Epilepsy is a chronic disorder in which onset occurs primarily during childhood and adolescence. Traditionally, treatment of childhood epilepsy emphasizes neurologic aspects over psychological factors. The attention to the seizure control in the clinical setting will not address the full range of quality of life problems of childhood epilepsy. Many of the psychosocial difficulties that plague adults with epilepsy develop from the complications associated with this early onset. Children with epilepsy have a high prevalence of behavior problems and learning problems. Assessing quality of life in…

We report the case of a 49 year-old man who presented a partial status epilepticus with left-sided clonic seizures after the occurrence of a … atrophy observed one year later were significantly increased. Cerebral hemiatrophy associated with epilepsy in adulthood is exceptional. Our case suggests that the occurrence of partial status epilepticus, even during adulthood, may aggravate cerebral hemiatrophy formally silent.

Neurocysticercosis (NC) remains a major public health problem in developing and some developed countries. Currently, the best procedures for … uent clinical manifestations of NC are seizures. We assume that NC is the main cause of symptomatic epilepsy in developing countries; however, no case-control or cohort studies demonstrate this association. Most patients with NC with seizures have a good prognosis; nevertheless, further studies analyzing factors related to recurrence of seizures and possib …

Some female gonadal and adrenal steroid hormones and their related pituitary peptides have neuroactive effects that can influence seizures. These effects may play a significant role in the pathophysiology of epilepsy, the pattern of seizure occurrence, therapeutic interventions using naturally occurring hormones, and the development of hormone-based neuroactive synthetic analogues with potent antiepileptic properties.

Sleepiness, a common complaint of epilepsy patients, is frequently attributed to antiepileptic medications. To determine predictors of subjective sleepiness in epilepsy patients, we gave self-administered, validated surveys of sleepiness [Epworth sleepiness scale (our major outcome measure)] and sleep apnea [sleep apnea scale of the sleep disorders questionnaire (SA/SDQ)] to 158 epilepsy patients and 68 neurology patients without epilepsy (controls). An elevated Epworth score (>10) was more likely in epilepsy patients compared to controls after controlling for age and gender (p < 0.05). When E…

Mendelian forms of benign myoclonic epilepsies where a chromosomal locus has been defined include (1) the autosomal dominant (AD) juvenile myoclonic epilepsy (JME) in chr. 6p11, (2) the autosomal dominant childhood absence epilepsy which evolves to JME in chr. 1p, (3) familial adult myoclonic epilepsy of Yasuda and Inazuki, and (4) possibly JME within the idiopathic generalized epilepsy susceptibility gene in chr. 8 reported by Zara et al (1995). Other myoclonic epilepsy syndromes with onset in the first year of life (Aicardi's Neonatal (Early) Myoclonic Encephalopathy, West's Syndrome, Dravet…

Functional brain imaging with positron emission tomography (PET) has led to significant advances in the diagnosis and management of various forms of childhood epilepsy. Interictal PET in temporal lobe epilepsy typically shows decreased glucose utilization (hypometabolism) in one or both temporal lobes. Children with early-onset refractory extratemporal lobe epilepsy who do not manifest structural abnormalities on magnetic resonance imaging often show single or multiple cortical foci of hypometabolism indicating underlying microdysgenesis that corresponds to the area of epileptogenicity. The fi…

Based on the data of a comprehensive examination of 40 patients with Kozhevnikov's epilepsy, 24 with hyperkinesis epilepsy (Sokolianskiĭ-Kliuchikov syndrome), 10 with myoclonus epilepsy (Unverricht-Lundborg syndrome) and 6 with cerebellar myoclonic dyssynergy, the most informative differential diagnostic criteria were distinguished. Their use will make it possible to determine one or another above-indicated disease entity more reliably. The structure of the paroxysmal syndrome and the character of myoclonic hyperkinesis are determined by dissimilar involvement into the pathological process of …

The pathogenesis oriented diagnoses and therapies of epilepsy and myoclonus was tended to describe, but their pathogeneses were partially elucidated. The pathogenesis of both diseases was firstly demonstrated to have genuine or hereditary factors to be discharged abnormal electrical potentials which were provoked varieties of symptoms of both epilepsy and myoclonus. The careful observation of clinical symptoms, the introduction of monitoring EEG, computed tomogram, spike voltage topography, magnetoencephalography, somatosensory evoked potential (SEP) and MRI were comparatively easily provided …

