The preventive effect of nedocromil or furosemide alone or in combination on exercise-induced asthma in children.

BACKGROUND: Recent evidence suggests that inhaled nedocromil and furosemide are effective in preventing asthma by ultrasonically nebulized distilled water, allergen, and exercise. There are, however, no studies that compare the effects of these two drugs. The aim of this study was to investigate the effect of inhaled furosemide (30 mg), nedocromil (4 mg), the combination of these two drugs, and placebo aerosol in preventing exercise-induced asthma. METHODS: Twenty-four children with exercise-induced asthma, aged 6 to 16 years, performed a treadmill test before and 20 minutes after a single do

Comparison of mannitol and methacholine to predict exercise-induced bronchoconstriction and a clinical diagnosis of asthma.

BACKGROUND: Asthma can be difficult to diagnose, but bronchial provocation with methacholine, exercise or mannitol is helpful when used to identify bronchial hyperresponsiveness (BHR), a key feature of the disease. The utility of these tests in subjects with signs and symptoms of asthma but without a clear diagnosis has not been investigated. We investigated the sensitivity and specificity of mannitol to identify exercise-induced bronchoconstriction (EIB) as a manifestation of BHR; compared this with methacholine; and compared the sensitivity and specificity of mannitol and methacholine for a

Severity of exercise-induced bronchoconstriction is related to airway eosinophilic inflammation in patients with asthma.

Exercise-induced bronchoconstriction (EIB) is widely prevalent in asthmatic patients. Eosinophilic airway inflammation is considered to be a major factor in the pathogenesis of asthma. However, the effects of eosinophilic airway inflammation on EIB have been elucidated insufficiently. To examine the relationship between the severity of EIB and eosinophilic inflammation, sputum induction and exercise challenge were performed in 21 asthmatic patients. Significantly higher percentages of eosinophils and levels of eosinophil cationic protein (ECP) were found in induced sputum in EIB-positive asthm

Exercise-induced bronchoconstriction in patients with bronchial asthma. Its prevention with an antihistaminic agent.

14 asthmatic patients developing regularly exercise-induced bronchoconstriction were subjected to a submaximal exercise on a bicycle ergometer. On the first day the exercise was not preceded by any medication; on the second, 50 mg thiazinamium was given, and on the third day 2 mg atropine was given before the exercise. The changes in the calibre of the bronchi were assessed with a Wright peak flow meter. With thiazinamium a complete protection against the bronchoconstriction was observed in 12 patients, in one the protection was partial and in an other no beneficial effect of the drug was foun

An Algorithm for Strategic Continuation or Restriction of Asthma Medication Prior to Exercise Challenge Testing in Childhood Exercise Induced Bronchoconstriction.

Exercise induced bronchial (EIB) constriction is a common and highly specific feature of pediatric asthma and should be diagnosed with an exercise challenge test (ECT). The impact of EIB in asthmatic children's daily lives is immense, considering the effects on both physical and psychosocial development. Monitoring childhood asthma by ECT's can provide insight into daily life disease burden and the control of asthma. Current guidelines for bronchoprovocation tests restrict both the use of reliever and maintenance asthma medication before an exercise challenge to prevent false-negative testing,

Wheat-Dependent Exercise-Induced Anaphylaxis Mimicking Acute Asthma in an Adolescent: A Case Report.

BACKGROUND: Wheat-dependent exercise-induced anaphylaxis (WDEIA) is an acute immediate hypersensitivity reaction triggered by exercise following the ingestion of wheat products, characterized by severe manifestations including urticaria, angioedema, dyspnea, hypotension, and even anaphylactic shock. Notably, WDEIA is rarely induced by wheat ingestion alone or exercise alone, and it is frequently misdiagnosed as asthma due to insufficient clinical awareness, leading to recurrent episodes. CASE PRESENTATION: We report an 18-year-old male patient who developed dyspnea, laryngeal tightness, gener

Screening elite winter athletes for exercise induced asthma: a comparison of three challenge methods.

Military sexual trauma (MST), an unfortunately common experience reported by U.S. service members and veterans, frequently leads to symptoms of posttraumatic stress disorder (PTSD) and other related conditions. However, little is known about how contextual features of MST correlate with specific clinical phenotypes and symptom presentations. The current study examined correlations between contextual factors of MST and cumulative interpersonal trauma history with diverse clinical outcomes, including PTSD symptom clusters, depressive symptoms, worry, and posttraumatic cognitions in a sizeable tr

Respiratory heat/water loss alone does not determine the severity of exercise-induced asthma.

Respiratory heat loss (RHL) or water loss (RWL) have been proposed as possible triggering factors in exercise and hyperventilation-induced asthma (EIA and HIA). It has recently been demonstrated that exercise intensity and climatic factors are both important in determining the severity of EIA. Eight young asthmatics performed both exercise and isocapnic hyperventilation (IHV) manoeuvres under identical climatic conditions, as part of our investigation of these interactive factors which determine the severity of the asthmatic response. It was found that, when challenged at low ventilatory level

Acid reflux into the oesophagus does not influence exercise-induced airway narrowing in bronchial asthma.

