Osseous changes following reverse total shoulder arthroplasty combined with latissimus dorsi transfer: a case series.

BACKGROUND: This is the first report on the incidence of proximal humerus osseous changes and associated clinical consequences in reverse total shoulder arthroplasty combined with a latissimus dorsi tendon transfer (RTSA+LDT). METHODS: A multicenter, retrospective review identified all patients who had undergone a primary RTSA+LDT and had at least 3-month radiographic follow-up between 2012 and 2017. Data collection included demographics, oral steroid use, repair technique for LDT fixation, radiographic humeral osseous changes, complications, and need for revision surgery. RESULTS: Twenty-fo

Improving arm function by prosthetic limb replacement in a patient with severe arthrogryposis multiplex congenita.

OBJECTIVE: In patients with severe bilateral congenital arm deficiencies, even simple activities of daily living, such as feeding, may be major challenges. We report here a case of a patient with arthrogryposis multiplex congenita affecting all 4 extremities, who underwent prosthetic replacement after elective transhumeral amputation of his right functionless arm. CASE REPORT: A 22-year-old man with arthrogryposis multiplex congenita had severe deficits of his upper limbs. Previous surgeries for upper and lower limbs had enabled this patient to walk without aids; however, his upper limbs rema

Incidence, predictive factors, and prognosis for winged scapula in breast cancer patients after axillary dissection.

PURPOSE: Axillary lymph node dissection is part of breast cancer surgery, and winged scapula is a possible sequela. Data regarding its incidence, predictive factors, and patient prognosis remains inconsistent. Ignorance of its diagnosis may lead to undertreatment with physical morbidity. METHODS: Breast cancer patients with axillary lymph node dissection were prospectively recruited. Postoperative examinations by the physiotherapy staff were performed. RESULTS: One hundred eighty-seven patients were recruited during July-October 2012; 51 patients had a positive diagnosis (27.2 %), with 38 pa

Complication rates following total shoulder arthroplasty for osteoarthritis versus proximal humerus fracture: a propensity-matched cohort comparison of 9,190 patients.

BACKGROUND: Total shoulder arthroplasty (TSA) has seen a dramatic rise in utilization, particularly in the elderly population. While indications have expanded beyond glenohumeral osteoarthritis (OA) to include proximal humerus fractures (PHFs), robust evidence comparing outcomes between these groups remains limited. We sought to compare the rate of surgical and medical complications of TSA (reverse or anatomic) performed for PHFs versus OA in elderly patients at 1 and 3 years post-operatively. METHODS: The TriNetX Global Collaborative Network, comprising 183 million patients across 155 health

Total elbow arthroplasty for unreconstructible distal humerus fracture sequelae in patients less than 40 year old. Long-term outcomes.

BACKGROUND: Total elbow arthroplasty (TEA) is rarely recommended in young patients because of a high risk of early failure. This study analyzes the long-term outcomes of a cohort of post-traumatic TEA patients under 40 years of age who received a linked semiconstrained implant to manage the sequelae of an unreconstructible distal humerus fracture. MATERIAL AND METHODS: A review of the patients who received a Coonrad-Morrey prosthesis between 1993 and 2013 to manage the sequelae of a distal humerus fracture identified 16 individuals with a minimum follow-up of 10 years who at the time of surge

Angled BIO-RSA (bony-increased offset-reverse shoulder arthroplasty): a solution for the management of glenoid bone loss and erosion.

BACKGROUND: Glenoid deficiency and erosion (excessive retroversion/inclination) must be corrected in reverse shoulder arthroplasty (RSA) to avoid prosthetic notching or instability and to maximize function, range of motion, and prosthesis longevity. This study reports the results of RSA with an angled, autologous glenoid graft harvested from the humerus (angled BIO-RSA). METHODS: A trapezoidal bone graft, harvested from the humeral head and fixed with a long-post baseplate and screws, was used to compensate for residual glenoid bone loss/erosion. For simple to moderate (<25°) glenoid defects,

Antibiotic Spacers for Shoulder Periprosthetic Joint Infection: A Review.

