Space "phobia": a pseudo-agoraphobic syndrome.

Space phobia in 13 patients is described with features distinct from agoraphobia. Its hallmark is fear of absent visuospatial support (open spaces) and of falling, unlike the fear of public places found in agoraphobia. Other distinctions are the much later mean age of onset (55 years compared with 24 years), rarity of depression or free floating anxiety, limited response to treatment by exposure in vivo, and the frequent presence of diverse neurological and cardiovascular disorders which can progress until the patient is confined to a wheelchair. Space phobia may indicate disturbed integration…

Heart rate after trauma and the specificity of fear circuitry disorders.

BACKGROUND: Fear circuitry disorders purportedly include post-traumatic stress disorder (PTSD), panic disorder, agoraphobia, social phobia and specific phobia. It is proposed that these disorders represent a cluster of anxiety disorders triggered by stressful events and lead to fear conditioning. Elevated heart rate (HR) at the time of an aversive event may reflect strength of the unconditioned response, which may contribute to fear circuitry disorders. METHOD: This prospective cohort study assessed HR within 48 h of hospital admission in 602 traumatically injured patients, who were assessed …

Cognitive behavioral therapy for panic disorder and comorbidity: more of the same or less of more?

This study compared the effects of a higher dose of cognitive behavioral therapy (CBT) for panic disorder versus CBT for panic disorder combined with "straying" to CBT for comorbid disorders in individuals with a principal diagnosis of panic disorder with or without agoraphobia. Sixty-five participants were randomly assigned to one of two treatment conditions, either CBT focused solely upon panic disorder and agoraphobia or CBT that simultaneously addressed panic disorder and agoraphobia and, to a lesser degree, the most severe comorbid condition. Results indicated a significant reduction in p…

Expectations and attributions in social anxiety disorder: diagnostic distinctions and relationship to general anxiety and depression.

Contemporary cognitive models suggest that social anxiety disorder arises from a number of cognitive factors, including tendencies to form pessimistic (rather than optimistic) attributions and expectations for socially-related events. These models also assume that the strengths of such attributions and expectations are more closely linked with social anxiety than with general anxiety or depression. To test these assumptions, a battery of self-report measures was completed by participants with a primary diagnosis of generalized social anxiety disorder (n = 75), panic disorder with agoraphobia (…

An examination of syndromal validity and diagnostic subtypes in social phobia and panic disorder.

BACKGROUND: We investigated whether patients with DSM-III-R panic disorder and patients with social phobia could be distinguished on the basis of selected demographic variables and by several commonly used anxiety and phobia rating scales. METHOD: Sixty-six patients with social phobia and 60 patients with panic disorder (42 with and 18 without agoraphobia) were studied. Subjects completed a battery of self-report measures that assessed phobic fears, avoidance, and related problems. RESULTS: Social phobic patients showed an earlier age at onset than the panic disorder group, and there was a t…

Trends in psychopathology across the adolescent years: what changes when children become adolescents, and when adolescents become adults?

BACKGROUND: Little is known about changes in the prevalence of psychiatric disorders between childhood and adolescence, and adolescence and adulthood. METHODS: We reviewed papers reporting prevalence rates of psychiatric disorders separately for childhood, adolescence, and early adulthood. Both longitudinal and cross-sectional papers published in the past 15 years were included. RESULTS: About one adolescent in five has a psychiatric disorder. From childhood to adolescence there is an increase in rates of depression, panic disorder, agoraphobia, and substance use disorders (SUD), and a decre…

Reliving and disorganization in posttraumatic stress disorder and panic disorder memories.

Intense, disorganized recollections are one of the core symptoms of posttraumatic stress disorder (PTSD), and considered to be the result of inadequate processing of trauma information. A first panic attack resembles trauma in being an unexpected frightening and subjectively life-threatening event, and like PTSD, panic disorder with agoraphobia also involves fear conditioning after the first event. Therefore, a panic attack may be processed similarly to a trauma, and as a result, memories of a panic attack may share characteristics like reliving and disorganization with PTSD trauma memories. T…

Pathological worry, anxiety disorders and the impact of co-occurrence with depressive and other anxiety disorders.

The Penn State Worry Questionnaire (PSWQ) was administered to 123 outpatients with principal diagnoses of generalized anxiety disorder (GAD), social anxiety disorder (SAD), panic disorder with agoraphobia, and panic disorder without agoraphobia (PD) to examine the specificity of pathological worry for GAD. The mean PSWQ scores in patients with GAD and SAD were significantly higher than the mean PSWQ scores in patients with PD, while not differing significantly in the subgroups without any co-occurring depressive or anxiety disorders. Patients with any co-occurring depressive or anxiety disorde…

Psychiatric disorders in asthmatic outpatients.

It has been reported that the lifetime prevalence of panic disorder in patients with pulmonary disease is higher than epidemiologic estimates of population prevalence. We evaluated the frequency of anxiety disorders in 86 subjects from the Outpatient Asthma Clinic. Psychiatric diagnoses were assessed with the Mini-International Neuropsychiatric Interview 4.4 Version (MINI). Forty-five asthmatic patients (52.3%) reported at least one current anxiety disorder. The frequency of panic disorder with or without agoraphobia was 13.9% (n=12) and that of agoraphobia without panic disorder was 26.8% (n=…

An exploration of the relationship between expression of hostility and the anxiety disorders.

The purpose of the present study was to investigate differences between four anxiety disorder groups with respect to their levels of hostility. Patients with diagnoses of panic disorder, agoraphobia with panic, generalized anxiety disorder and social phobia were compared as regards their performance on the Hostility and Direction of Hostility Questionnaire (HDHQ). Results showed that while the groups did not differ on their extrapunitiveness, there were significant differences on intropunitive scores, with social phobics showing the most self criticism and guilt, followed in order by the agora…

Do panic-agoraphobics overestimate distances?

Twenty-five subjects suffering from Panic Disorder with agoraphobia (PDA) and 25 normal controls (NC) were asked to estimate distances of familiar places of their town and non-distance stimuli. PDA subjects also completed the Mobility Inventory for Agoraphobia (MIA). Subjects with PDA significantly overestimated distances as compared to NC, whereas no significant difference was found on estimates of non-distance stimuli. Within the PDA group, significant correlations were found between distance estimates and subsets of MIA concerning the ways of moving through the environment. Although with ca…

Perception of racial discrimination and psychopathology across three U.S. ethnic minority groups.

To examine the association between the perception of racial discrimination and the lifetime prevalence rates of psychological disorders in the three most common ethnic minorities in the United States, we analyzed data from a sample consisting of 793 Asian Americans, 951 Hispanic Americans, and 2,795 African Americans who received the Composite International Diagnostic Interview through the Collaborative Psychiatric Epidemiology Studies. The perception of racial discrimination was associated with the endorsement of major depressive disorder, panic disorder with agoraphobia, agoraphobia without …

A stimulus portraying a mother figure who is leaving a room where a baby lies alone on the floor was presented 22 times at increasing exposure durations in a single-view tachistoscope.