Complex partial epilepsy arising in the temporal lobe has been associated with several types of pathologic lesions including Ammon's horn sclerosis, malformations, neoplasms, and inflammatory scars from infarcts or infection. These lesions are usually situated at various sites in the medial temporal lobe, so that one of the enigmas of attempting to understand the pathogenesis of TLE pertains to the clinical manifestation of a single epileptic disorder which is associated with dissimilar lesions at dissimilar sites. Recent demonstrations of an alteration in temporal lobe anatomy, i.e. malformat…

Until relatively recently, genetic influences in partial seizures were thought to be of minimal importance. However, with further identification of childhood benign partial seizures it is becoming clear that inheritance plays a major role in the pathogenesis of these seizures. Diagnostic criteria proposed for benign partial seizures include absence of neurologic or intellectual deficits, family history of epilepsy, onset of seizures after age 2 years, stereotyped brief seizures, frequent nocturnal occurrence, spontaneous remission in adolescence, and electroencephalograms (EEGs) demonstrating …

Fifty-six consecutive epileptic patients with partial seizures (30 temporal, 26 extratemporal) and facial asymmetry were studied. Facial asymmetry was compared with EEG, radiologic, and other clinical findings. Thirty patients had a lesional epilepsy whereas 26 were considered cryptogenic. In lesional epilepsies, 60% of patients had EEG foci ipsilateral to the smaller hemiface and only 20% had EEG foci contralaterally. In the cryptogenic group, the EEG focus was ipsilateral in 50% and contralateral to the facial smallness in 46%. No differences were noted between temporal and extratemporal epi…

Pregnant women with epilepsy are at increased risk of seizures and complications. An increase in seizure frequency is seen in 25-30% of pregnant women with epilepsy; the offspring of mothers who experienced seizures during pregnancy are at a 2.5 times higher risk for seizures later in life. One of the main reasons for the increase in seizures during pregnancy is a decline in plasma concentrations of antiepileptic drug (AED) that occurs as pregnancy progresses, largely as a result of marked alterations in plasma protein binding. It is well known that epilepsy represents a risk for a variety of …

The coincidence of recent advances in human genetics and the development of an international classification system in the field of epilepsy, mostly for practical reasons, has resulted in unexpected progress in syndrome-related epilepsy genetics. This review of the International Classification of Epileptic Seizures as well as of Epilepsies and Epilepsy Syndromes, in addition to presenting up-to-date clinical views on epilepsy classification, tries to draw special attention to problematic areas of the classification and its underlying concepts, particularly to those to which genetic studies coul…

Antiepileptic drug selection is based on efficacy for specific seizure types and epileptic syndromes. For idiopathic generalized epilepsies … f these five major antiepileptic drugs may be necessary for inadequately controlled patients. Other epilepsy syndromes such as neonatal and infantile epilepsies, febrile epilepsy, and alcoholic epilepsy require specific drug treatment. For all these seizure types and epilepsy syndromes, treatment ultimately must be selected to provide maximal efficacy and …

The research literature is consistent in reporting somewhat less familial aggregation in partial than in generalized epilepsy. However, relatives of patients with partial seizures do appear to have higher seizure risks than relatives of controls, suggesting that genetic factors are important in at least some partial epilepsies. Complex partial epilepsy appears to be only slightly less familial than other types of epilepsy. Relatives of patients with focal EEG abnormalities generally have been found to have lower risk of both EEG abnormalities and epilepsy than relatives of patients with genera…

1. Out of 295 outpatients who were followed up by our staff for longer than 4 years, a total of 196 patients was subjected to this retrospective study. The inclusion criterion was that they had inevitably been placed on polytherapy of two or more anti-epileptic drugs (AEDs). In other words, they had failed to reach monotherapy for some reasons. 2. One of the determinant factors for the success or failure of monotherapy was the type of epilepsies. Namely, primary generalized epilepsy (PGE) was relatively easy to attain by monotherapy regardless of the seizure type, whereas secondary generalized…