Periprosthetic joint infection is a rare but potentially devastating complication of shoulder arthroplasty. The most conservative treatment approach is a two-stage revision involving interval placement of an antibiotic cement spacer. The purpose of this study was to contextualize the use of antibiotic spacers in the current treatment paradigm of shoulder periprosthetic joint infection and to review the history of shoulder spacers, the different types (eg, stemmed versus stemless and prefabricated versus handmade), the antibiotic composition and dosage, and their efficacy and complications.

Biomechanical analysis of conventional anchor revision after all-suture anchor pullout: a human cadaveric shoulder model.

HYPOTHESIS AND BACKGROUND: The possibility of implanting a conventional anchor at the pullout site following all-suture anchor failure was evaluated in a biomechanical cadaveric model. The hypothesis of the study was that anchor revision would yield equal biomechanical properties. METHODS: Ten human humeri were obtained, and bone density was determined via computed tomography. After all-suture anchor (n = 5) and conventional 4.5-mm anchor (n = 5) insertion, biomechanical testing was conducted. Following all-suture anchor pullout, a conventional 5.5-mm anchor was inserted at the exact site of

[The Extended Deltoid-Split Approach for Plating Four-Part Proximal Humeral Fractures].

The deltoideopectoral approach is established as the gold standard in the surgical treatment of proximal humeral fractures. As an alternative, we demonstrate the extended deltoid approach with an intraoperative video. A direct lateral incision is performed and the anterior parts of the axillary nerve are identified and preserved. In our experience, this approach allows improved visualisation of the greater tuberosity and easier positioning of locking plates. Clinically relevant neurological injuries cannot be seen in our patients or in the literature.

Preventing shoulder pain after cardiac rhythm management device implantation: a randomized, controlled study.

INTRODUCTION: Shoulder pain and disability ipsilateral to the implant site is a common complication of cardiac rhythm device implantation, yet very little has been published about this morbidity. We designed a study to assess the potential benefit of a simple exercise protocol in preventing shoulder pain postoperatively. METHODS AND RESULTS: Patients undergoing subcutaneous device implantation were randomized to one of two groups. The control group received standard instructions, whereas the exercise group was instructed on specific exercises aimed at strengthening or stretching the shoulder

Superior humeral dislocation. A complication following decompression and debridement for rotator cuff tears.

Very large rotator cuff tears may be surgically irreparable and under such circumstances debridement of the edges of the cuff and bursal decompression may relieve pain. In this paper, four cases are described of the development of superior migration of the humeral head following a debridement and bural decompression. In all four patients attempts were made to repair the rotator cuff. In each case the repair failed and the humeral head migrated. Two of the patients had the humeral head replaced because of a fracture; in these patients the prosthesis displaced upwards. This serious complication

Arthrodesis of the shoulder used in treating painful conditions.

By the time a patient's shoulder problem has reached the stage in which an arthrodesis is being considered, the patient usually is willing to accept the restraints of an arthrodesed shoulder. If there is a chance of relieving chronic pain and restoring some reasonable degree of function in the upper extremity, arthrodesis warrants consideration. However, failure to relieve pain and restore function may cause unhappiness. Because of recent advances in the surgical technique and the improved, more comfortable anatomic position of the arm at the side of the body, the patient is considering arthro

Impact of implant-abutment connection design on biological and mechanical outcomes in posterior single-tooth restorations: A randomized clinical trial.

STATEMENT OF PROBLEM: The implant-abutment interface has been identified as a key factor in preserving peri-implant marginal bone levels. However, current evidence remains insufficient to identify the most suitable connection design since most published studies fail to control critical variables such as the implant surface or design. PURPOSE: The purpose of this randomized clinical trial was to assess vertical marginal bone loss after 1 year of function of implants with identical macroscopic and microscopic designs and 3 different implant-abutment connections (external hexagon, internal hexag

Midterm follow-up of the Nexel total elbow arthroplasty.