OBJECTIVES: A few studies on small patient series have investigated the relationship between gastroesophageal reflux and bronchial responsiveness as expressed by exercise-induced bronchoconstriction (EIB), with non-conclusive results. The aim of this study was to evaluate whether the presence of acid in the oesophagus may influence EIB. METHODS: 45 patients with bronchial asthma underwent spirometry, exercise challenge on bicycle ergometer and 24 h oesophageal pH monitoring. Subjects with EIB (Forced expiratory volume in the first second (FEV1)) percentage decrease after exercise (DeltaFEV1)

The effect of alpha adrenergic blockade on non-specific airways reactivity and exercise induced asthma.

In asthmatics, alpha-adrenergic blockage with oral phentolamine partially or completely blocked bronchospasm after maximal exercise challenge. Bronchial reactivity to isocapnic hyperventilation with conditioned cold air was also reduced. However, non-specific airways reactivity to inhaled histamine did not change. Therefore, exercise and cold induced bronchospasm in asthma may be partially mediated by alpha adrenergic neural mechanisms perhaps through sensitization of alpha receptors during airway cooling.

Cut-off values to evaluate exercise-induced asthma in eucapnic voluntary hyperventilation test for children.

BACKGROUND AND AIM: The eucapnic voluntary hyperventilation (EVH) testing is a diagnostic tool for diagnostics of exercise-induced bronchoconstriction; while the testing has become more common among children, data on the test's feasibility among children remain limited. Our aim was to investigate EVH testing feasibility among children, diagnostic testing cut-off values, and which factors affect testing outcomes. METHODS: We recruited 134 patients aged 10-16 years with a history of exercise-induced dyspnoea and 100 healthy control children to undergo 6-min EVH testing. Testing feasibility was

Comparison of PY 108-068, a new calcium antagonist, with nifedipine in exercise-induced asthma.

Several calcium antagonists, each with significantly different chemical structures, have demonstrated variable attenuation of exercise-induced asthma. Quantitative comparisons have been hampered by differences in the intensity of challenge and the severity of the underlying disease between groups of patients. In 12 asthmatic adults with relatively severe exercise-induced asthma, we compared the effect of a new calcium antagonist, PY 108-068, in doses of 75 mg and 150 mg with nifedipine (30 mg) and placebo on resting flow rates and flow rates after exercise. Over a three-week period, each patie

Comparative efficacy of inhaled furosemide and disodium cromoglycate in the treatment of exercise-induced asthma in children.

BACKGROUND: Inhaled furosemide has been shown, in patients with asthma, to have prophylactic properties similar to those of disodium cromoglycate. OBJECTIVE: The aim of this study was to evaluate the protective effect of these drugs in the treatment of exercise-induced asthma. METHODS: Fifteen children with exercise-induced asthma (mean age, 10.8 years) underwent exercise challenge after a single dose of nebulized placebo, furosemide (20 mg/m7 body area), or disodium cromoglycate (20 mg), 2 days apart, in a single-blind crossover trial. RESULTS: Both furosemide and disodium cromoglycate exe

Mid-expiratory flow versus FEV1 measurements in the diagnosis of exercise induced asthma in elite athletes.

BACKGROUND: A fall in FEV(1) of > or =10% following bronchoprovocation (eucapnic voluntary hyperventilation (EVH) or exercise) is regarded as the gold standard criterion for diagnosing exercise induced asthma (EIA) in athletes. Previous studies have suggested that mid-expiratory flow (FEF(50)) might be used to supplement FEV(1) to improve the sensitivity and specificity of the diagnosis. A study was undertaken to investigate the response of FEF(50) following EVH or exercise challenges in elite athletes as an adjunct to FEV(1). METHODS: Sixty six male (36 asthmatic, 30 non-asthmatic) and 50 fe

Fractional exhaled nitric oxide (FeNO) may predict exercise-induced bronchoconstriction (EIB) in schoolchildren with atopic asthma.

BACKGROUND: There is a need for the performance of exercise-induced bronchoconstriction (EIB) tests in the monitoring of childhood asthma control. We aimed to evaluate whether in children with atopic asthma, EIB can be predicted by one or more of the following parameters or by their combination: fractional exhaled nitric-oxide (FeNO), allergy profile, asthma treatment, total IgE serum concentration and eosinophil blood count (EBC). METHODS: It was a retrospective, cross-sectional study. We evaluated data from medical documentation of children with atopic asthma who had performed standardized

Vocal cord dysfunction masquerading as exercise-induced asthma. a physiologic cause for "choking" during athletic activities.

Exercise-induced bronchospasm is a common clinical problem that is particularly troubling for patients who engage in strenuous physical activity, such as athletes. When such individuals develop this condition, the associated airway narrowing can materially interfere with performance; however, events other than asthma can also produce these symptoms and their differentiation is essential for proper treatment. The present report describes seven elite athletes with psychogenic vocal cord dysfunction who presented with acute dyspnea during sporting competitions. Although the combination of exertio

Relation between efficacy of sodium cromoglycate and baseline lung function in exercise- and hyperventilation-induced asthma.

The protective effect of sodium cromoglycate (SCG) against exercise- and hyperventilation-induced asthma with respect to basal lung function was investigated in young asthmatics. The subjects performed standardized exercise or isocapnic hyperventilation challenge tests breathing cold dry air; in each case the effect of SCG was compared with that of a placebo in a double-blind fashion. With exercise as the challenge in 24 subjects, there was a strong positive correlation between the protective effect of SCG and the basal level of lung function.
