Atrial fibrillation (AF) is the most common arrhythmia encountered in clinical practice. The prevalence and incidence of AF are rising, as confirmed in several European and American registries. Guidelines published in 2008 from the European Society of Cardiology/American Heart Association and from the American College of Chest Physicians, clarified the strategy of antithrombotic treatment in AF, which is based on the presence of risk factors for thromboembolism. This approach allows physicians to classify patients as at low, moderate or high risk, according to their individual risk characteris

BACKGROUND: Electrical isolation of the left atrial appendage (LAA) during catheter ablation may increase the risk of thrombo-embolism, even in patients with low stroke risk scores and sinus rhythm. Late incidental isolation of the LAA has been rarely reported as a cause of transient ischaemic attack (TIA).
CASE SUMMARY: A 73-year-old woman with a CHADS2 score of 0 underwent three catheter ablation sessions for long-standing persistent atrial fibrillation. Incomplete anterior mitral line ablation during the first and second sessions led to recurrent mitral flutter. At the third session, mitra

Surgical atrial ablation is evaluated by surgeons in relation to the estimated surgical risk. We analyze whether high-risk patients (HRPs) e … re increased in HRPs. LRPs underwent stand-alone ablations as well. Ablation energy did not differ. Left atrial appendage closure was performed in up to 86.1% (mainly cut-and-sew procedures). Mortality corresponded to the original risk class without an escalation that may be related to ablation, stroke rate, or myocardial infarction. A total of 60.6% of HRPs versus 75.1% of LR …

Objective: Left atrial appendage (LAA) occlusion at the time of cardiac surgery in patients with atrial fibrillation has been shown to reduce the incidence of postoperative embolic stroke. However, the optimal method for LAA occlusion is not universally accepted. We sought to examine the safety and effectiveness of LAA occlusion with the AtriClip epicardial occlusion device. Methods: Cardiac surgical patients with atrial fibrillation who underwent LAA AtriClip placement were evaluated prospectively. Clip placement and clinical outcomes were examined after 1 year of follow-up with transesophage

BACKGROUND: The atrial septal pouch (SP) is a new anatomical entity within the interatrial septum. The left-sided SP may be the source of thrombus and contribute to ischemic stroke. The aim of this study was to provide a detailed morphometric description of the SP.
METHODS: Two hundred autopsied hearts (23% deriving from females) with a mean age of 46.7±19.1years were investigated. We assessed the morphology of the interatrial septum. We obtained measurements and casts of the SPs, and we conducted histological staining of the left-sided SPs.
RESULTS: Patent foramen ovale was observed in 25%

IMPORTANCE: Accurately predicting major bleeding events in non-valvular atrial fibrillation (AF) patients on direct oral anticoagulants (DOACs) is crucial for personalized treatment and improving patient outcomes, especially with emerging alternatives like left atrial appendage closure devices. The left atrial appendage closure devices reduce stroke risk comparably but with significantly fewer non-procedural bleeding events.
OBJECTIVE: To evaluate the performance of machine learning (ML) risk models in predicting clinically significant bleeding events requiring hospitalization and hemorrhagic…

BACKGROUND: Patients with severe mitral regurgitation (MR), atrial fibrillation (AF), and contraindications to long-term anticoagulation often require multiple interventions. The feasibility of combining mitral transcatheter edge-to-edge repair, left atrial appendage closure (LAAC), and AF ablation in a single session is unknown.
CASE SUMMARY: A 78-year-old woman with dilated cardiomyopathy, severe functional MR, and high-burden paroxysmal AF underwent a single-session procedure including pulsed-field AF ablation, mitral transcatheter edge-to-edge repair, and LAAC. Pulmonary veins were isolat…

Pseudoaneurysms at the coronary artery bypass graft (CABG) anastomosis site are rare. Here, we report a rare case of spontaneous resolution of a pseudoaneurysm at the anastomosis site developed soon after CABG. A 66-year-old man was referred to our hospital because of chest pain with paroxysmal atrial fibrillation. Pulmonary vein isolation (PVI), atrial appendage closure, and CABG were performed under cardiopulmonary bypass. Contrast computed tomography (CT) on postoperative day (POD) 5 revealed a pseudoaneurysm at the anastomosis of the left internal mammary artery (LIMA) to the circumflex ar

PURPOSE OF REVIEW: With an increasing number of interventional procedures performed for structural heart disease and cardiac arrhythmias each year, echocardiographic guidance is necessary for safe and efficient results. The purpose of this review article is to overview the principles of intracardiac echocardiography (ICE) and describes the peri-interventional role of ICE in a variety of structural heart disease and electrophysiological interventions.
RECENT FINDINGS: Both transthoracic (TTE) and transesophageal echocardiography have limitations. ICE provides the advantage of imaging from with

We report our experience with trans-catheter closure of ASD without fluoroscopy and introduce a well-established technique for alternative use in children.
METHODS AND RESULTS: From November 2013 to May 2015, a total of 114 children with isolated secundum ASD (diameter 6 mm to 24 mm, median 13.8 mm) underwent attempted transcatheter device closure under the guidance of transoesophageal echocardiography without fluorosc

The diagnosis of superior vena cava (SVC) syndrome is always challenging, even more so when it happens intraoperatively. An usual transgastric-transesophageal projection allows for the visualization and quantification of caval flow acceleration, critical in leading to rapid diagnosis and quick reaction in the operative setting. We present the case of a patient who underwent successful mitral valve reconstruction and left atrial appendage closure via median sternotomy on cardiopulmonary bypass and, intraoperatively, developed iatrogenic SVC syndrome with hypotension, facial cyanosis, and conjun…

Structural heart disease interventions are the transcatheter techniques used for treating non-coronary heart disease. In recent years, these techniques have generated considerable interest even though they still comprise only a small percentage of the total volume of interventions performed in interventional cardiology departments. The level of interest in these techniques is high probably because their application is characterized by a number of special features: a) the need for multidisciplinary teams; b) the need for specialized education and training; c) the requirement for special skills