BACKGROUND: Aseptic loosening is one of the most common complications of total elbow arthroplasty (TEA). Modern implants, such as the Nexel, have been designed in an attempt to decrease loosening. The present study aims to report implant survivorship, radiographic assessment of loosening and lucency, and patient-reported outcome measures (PROMs) in patients treated with the Nexel TEA at midterm follow-up. METHODS: Consecutive series of adult patients underwent TEA using the Nexel by a single surgeon via standardized technique. Patients with minimum 3-year follow-up with radiographic and PROM

Functional and clinical outcome of total shoulder arthroplasty with oversized glenoid.

BACKGROUND: The Epoca-Reconstruction-(Reco)®-Glenoid has been developed to treat patients with cuff-tear-arthropathy. The glenoid component of this system has a hemispheric shape that canopies the humeral head. This design is believed to provide a stable fulcrum and restore normal deltoid function. The purpose of this study was to analyse strengths and disadvantages of the Epoca-Reco®-Glenoid in cuff-tear-arthropathy patients. Changes in functional outcome using Constant-Murley-Scoring(CMS), CMS sub-scoring parameters and radiological outcome were analysed. For this purpose, a classification f

Bone density and anisotropy affect periprosthetic cement and bone stresses after anatomical glenoid replacement: A micro finite element analysis.

Glenoid loosening is still a main complication for shoulder arthroplasty. We hypothesize that cement and bone stresses potentially leading to fixation failure are related not only to glenohumeral conformity, fixation design or eccentric loading, but also to bone volume fraction, cortical thickness and degree of anisotropy in the glenoid. In this study, periprosthetic bone and cement stresses were computed with micro finite element models of the replaced glenoid depicting realistic bone microstructure. These models were used to quantify potential effects of bone microstructural parameters under

Influence of scapular tilt on radiographic assessment of the glenoid component after total shoulder arthroplasty: which radiographic landmarks are reliable?

BACKGROUND: Total shoulder arthroplasty has been shown to improve function and to reduce pain in cases of osteoarthritis. To assess loosening of the glenoid component, serial evaluation of frontal plane radiographs of the scapula has been established as the "gold standard." The aim of this study was to evaluate the reliability of different bone landmarks when the scapula is tilted compared with the ideal view. METHODS: Glenoid components were implanted into 6 human cadaveric scapulae. Radiographs were taken exactly anterior-posterior in the frontal plane as well as craniocaudal tilted (±15° a

Elucidating trends in revision reverse total shoulder arthroplasty procedures: a retrieval study evaluating clinical, radiographic, and functional outcomes data.

BACKGROUND: The purpose of this study was to explore relationships between damage modes in explanted reverse total shoulder arthroplasty (RTSA) components, patient and radiographic risk factors, and functional data to elucidate trends in RTSA failure. METHODS: Fifty RTSA systems, retrieved from 44 patients, with 50 polyethylene (PE) liners, 30 glenospheres, 21 glenoid baseplates, 13 modular humeral metaphases, and 17 humeral stems, were examined for damage modes, including abrasion, burnishing, dishing, embedding, scratching, and pitting. PE liners were also analyzed for delamination and edge

Evaluating trunnionosis in modular anatomic shoulder arthroplasties: a retrieval study.

BACKGROUND: Shoulder arthroplasty procedures are widely indicated, and the number of shoulder arthroplasty procedures has drastically increased over the years. Rapid expansion of the utilization of reverse total shoulder arthroplasty has outpaced the more modest growth of anatomic total shoulder arthroplasty (aTSA) while shoulder hemiarthroplasty (HA) has trended down. Recently, shoulder prostheses have transitioned to increasingly modular systems offering more individualized options with the potential for decreased pain and increased range of motion. However, increased primary procedures has
