Acute decreases in cerebrospinal fluid glutathione levels after intracerebroventricular morphine for cancer pain.

UNLABELLED: Intracerebroventricular (ICV) morphine administration is effective for the management of refractory cancer pain. Recent preclinical observations of acute depletion of the major endogenous intracellular antioxidant glutathione (GSH) in brain and peripheral organs after ICV morphine in rodents led us to apply microchemical methods to profile the neurochemical effects of ICV morphine in three patients treated for intractable cancer pain. Assessment of morphine, morphine-6-glucuronide, and a panel of endogenous compounds and metabolites in ventricular and cisternal cerebrospinal fluid …

When all else fails: stepwise multiple solutions for a complex cancer pain syndrome.

In the presence of insufficient pain relief and substantial adverse effects, application of alternative routes of administration and a change of opioid are the main methods used to improve the analgesic response. When all else fails, the result may further be optimized using adjuvant drugs by an alternative route, namely the intrathecal route. A stepwise approach with multiple drugs and routes of administration is described. This was implemented to resolve a complex pain syndrome otherwise considered uncontrollable. We found it possible to manage a very difficult pain situation by intrathecal

Mindfulness-based Interventions for Pain and Symptom Management in Adolescent Cancer: Systematic Review.

BACKGROUND: This systematic review aimed to examine how mindfulness approaches affect pain, symptom cluster severity, and quality of life among adolescents receiving cancer chemotherapy. Contrasting with prior syntheses emphasizing adult populations or isolated symptoms, this investigation specifically targets symptom clusters in adolescent cancer patients (aged 10-19 years), a unique population requiring specialized nursing care approaches given their distinct developmental characteristics. METHODS: A comprehensive literature evaluation was conducted across Scopus, Cochrane Embase, PubMed, C

Implanted intrathecal pump catheter migration rates in the management of cancer-related pain.

INTRODUCTION: Intrathecal drug delivery systems offer a potent modality for managing cancer-related pain that is resistant to conservative measures. However, catheter-related complications, particularly migration, remain a concern. This study assessed the prevalence, risk factors, and implications of catheter migration rates in patients with intrathecal pumps (ITP) for cancer-related pain. MATERIALS AND METHODS: A retrospective database and chart review of 441 patients with cancer with an ITP at the Huntsman Cancer Institute from May 2014 to December 2023 was performed to identify patients wh

Different titration protocols in pain management after radical gastrectomy for gastric cancer patients.

BACKGROUND: Effective pain management after radical gastrectomy is crucial for patient recovery. With the promotion of enhanced recovery after surgery protocols, postoperative pain management has become a core component of perioperative care. However, controversy remains regarding the optimal frequency of analgesic titration for pain control. AIM: To compare the efficacy of 12-hour vs 24-hour titration regimens in postoperative pain management following radical gastrectomy for gastric cancer. METHODS: This retrospective comparative study analyzed data from 120 patients who underwent radical

Na+/K+-ATPase Modulates Purinergic P2X3 Receptor Function to Drive Bone Cancer Pain.

Bone cancer pain (BCP) is one of the most common types of chronic pain in cancer patients, with a prevalence of up to 75%. However, the pathological mechanism and therapeutic approaches are limited. Here, we demonstrated that Na+/K+-ATPase α1 (NKAα1) is a critical regulator of nociception through interaction with purinergic P2X3 receptor (P2X3R) in the dorsal root ganglion (DRG). Conditional knockout of NKAα1 in transient receptor potential vanilloid 1-positive (TRPV1+) neurons led to an increase in P2X3R-dependent Ca2+ influx and neuronal hyperexcitability and also promoted pain hypersensitiv…

Splanchnic neurolysis for severe cancer pain caused by abdominal paraaortic lymph node metastasis.

PURPOSE: Noxious stimuli caused by enlarged metastatic para-abdominal aortic lymph nodes (PAN) enter the spinal cord via the sympathetic nervous system, and the pain is sometimes resistant to conventional pharmacotherapy. The aim of this retrospective clinical investigation was to verify the efficacy and safety of splanchnic nerve neurolysis (SNN) for intractable abdominal and/or back pain caused by enlarged PAN. METHODS: Among cancer pain patients referred to the Palliative Care Team in our hospital between April 2016 and November 2023, those who received SNN for pain relief, with abdominal …

Managing chronic pain after breast cancer treatments: are web-based interventions the future?

