Blackout: Understanding transient vision loss.

BACKGROUND: Transient vision loss may indicate underlying disease of the eye, optic nerve, orbit, brain or heart. Detailed history-taking followed by a complete ocular and neurological examination is therefore a crucial part of any consultation. OBJECTIVE: It is important to determine whether a patient with transient vision loss can be reassured or requires urgent referral for further investigation. This review examines monocular and binocular transient vision loss and provides a structured approach to the examination of a patient with transient vision loss. The aim of this article is to prov

The heterophoria of 3-5 year old children as a function of viewing distance and target type.

PURPOSE: Heterophoria is the misalignment of the eyes in monocular viewing and represents the accuracy of vergence driven by all classical cues except disparity. It is challenging to assess restless children using clinical cover tests, and phoria in early childhood is poorly understood. Here we used eye tracking to assess phoria as a function of viewing distance and target in adults and young children, with comparison to clinical cover tests. METHODS: Purkinje image tracking (MCS PowerRefractor) was used to record eye alignment in adults (19-28 years, N = 24) and typically developing children

[Do prisms according to Hans-Joachim Haase influence ocular prevalence?].

BACKGROUND: Ocular prevalence is defined as an unequal weighting of the eyes in the directional perception of stereo objects. Opinions differ as to the cause and relevance of ocular prevalence. Hans-Joachim Haase suggested that ocular prevalence is due to fixation disparity, brought about by incomplete compensation of heterophoria. He further suggested that prismatic spectacles determined by his "measuring and correcting methodology" (MKH) could restore bicentral fixation and thus establish a perceptual balance between both eyes. METHODS: We examined 10 non-strabismic subjects with a visual a

Orbital tuberculosis presenting as vision loss and headache: early management is paramount.

Orbital tuberculosis is a rare form of extra pulmonary TB and may arise either by hematogenous route or spread directly from the paranasal sinus. We herein report two cases of orbital TB with a vision threatening complication. Case-1 is a 31-year-old female with a headache, a diminution of vision in the right eye, and pain in ocular movement. On examination, there was no proptosis with RAPD present in right eye and tenderness on palpation. CEMRI revealed a diffuse infiltrating lesion at the orbital apex, suggesting of inflammatory pathology. Case-2 is a 40-year-old male with similar complaints

Vision loss in juvenile neuronal ceroid lipofuscinosis (CLN3 disease).

Juvenile neuronal ceroid lipofuscinosis (JNCL; also known as CLN3 disease) is a devastating neurodegenerative lysosomal storage disorder and the most common form of Batten disease. Progressive visual and neurological symptoms lead to mortality in patients by the third decade. Although ceroid-lipofuscinosis, neuronal 3 (CLN3) has been identified as the sole disease gene, the biochemical and cellular bases of JNCL and the functions of CLN3 are yet to be fully understood. As severe ocular pathologies manifest early in disease progression, the retina is an ideal tissue to study in the efforts to u

Construction of Photochromic Ocular Contact Lenses with Superior Antibacterial Activity Derived from Polyoxometalate Nanozymes.

Contact lenses exhibit outstanding practicality in vision correction and visual optimization owing to their inherent advantages, such as unobstructed clear vision, comfortable conformal fitting, aesthetic invisibility, and compatibility with daily activities. However, conventional contact lenses are prone to bacterial colonization, which can cause ocular infections and further induce keratitis. Herein, we designed and fabricated color-changing contact lenses based on polyoxometalate nanozymes for antibacterial and anti-inflammatory ocular therapy. In these contact lenses, polyoxometalate nanoz

Optic disc laceration with combined retinal artery and vein occlusion following penetrating injury.

Ocular trauma is a major cause of unilateral blindness and male adults in the working age group have been found to have higher rates of ocular injury. A case of a work-related penetrating ocular trauma with an intraocular foreign body causing an optic disc laceration and consequently a combined retinal arterial and venous occlusion is presented. The patient did not recover useful vision despite early surgical intervention. This case highlights an unusual sequelae following penetrating ocular trauma as well as the importance of safety eyewear for individuals in high-risk occupations.

Assessment of ocular manifestations in spondyloarthropathies: An observational study.

