Evaluation of the emergency department as a site for hypertension screening.

To see if the emergency department is an appropriate site for hypertension screening, the coverage, appropriateness of the target population and the adequacy of follow-up procedures were evaluated. The study was conducted in the Adult Emergency Department of The Johns Hopkins Hospital, which 61% of the community regard as their regular source of care. All patients had their blood pressure read in the emergency department. A reading of 95 diastolic or greater was used as the definition of hypertension. Data from the 1970 census and The Johns Hopkins outpatient department billing system were use

Possible association between the microbiota in subgingival and atherosclerotic plaque in a cohort of patients with carotid stenosis.

BACKGROUND: Periodontal pathogens have been implicated in systemic diseases, including atherosclerosis. This study investigates the relationship between subgingival and microbial colonization of carotid plaque in patients undergoing carotid endarterectomy (DRKS00021598). METHODS: Microbial samples from both subgingival and carotid plaques were collected from 25 hospitalized patients. Preoperative all patients underwent periodontal examination. The microbial composition was assessed using metagenomics sequencing of 16S rRNA gene (V3/V4 regions). RESULTS: Bacterial DNA was detected in both sub

Primary care for the urban poor in India during the pandemic: Uninterrupted management of non-communicable diseases and home-based care of patients with COVID-19 infection.

PROBLEM: The two waves of COVID-19 severely affected the healthcare system in India. The government responded to the first wave with a strict nationwide lockdown which disrupted primary care, including the management of non-communicable diseases (NCDs). The second wave overwhelmed healthcare facilities leading to inadequate access to hospital services. Collectively, these issues required urgent responses, including the adaptation of primary care. APPROACH: The Low-Cost Effective Care Unit (LCECU) of Christian Medical College, Vellore (CMC) has a network of community volunteers, community heal

What Matters Most: A Qualitative Study Exploring Common Values Identified by Hospitalized Older Adults.

BackgroundThe Age-Friendly Health System (AFHS) promotes better care by providing evidence-based guidance known as the "4 Ms"-Matters Most, Medication, Mentation, and Mobility. Using the "Matters Most" framework to assess older patients' values and preferences during hospitalization helps care teams individualize care approaches and align patients' healthcare needs and priorities. Having a better understanding of patients' values and preferences during hospitalization is important for improving care, outcomes, and patient satisfaction.ObjectiveIdentify and describe core themes related to "What

Travel-time accessibility and adaptive spatial planning solutions for the healthcare system.

Ensuring equitable healthcare access remains a significant challenge, particularly in rural areas where aging populations face increasing barriers due to facility closures. This study employs a GIS-based spatial network model to assess hospital accessibility, integrate road characteristics, and estimate travel times across all officially registered population nodes in a region. Analyzing disparities by age composition, we identify high-risk exclusion areas and propose an optimized model prioritizing vulnerable populations. Through four optimization scenarios, we evaluate the impact of hospital

Safety-net Hospitals Face More Barriers Yet Use Fewer Strategies to Reduce Readmissions.

OBJECTIVE: US hospitals that care for vulnerable populations, "safety-net hospitals" (SNHs), are more likely to incur penalties under the Hospital Readmissions Reduction Program, which penalizes hospitals with higher-than-expected readmissions. Understanding whether SNHs face unique barriers to reducing readmissions or whether they underuse readmission-prevention strategies is important. DESIGN: We surveyed leadership at 1600 US acute care hospitals, of whom 980 participated, between June 2013 and January 2014. Responses on 28 questions on readmission-related barriers and strategies were comp

Accidents and trauma care--1983.

Except for categoric grants, the federal government has divested itself of financing emergency medical services. Now the leadership for regional planning of emergency medical services must come from the state, usually from the health department. If we are to obtain improved hospital care of trauma patients, it is critical that we separate hospitals that have made a genuine commitment to the care of the multiply injured patient from hospitals that have not in order to avoid the tragedy of patients being delivered to hospitals that have inadequate resources or commitment or organization to meet

Incidence, clinical and genomic trends of hospital- and Non-hospital-onset KPC-producing Klebsiella pneumoniae infections before and during the COVID-19 era: a ten-year interrupted time series study.

BACKGROUND: Infections caused by KPC-producing Klebsiella pneumoniae (KPC-KP) represent a persistent public health challenge. This prospective study examines ten-year trends, clinical features, and genomic epidemiology of hospital-onset (HOI) and non-hospital-onset (non-HOI, including healthcare-associated [HcAI] and community-acquired [CA]) KPC-KP infections following a 2012 outbreak. We evaluated the impacts of a 2014 antimicrobial stewardship program (ASP) and COVID-19-related infection prevention and control (IPC) measures, with emphasis on hospital-to-community dissemination. METHODS: We

Antibiotic susceptibility of pathogens isolated in respiratory tract samples of recently hospitalized patients.

Community-acquired lower respiratory tract infections (CA-LRTIs) treatment is largely empirical as microbiologic testing is rarely performed. Here, we provide microbiologic data of severe CA-LRTI cases requiring hospitalization. We aim to describe the distribution and susceptibility patterns of pathogens causing severe CA-LRTI. We analyzed respiratory samples from recently admitted patients with CA-LRTIs to assess pathogen distribution and antibiotic susceptibility patterns. We divided patients into three groups: CA-LRTI and no prior healthcare exposure, CA-LRTI with healthcare exposure, and p

The effects of observed sudden infant death syndrome (SIDS) on hospital staff.

We have described an observed case of SIDS in a hospital setting. The victim was "well baby" hospitalized for psychosocial reasons. Death occurred on the fifth hospital day, during sleep and two hours after feeding. The death was silent and was preceded by cyanosis and respiratory arrest. Hospital staff who attended the baby during his admission experienced the same traumatic reactions as families of SIDS victims, ie, shock, disbelief, anger, guilt, fear, blaming, sadness, and behavioral manifestations. Information on SIDS, communication about feelings and continued group support were of utmos

Intrahepatic biliary stones: imaging features and a possible relationship with ascaris lumbricoides.

Intrahepatic (IH) biliary stones are common in East Asia as part of a disease known as Oriental cholangiohepatitis (OCH). At a hospital serving non-Oriental communities, 40 patients were diagnosed on ultrasound (US) during an 8-year period as having IH stones. Follow-up showed that the diagnosis was false in three cases. In the 37 patients with IH stones, 33 conventional retrograde cholangiograms were done; 26 underestimated the IH abnormalities or missed them entirely. Computed tomography (CT) was done in 15 of these 37 patients; the attenuation of the stones was found to be only slightly abo

Identifying priorities for primary care investment in Ireland through a population-based analysis of avoidable hospital admissions for ambulatory care sensitive conditions (ACSC).

BACKGROUND: In 2016, the Irish acute hospital system operated well above internationally recommended occupancy targets. Investment in primary care can prevent hospital admissions of ambulatory care sensitive conditions (ACSCs). OBJECTIVE: To measure the impact of ACSCs on acute hospital capacity in the Irish public system and identify specific care areas for enhanced primary care provision. DESIGN: National Hospital In-patient Enquiry System data were used to calculate 2011-2016 standardised bed day rates for selected ACSC conditions. A prioritisation exercise was undertaken to identify the