PURPOSE OF THE REVIEW: Chronic post-treatment pain in breast cancer affects a high proportion of patients. Symptom burden and financial costs are increasingly impacting patients and healthcare systems because of improved treatments and survival rates. Supporting long-term breast cancer symptoms using novel methodology has been examined, yet few have explored the opportunity to utilise these interventions for prevention. This review aims to explore the need for, range of, and effectiveness of such interventions. RECENT FINDINGS: Three papers describe risk factors for chronic pain, with six rec

FTY720/Fingolimod mitigates paclitaxel-induced Sparcl1-driven neuropathic pain and breast cancer progression.

Paclitaxel is among the most active chemotherapy drugs for the aggressive triple negative breast cancer (TNBC). Unfortunately, it often induces painful peripheral neuropathy (CIPN), a major debilitating side effect. Here we demonstrate that in naive and breast tumor-bearing immunocompetent mice, a clinically relevant dose of FTY720/Fingolimod that targets sphingosine-1-phosphate receptor 1 (S1PR1), alleviated paclitaxel-induced neuropathic pain. FTY720 also significantly attenuated paclitaxel-stimulated glial fibrillary acidic protein (GFAP), a marker for activated astrocytes, and expression o

High-dose opioids for chronic non-cancer pain: an overview of Cochrane Reviews.

BACKGROUND: This overview was originally published in 2017, and is being updated in 2022.  Chronic pain is typically described as pain on most days for at least three months. Chronic non-cancer pain (CNCP) is any chronic pain that is not due to a malignancy. Chronic non-cancer pain in adults is a common and complex clinical issue, for which opioids are prescribed by some physicians for pain management. There are concerns that the use of high doses of opioids for CNCP lacks evidence of effectiveness, and may increase the risk of adverse events. OBJECTIVES: To describe the evidence from Cochran…

Opioid Prescription Denials by Community Pharmacies for Cancer-Related Pain: A Case Series.

Pain is one of the most common symptoms experienced by patients living with cancer. Guidelines recommend opioids as the mainstay in the management of cancer-related pain. However, the opioid epidemic has resulted in policymakers recommending limitations on opioid prescribing which led to community pharmacies implementing various parameters. These restrictions have created barriers for patients with cancer-related pain attempting to fill opioid prescriptions from their community pharmacies. Additionally, in the setting of the opioid epidemic, there have been reports of systemic bias within comm

Addiction stigma and opioid use in chronic non-cancer pain management in Nigeria.

This study uses 26 in-depth interviews conducted with people who use drugs (PWUD) who had sought care for chronic non-cancer pain in public health facilities in Nigeria, to explore how drug consumption stigma constitutes patient legitimacy based on neoliberal ideals. It found drug consumption stigma to be salient and pervasive in PWUD health-care encounters, operating through interpersonal interactions and institutionalised policies and practices to shape access to care. Crucially, stigma emerged through disciplinary opioid prescribing and dispensing practices that defined, categorised and mar…

Ion channels in cancer-induced bone pain: from molecular mechanisms to clinical applications.

Cancer-induced bone pain (CIBP) caused by bone metastasis is one of the most prevalent diseases, and current treatments rely primarily on opioids, which have significant side effects. However, recent developments in pharmaceutical science have identified several new mechanisms for CIBP, including the targeted modification of certain ion channels and receptors. Ion channels are transmembrane proteins, which are situated on biological cell membranes, which facilitate passive transport of inorganic ions across membranes. They are involved in various physiological processes, including transmission

Tapia Syndrome and Severe Pain Induced by Occipital Bone Metastasis of Prostate Cancer.

Tapia syndrome is characterized by unilateral tongue paralysis, hoarseness, and dysphagia. It is often associated with issues in the lower cranial nerves and is rarely caused by malignant tumors. A 71-year-old Japanese male with prostate cancer and bone metastases experienced severe headaches, oral discomfort, dysphagia, and hoarseness for a month. Neurological examination revealed left-sided tongue atrophy and left vocal cord paralysis, suggesting problems with cranial nerves IX and XII. CT scans showed bone metastasis in the left occipital bone. Brain MRI showed no brain or meningeal metasta
