Choriocarcinoma of the nonmolar placenta is presumptively a rare entity and is usually associated with widespread maternal metastases at the time of diagnosis. Nonmetastatic disease goes unrecognized and undiagnosed because placental carcinoma can be limited to a few villi, and grossly evident disease is often misinterpreted as a placental infarct. The optimal treatment for patients with choriocarcinoma of the placenta is not known but aggressive combination chemotherapy is suggested for patients with metastatic disease. Historically, the prognosis for both mother and infant has been poor.

During an epidemic of visceral leishmaniasis in the Sudan, two cases of congenital kala-azar were seen. The first child, whose mother had contracted kala-azar in southern Sudan, was born in Khartoum, where no transmission of leishmaniasis is currently occurring. At seven months, the child had fever, lymphadenopathy, and hepatosplenomegaly; leishmania parasites were detected in the bone marrow. Acute cocaine intoxication has been associated with elevated blood pressure and placental abruption. A retrospective analysis was performed comparing gestational age at the time of placental abruption and response to conventional therapy for elevated blood pressure between patients known to have ingested cocaine and those who were drug free. Data suggest that cocaine ingestion during pregnancy increases the risk of early placental abruption and an elevation of blood pressure that is not as responsive to conventional therapy as pregnancy-induced hypertension.

The distribution of fibronectin and laminin was investigated in early intrauterine and ectopic tubal pregnancy using a standard immunoperoxidase technique on paraffin-embedded tissues. Localization of both fibronectin and laminin appeared identical in intrauterine and in ectopic pregnancy. The management of abruptio placentae remains a problem, despite advances in medical science. This study analyses severity of abruption, resultant morbidity, and maternal and fetal mortality in 105 cases. Among the contributory factors, hypertension (44%) and grand multiparity (24%) were common. There was no maternal death among these cases, mainly because of liberal use of blood transfusion, prompt delivery, and careful fetal monitoring, but perinatal mortality remained high (73%). Factors responsible for this high perinatal mortality and its prevention are discussed.

An investigation was done to clarify the pathological background of pre-eclampsia. Thirty four placentae were studied immunopathologically in comparison to control cases. The results showed slightly positive stains of hCG containing alpha- and beta-subunits in syncytiocytotrophoblastic layers especially in severe pre-eclampsia cases. In the authors' department, 175 cases of abruption of the placenta - 0.65% of all births at the department - were treated in the course of 15 years. The sex ratio of fetuses in association with this complication was 131.08; higher than those of all neonates during the same period of time. A case of severe abruptio placentae evolved during the course of prepartum electronic fetal heart rate (FHR) monitoring which was associated with nipple stimulation. The fetal monitor tracing and clinical course are discussed. The unpredictability of abruptio placentae is underscored by the normal appearance of the FHR tracing immediately preceding the event. The association of nipple stimulation with abruptio placentae in this case raises concerns about the potential dangers of this surveillance technique.

The results of studies on 2396 women are presented. Relations were found between location of placenta and 'ripeness' of uterine cervix, duration of labour, prognostic value of intrauterine pressure, complications of the third stage, fate of the newborn and success of induction of labour. The authors arrived at the conclusion that attachment of the placenta to the fundus uteri may be considered an unfavourable location causing an increase in the number of different deviations from normal labour.

Regionally different structures of placental terminal villi in the placentones have been shown in earlier publications. In this study the earlier results should be tested by means of histometry. The earlier results could be proved. It was shown that the villi in the centers of the circulation units are less differentiated than those in the peripherie of the placentones. The results were statistically significant. The reasons for the regionally different architecture of the placental villi are discussed with respect to the results of other authors working in the same field.

Angiosarcoma of the breast with spread to the ovary and placenta during a pregnancy 6 years after initial diagnosis is described. The ovarian lesion was discovered during Caesarean section. It is postulated that the pregnancy altered the biological behaviour of the tumor.
