We present clinical experience with 110 reconstructive procedures in 25 patients with epidermolysis bullosa, a group of rare heritable disorders characterized by marked fragility of the skin and mucosa. We discuss management of hand and foot deformities unique to epidermolysis bullosa patients, excision of squamous cell carcinoma, and reconstruction of oral, nasal, and ocular tissues. All patients underwent these procedures without major surgical or anesthetic complications. We analyze surgical outcome and formulate guidelines to avoid damaging the skin and mucosa.

In 1991, 146 consultant plastic surgeons in Britain and Ireland were sent a short questionnaire about their policies for excision margins for primary cutaneous malignant melanoma. 106/146 (73%) replied. 39/106 (37%) considered narrow margin (2-3 mm) excision biopsies adequate in certain cases. The minimum tumour thickness for a margin of more than 1 cm was 1 mm or more for 67/106 (63%) on the leg and for 57/106 (54%) on the back. We performed laryngotracheal plastic surgery by transplantation of autogenous costal cartilage graft in 9 cases of subglottic stenosis (2 with congenital type and 7 with acquired type). Of 9 cases, 3 cases had the lesion after long-term intubation, 2 cases after tracheotomy and 2 cases after laryngo-tracheo-bronchitis. Five cases aged one year and the remaining 4 cases 2 years, 3 years, 4 years and 6 years old, respectively. We could successfully decannulate in all the 9 cases after our plastic surgery. After a brief introductory presentation of typical regional flaps for surgical repair of cervicofacial defects, the author describes the principle, the advantages, and the disadvantages of myocutaneous island flaps. The central position and the great value of the acromiopectoral myocutaneous island flap (pectoralis major-flap) for reconstructive practice are emphasised. This flap allows the total one-stage repair of the hypopharynx and cervical oesophagus.

Xerophthalmia, or dry eye, is an old human affliction. Among the many causes are trachoma, surgical or traumatic destruction of the lacrimal gland, and paralysis of the greater superficial petrosal nerve after Bell's palsy or from the presence or removal of an intracranial posterior fossa tumor.