Spondyloarthropathies (SpA) are systemic inflammatory disorders often presenting with ocular involvement. This observational study conducted at the Department of Ophthalmology, SSMC Rewa, evaluated ocular manifestations among patients diagnosed with various spondyloarthropathies. Ankylosing spondylitis was the most frequent subtype associated with ocular involvement (49%), with uveitis (25%) and dry eye (9%) being the predominant findings. Ocular manifestations were more common in males aged 30-39 years and showed a significant association with longer disease duration. Anterior uveitis emerged

Ocular dominance stability and reading skill: a controversial relationship.

PURPOSE: Evidence is mixed concerning the relationship between stability of ocular dominance and reading deficits. Contrasting results may be due to the use of different tests of dominance, different samples of readers, and different scoring methods. The aim of this study was to investigate the relationship among ocular dominance, general visual abilities, and reading performance, and to evaluate the consistency and reliability of different tests of ocular dominance and the effects of different types of eye dominance scoring. METHODS: In a group of young adults, we measured: (a) main optometr

Axenfeld Rieger Syndrome Presenting with Open Angle Glaucoma in an Adult Patient: A Case Report.

Axenfeld Rieger Syndrome is autosomal dominant genetic condition, which can present with various ocular and non-ocular findings. Anterior segment dysgenesis is the most common ocular finding leading to glaucoma. Non-ocular findings include craniofacial abnormalities, cardiac, dental as well as neurological problems. Mutations in PITX2 and FOXC1 genes have been associated with this condition. FOXC1 mutation causes more ocular findings where as PITX2 mutation has been linked with ocular as well as craniofacial abnormalities. Racial or gender predilection has not been suggested by any literature.

Surgery on the extra-ocular muscles after orbital fracture.

We analyzed the files of 61 patients presenting with diplopia due to orbital fractures. Thirty-four patients needed a primary orbital reconstruction and this diminished the complaints of diplopia in most cases. In only 5 cases additional ocular surgery on the extra-ocular muscles was indicated. Of the 27 patients who did not require orbital surgery, 5 underwent extra-ocular muscle surgery to diminish diplopia. We present our guidelines for treatment, based on forced duction testing, ocular motility, primary position and field of binocular single vision.

The use of multimodal imaging in the evaluation of a patient with central retinal artery occlusion in the setting of asteroid hyalosis: a case report.

BACKGROUND: A central retinal artery occlusion (CRAO) is an ophthalmic emergency due to its strong association with cerebrovascular and cardiovascular morbidity and mortality. A timely diagnosis is necessary but difficult in the setting of dense asteroid hyalosis, as typical fundoscopic findings can be obscured. We present a case where multimodal imaging in an eye with an obscured fundus could lead to timely diagnosis and management of CRAO in a patient with acute vision loss. CASE PRESENTATION: A 94-year-old Caucasian woman with a history of exudative macular degeneration presented to the re

Herpetic ocular infections in solid organ transplantation: A narrative review.

Herpetic ocular infections after solid organ transplantation are uncommon but carry disproportionate risks of morbidity, vision loss, and mortality. The widespread use of modern immunosuppressive regimens and antiviral prophylaxis has reshaped their epidemiology, yet it has not eliminated the risk. Cytomegalovirus retinitis remains the most prototypical and vision-threatening manifestation, increasingly presenting as late-onset disease after cessation of prophylaxis or treatment for rejection. Herpetic ocular syndromes, including acute retinal necrosis caused by herpes simplex and varicella-zo

Pembrolizumab-Induced Choriocapillaritis and Orbital Inflammation: A Case Report.

Immune checkpoint inhibitors (ICIs) such as pembrolizumab have significantly extended patient survival in metastatic melanoma; however, they may rarely induce heterogeneous and potentially vision-threatening ocular immune-related adverse events (irAEs). We report a case of a 52-year-old man with metastatic superficial spreading melanoma who, after two years of pembrolizumab therapy, presented with bilateral ocular redness and photopsias. Visual acuity was preserved in both eyes. Fundus examination showed multifocal pigmentary changes. Fundus autofluorescence (FAF) revealed corresponding hypoau

Ocular Syphilis as a Primary Manifestation of Neurosyphilis: A Case Report.

Sexually transmitted infectious diseases could affect a variety of organs, generating significant symptomatology. In the elderly population, infectious causes for vision problems are not generally considered. We present the case of an elderly patient with blurred vision and darkening of visual fields. He underwent an unsuccessful biopsy of the temporal artery as his vision disturbances presented also with episodic headaches. He was found to have an elevated rapid plasma reagin (RPR) and venereal disease research laboratory (VDRL) test in his cerebrospinal fluid (CSF) analysis. He was treated f
