Airflow restriction mask induces greater central fatigue after a non-exhaustive high-intensity interval exercise.

The airflow restriction mask (ARM) is a practical and inexpensive device for respiratory muscle training. Wearing an ARM has recently been combined with high-intensity interval exercise (HIIE), but its effect on neuromuscular fatigue is unknown. The present study investigated the effects of ARM wearing on neuromuscular fatigue after an HIIE session. Fourteen healthy men performed two HIIE sessions (4 × 4 min at 90% HRmax , 3 min recovery at 70% HRmax ) with or without an ARM. Neuromuscular fatigue was quantified via pre- to post-HIIE changes in maximal voluntary contraction (MVC), voluntary ac

Digital Twin Control Framework Uncovers Individualized Cross-Scale Links Between Functional Connectivity Deviations, Communication Impairment, and Molecular Mechanisms in Autism.

BACKGROUND: Autism spectrum disorder (ASD) is hallmarked by pronounced inter-individual heterogeneity in brain function and clinical manifestations. Conventional neuroimaging studies yield highly inconsistent findings, as extrinsic confounding factors and intrinsic disease heterogeneity mask ASD-specific functional connectivity (FC) signatures, hampering the identification of individualized neural mechanisms linked to symptom severity. METHODS: We developed a digital twin control analytical framework based on deep contrastive learning. By comparing each ASD participant with their matched digi

Case Report: What Are We Doing for Our "20/20 Unhappy" Scleral Lens Patients?

SIGNIFICANCE: Scleral lenses (SLs) partially mask higher-order aberrations (HOAs) in highly aberrated eyes. Although visual acuity (VA) may show satisfactory quantitative clinical outcomes during SL wear, residual (uncorrected) HOAs can leave subjective visual quality goals unmet. PURPOSE: The purpose of this study was to report a case where a "20/20 unhappy" patient with SLs was able to meet visual goals with wavefront-guided SLs. CASE REPORT: A 40-year-old male with bilateral keratoconus, whose Snellen VA with SLs was 20/20 right eye (OD) 20/16 left eye (OS), reported halos and glare at ni

Neural correlates of "analytical-specific visual perception" and degree of task difficulty as investigated by the Mangina-Test: a functional magnetic resonance imaging (fMRI) study in young healthy adults.

The Mangina-Test is a neuropsychometric method for evaluating varying degrees of "analytical-specific perception" as they relate to learning abilities and disabilities. It consists of the identification of simple stimuli which are masked within a complex configuration according to their exact size, dimension, direction, spatial orientation, and shape within a limited span of time. This test has been successfully applied in clinical settings for the assessment of cognitive abilities and disorders in young and elderly populations. This investigation aimed to examine the neural correlates of anal

Dual-task costs of listening while driving in older adults with and without audiometric hearing loss: Behavioural and neurophysiological outcomes.

Age-related hearing loss may increase listening difficulties in challenging listening conditions (e.g., speech-in-noise), limiting cognitive resources available to perform common, complex multitasking behaviours like listening while driving. Older adults with hearing loss may compensate by increasing prefrontal cortex (PFC) activation in response to multitasking demands. Few realistic, controlled studies have examined how competing attentional demands of listening while driving affect performance and brain activation, and how these patterns may differ between older adults with and without audi
